Inhibition of daptomycin by pulmonary surfactant: in vitro modeling and clinical impact.

The lipopeptide daptomycin has been approved for use in skin and skin-structure infections but has failed to meet statistical noninferiority criteria in a clinical trial for severe community-acquired pneumonia. Daptomycin exhibited an unusual pattern of activity in pulmonary animal models: efficacy in Staphylococcus aureus hematogenous pneumonia and inhalation anthrax but no activity against Streptococcus pneumoniae in simple bronchial-alveolar pneumonia. Daptomycin was shown to interact in vitro with pulmonary surfactant, resulting in inhibition of antibacterial activity. This effect was spec

Speed heterogeneity and non-Gaussian transport in active particles: a survival-hazard framework.

Non-Gaussian displacement statistics in active-particle systems are often associated with heterogeneity in particle motility. In this work, we develop an estimation-based statistical framework that links speed distributions inferred from experimental data to effective-diffusivity and displacement statistics in active Brownian particles (ABPs) and active particles with passive noise (APPs). Rayleigh, Weibull, and generalized gamma distributions are fitted by maximum likelihood to a sample of 53 representative speeds extracted from experimental cell trajectories, and the candidate models are com

Assessing atrophy measurement techniques in dementia: Results from the MIRIAD atrophy challenge.

Structural MRI is widely used for investigating brain atrophy in many neurodegenerative disorders, with several research groups developing and publishing techniques to provide quantitative assessments of this longitudinal change. Often techniques are compared through computation of required sample size estimates for future clinical trials. However interpretation of such comparisons is rendered complex because, despite using the same publicly available cohorts, the various techniques have been assessed with different data exclusions and different statistical analysis models. We created the MIRI

Nonlinear identification of the total baroreflex arc.

The total baroreflex arc [the open-loop system relating carotid sinus pressure (CSP) to arterial pressure (AP)] is known to exhibit nonlinear behaviors. However, few studies have quantitatively characterized its nonlinear dynamics. The aim of this study was to develop a nonlinear model of the sympathetically mediated total arc without assuming any model form. Normal rats were studied under anesthesia. The vagal and aortic depressor nerves were sectioned, the carotid sinus regions were isolated and attached to a servo-controlled piston pump, and the AP and sympathetic nerve activity (SNA) were

A nationwide spatiotemporal forecasting model for bacterial gastroenteritis surveillance in South Korea.

BACKGROUND: Bacterial gastroenteritis is a public health challenge in South Korea, largely driven by foodborne pathogens including Salmonella, Vibrio, enteropathogenic Escherichia coli (EPEC), and Campylobacter. While national surveillance systems provide valuable epidemiological data, they primarily rely on retrospective reporting and have limited capacity to anticipate future outbreaks. This limitation highlights the need for predictive approaches that can support timely public health interventions. METHODS: In this study, we constructed a two-stage probabilistic forecasting framework that

Externally Validated Probabilistic Modeling of a Predefined Entecavir Resistance Pathway in HBV Using Independent Public Repositories.

BACKGROUND: Accurate interpretation of hepatitis B virus (HBV) polymerase sequences is essential for identifying antiviral resistance, particularly for high-genetic-barrier agents such as entecavir. Current resistance interpretation relies largely on deterministic rule-based systems that do not quantify uncertainty and are difficult to evaluate across independent datasets. We aimed to develop and externally validate a transparent probabilistic framework for reconstructing a predefined entecavir resistance pathway from HBV polymerase sequences. METHODS: HBV polymerase sequences were retrieved

Generalized Multilevel Multisource Functional Regression With an Application to Alzheimer's Disease.

With population aging, Alzheimer's disease has gained increasing attention. Advances in medical technology and longitudinal studies such as the Alzheimer's Disease Neuroimaging Initiative (ADNI) have generated diverse follow-up data, which can be viewed as multilevel multisource functional data for understanding disease progression. In this paper, we propose a new estimation method for the multilevel multisource functional principal components. This method first applies the multilevel functional principal component analysis on the univariate multilevel functional data, then obtains the estimat

A Latent Variable Approach for Causal Effect Estimation Under Misclassified Treatment Assignment.

Misclassification in treatment assignment is a common issue in causal inference with observational studies, often leading to biased estimates of causal effects if unaddressed. Several methods have been developed to handle this issue by making use of a validation dataset. This paper proposes a robust latent variable approach for causal effect estimation without the need of validation data. By employing a potential outcome modeling framework that incorporates true treatment assignment as a latent variable, we construct a likelihood function that involves three models: the outcome model, the meas

Improving Assessment of Vaccine Effectiveness by Coupling Test-negative Design Studies with Survival Models.

The test-negative design (TND) has become a widely used observational study design for evaluating vaccine effectiveness, especially during the COVID-19 pandemic. Traditionally, TND has been viewed as a variant of the case-control study and largely limited to use with logistic regression models. In this paper, we first establish that TND can be framed as a special case of a cohort study, thereby opening the door to a wider range of analytical approaches. We then introduce the Prentice, Williams, and Peterson gap-time (PWP-GT) frailty model as a novel method for analyzing TND data, accounting fo

Interrupted Time Series Methods for Nonrandom Sampling Study Designs With Known Sampling Weights.

