The importance of comprehensive geriatric assessment in predicting the outcome of patients with proximal humerus fractures.

BACKGROUND: Proximal humeral fractures (PHF) are common in geriatric patients. Existing frailty scores insufficiently capture the multidimensional vulnerability influencing long-term outcomes. AIMS: This study investigates if geriatric-typical characteristic complexes (GTMK), incorporating a broader range of geriatric domains, can improve prediction of long-term outcomes following PHF. METHODS: In this retrospective cohort study, patients aged ≥ 65 years with PHF (ICD-10 S42.2) were identified from 2011 to 2022 using nationwide inpatient and outpatient claims data from the German BARMER health…

An Unusual Presentation of Multiple Myeloma: A 71-Year-Old Female With a Single Lytic Lesion of Her Appendicular Skeleton.

Multiple myeloma is a devastating illness with a hallmark of end-organ damage. The clinical presentation of multiple myeloma often includes the involvement of CRAB (hypercalcemia, renal failure, anemia, bone lesions) symptoms. We present a case of a patient who did not exhibit the typical presentation of multiple myeloma making her case unique and her diagnosis more difficult. In addition to the CRAB criteria, typical symptomatology includes constipation, pain, fatigue, and peripheral sensory issues. The purpose of this case report is to bring awareness to both multiple myeloma and this partic

Chronic complex persistent elbow instability: a consecutive and prospective case series and review of recent literature.

BACKGROUND: Chronic complex persistent elbow instability (CCPEI) is a condition that even expert elbow surgeons find challenging to treat. The results of the few studies that have dealt with the treatment of this condition are conflicting. We describe the surgical results of a consecutive prospective series of patients with CCPEI and provide a review of the recent literature. METHODS: We assessed 21 patients with previous failed surgical or conservative treatment, with a terrible-triad injury in 13, Monteggia-like lesion in 6, humeral shear fracture-dislocation in 1, and radial head fracture-

A panorama of radial nerve pathologies- an imaging diagnosis: a step ahead.

The radial nerve has a long and tortuous course in the upper limb. Injury to the nerve can occur due to a multitude of causes at many potential sites along its course. The most common site of involvement is in the proximal forearm affecting the posterior interosseous branch while the main branch of the radial nerve is injured in fractures of the humeral shaft. Signs and symptoms of radial neuropathy depend upon the site of injury. Injury to the nerve distal to innervation of triceps brachii results in loss of extensor function with sparing of function of the triceps resulting in the characteri

Editorial Commentary: Both 3-Dimensional Magnetic Resonance Imaging and Computed Tomography Are Valuable for Determination of Glenoid and Humeral Bone Loss in Patients With On- and Off-Track Shoulder Instability.

Improving the modalities for advanced glenohumeral joint imaging has been an important area to address in the field of orthopaedic surgery. The current gold standard for imaging glenoid and humeral bone loss in patients with shoulder instability, 3-dimensional (3D) computed tomography (CT), provides high-quality 3D images of bones but comes with a cost of extra time, additional imaging because of the need for an additional magnetic resonance imaging (MRI) scan, and exposure to radiation. Three-dimensional MRI is a promising solution that can produce high-contrast images depicting both bony str

Intrathoracic shoulder dislocation causing rupture of the right main bronchus.

An intrathoracic shoulder dislocation is a rare injury, usually the result of high-energy trauma [Hawkes et al. in Am J Orthop 43(4):E74-E78, 2014; Tsai et al. in Ann Thorac Cardiovasc Surg 20:592-594, 2014, in Rupprecht et al. Bull Emerg Trauma 5(3):212-214, 2017; Abellan et al. J Orthop Surg (Hong Kong)18(2):254-257, 2010]. It often occurs in conjunction with thoracic, pelvic, and long bone injuries. In addition, there is often significant injuries to the shoulder girdle and chest wall associated with neurovascular compromise [Abellan et al. J Orthop Surg (Hong Kong)18(2):254-257, 2010; Lin

Patient characteristics are more important than preoperative radiographic factors in predicting the risk of acromial stress fracture following reverse total shoulder arthroplasty.

BACKGROUND: Acromial stress fractures (ASFs) after reverse total shoulder arthroplasty (rTSA) can have a devastating effect on a patient's overall function, causing decreased range of motion and patient satisfaction. The purpose of this study was to evaluate preoperative patient demographics and radiographic findings associated with risk for ASF after rTSA. METHODS: A retrospective review was performed on patients who underwent primary rTSA by a single surgeon between December 2011 and December 2022. Inclusion criteria were primary rTSA with minimum 6-month postoperative radiographs. Preopera

Revision of total shoulder arthroplasty to hemiarthroplasty: results at mean 5-year follow-up.

BACKGROUND: Although there is a trend to manage failed anatomic total shoulder arthroplasties (aTSA) with revision to a reverse total shoulder arthroplasty, such revisions can be complicated by difficulties in baseplate fixation, instability, and acromial stress fractures. Some cases of failed aTSA may be safely revised to a hemiarthroplasty (HA). The objectives of this study were to report patient-reported outcomes after conversion from aTSA to HA and assess patient and shoulder characteristics associated with a successful outcome. METHODS: Patients who underwent revision from aTSA to HA bet

Deltoid Fasciocutaneous Free Flap: Cadaveric Study and Clinical Outcomes of Treating Open Infected Limb Fracture with Soft Tissue Defect.

BACKGROUND: The deltoid fasciocutaneous free flap is a valuable reconstructive option for tissue defects following injury or tumor resection. However, limited studies have provided objective data on the anatomical characteristics of this flap. This study aimed to investigate the cadaveric anatomical characteristics of this flap and report our initial results for treating open infected fracture-related infection (FRI) with skin defects in the extremities. METHODS: This prospective cross-sectional study involved the dissection of 27 adult cadavers (13 male and 14 female), including 16 formalin-

The long-term results of shoulder hemiarthroplasty in irreducible four-part fracture-dislocation of the proximal humerus without rotator cuff tear arthropathy.

PURPOSE: Although there are concerns about nonunion of the greater tuberosity or shoulder weakness, hemishoulder arthroplasty (HSA) is a viable treatment option for irreducible four-part proximal humerus fractures-dislocation without rotator cuff tear arthropathy. This study aims to analyze the long-term radiological and functional outcomes of HSA. METHODS: This study enrolled 36 patients who underwent HSA due to irreducible four-part proximal humerus fracture-dislocation without rotator cuff tear arthropathy between March 2005 and May 2020. The exclusion group included 10 reverse total shoul

TRPV6 compound heterozygous variants result in impaired placental calcium transport and severe undermineralization and dysplasia of the fetal skeleton.

Transient receptor potential vanilloid 6 (TRPV6) functions in tetramer form for calcium transport. Until now, TRPV6 has not been linked with skeletal development disorders. An infant with antenatal onset thoracic insufficiency required significant ventilatory support. Skeletal survey showed generalized marked undermineralization, hypoplastic fractured ribs, metaphyseal fractures, and extensive periosteal reaction along femoral, tibial, and humeral diaphyses. Parathyroid hormone (PTH) elevation (53.4-101 pmol/L) initially suggested PTH signaling disorders. Progressively, biochemical normalizati

Implant stability is affected by local bone microstructural quality.

It is known that low bone quality, caused for instance by osteoporosis, not only increases the risk of fractures, but also decreases the performance of fracture implants; yet the specific mechanisms behind this phenomenon are still largely unknown. We hypothesized that especially peri-implant bone microstructure affects implant stability in trabecular bone, to a greater degree than more distant bone. To test this hypothesis we performed a computational study on implant stability in trabecular bone. Twelve humeral heads were measured using micro-computed tomography. Screws were inserted digital

Assessing nonunion and morbidity in pediatric clavicula pro humero reconstruction with vascularized fibular grafts.

The proximal humerus is a common site for pediatric malignant bone tumors, often necessitating complex surgical approaches when the tumor extends into the glenohumeral joint. Limb-salvage techniques like clavicula pro humero (CPH) reconstruction aim to balance limb function with oncological safety. However, nonunion and functional limitations remain significant challenges. This study evaluates whether adding a vascularized fibular graft (VFG) reduces nonunion in CPH reconstructions following proximal humeral tumor resection. A retrospective review of six pediatric patients (mean age 12 years)

Open stent graft repair with upper-half Sternotomy for blunt thoracic aortic injury: a case report.

BACKGROUND: Thoracic endovascular aortic repair is now widely applied to the treatment of blunt aortic injury. However, its long-term outcomes remain unclear. Endoleakage and migration might occur in the long term, especially when younger patients undergo endovascular aortic repair. In open stent grafting, the proximal end of the open stent graft is directly sutured to the native aorta, which may reduce the risk of endoleakage and migration. We applied open stent grafting to the treatment of blunt aortic injury in the subacute phase and herein report the patient's clinical course. CASE PRESEN