Including participants from diverse neighborhoods in research may improve generalizability of study results to broader populations. The University of California Irvine Consent-to-Contact (C2C) Registry recently initiated a novel Recruitment Strategy Study (named the C2C-RSS) to assess the effectiveness of interventions (Facebook versus traditionally mailed postcard advertisements) that aimed at recruiting individuals from more disadvantaged neighborhoods, as defined by high Area Deprivation Index (ADI) in Orange County, California. The C2C-RSS utilized a nonrandom sampling design to ensure uni

Heterogeneous causal mediation analysis using Bayesian additive regression trees.

Causal mediation analysis provides insights into the mechanisms through which treatments affect outcomes. While mediation effects often vary across individuals, most existing methods focus solely on population-average effects, overlooking individual-level heterogeneity. To address this limitation, we propose a Bayesian regression tree ensemble method that flexibly models nonlinear relationships and captures treatment-by-mediator interactions in the mediation process. Using hierarchical posterior sampling, our approach provides credible intervals with nominal coverage rates for inferring hetero

Joint Models of Two Longitudinal Biomarkers and Clustered Survival Data for Cluster-Informed Dynamic Prediction With Application to Periodontitis.

Joint modeling of longitudinal data and time-to-event data have been extended to accommodate multilevel data structures. In dental studies, data often exhibit a multilevel hierarchy: each patient has multiple teeth, and one or more biomarkers are measured repeatedly over time for each tooth. In addition to biomarker measurements, the time to tooth loss may vary differently between patients as some patients are more susceptible to tooth loss, conditional on other risk factors. In this paper, we account for intra-patient and intra-tooth correlations in the longitudinal measurement of a continuou

A flexible rank-based framework for individualized treatment selection with mixed-type multivariate outcomes.

Personalized treatment selection is often based on a single primary endpoint, even though complex diseases are typically monitored using multiple, heterogeneous outcomes. We develop a rank-based framework for individualized treatment selection that accommodates mixed collections of continuous, ordinal, binary, and right-censored survival outcomes. For each outcome and treatment, we summarize the conditional outcome distribution given patient covariates using clinically meaningful parameters, including conditional quantiles, survival probabilities, and tail probabilities at ordered thresholds,

Enhancing detection of polygenic adaptation: a comparative study of machine learning and statistical approaches using simulated evolve-and-resequence data.

BACKGROUND: Detecting signals of polygenic adaptation remains a significant challenge in population genomics, as traditional methods often struggle to identify the associated subtle, multi-locus allele-frequency shifts. Here, we introduced and tested several novel approaches combining machine learning techniques with traditional statistical tests to detect polygenic adaptation patterns in time-series of allele frequency changes from whole genome data. We implemented a Naive Bayesian Classifier (NBC) and One-Class Support Vector Machines (OCSVM), and compared their performance against the class

A Bayesian Hierarchical Mixture Cure Modelling Framework to Utilize Multiple Survival Datasets for Long-Term Survivorship Estimates: A Case Study From Previously Untreated Metastatic Melanoma.

Time to an event of interest over a lifetime is a central measure of the clinical benefit of an intervention used in a health technology assessment (HTA). Within the same trial, multiple end-points may also be considered. For example, overall and progression-free survival time for different drugs in oncology studies. A common challenge is when an intervention is only effective for some proportion of the population who are not clinically identifiable. Therefore, latent group membership as well as separate survival models for identified groups need to be estimated. However, follow-up in trials m

Assessment of Wiener Process Degradation Models With Application to Amyotrophic Lateral Sclerosis Decline.

Degradation models are commonly used in engineering to analyze the deterioration of systems over time. These models offer an alternative to standard longitudinal methods as they explicitly account for within-subject temporal variability through a latent stochastic process, allowing random fluctuations within a patient to be captured. This work investigates Wiener process-based degradation models with linear drift (i.e., slope) while considering a diffusion term to represent within-subject temporal variability, a random-effects term to capture between-subject variability of the slope, and a tim

A Joint Model for (Un)Bounded Longitudinal Markers, Competing Risks, and Recurrent Events Using Patient Registry Data.

Joint models for longitudinal and survival data have become a popular framework for studying the association between repeatedly measured biomarkers and clinical events. Nevertheless, addressing complex survival data structures, especially handling both recurrent and competing event times within a single model, remains a challenge. This causes important information to be disregarded. Moreover, existing frameworks rely on a Gaussian distribution for continuous markers, which may be unsuitable for bounded biomarkers, resulting in biased estimates of associations. To address these limitations, we

Evaluating longitudinal treatment effects for Duchenne muscular dystrophy using dynamically enriched Bayesian small sample, sequential, multiple assignment randomized trial (snSMART).

For progressive rare diseases like Duchenne muscular dystrophy (DMD), evaluating disease burden by measuring the totality of evidence from outcome data over time per patient can be highly informative, especially regarding how a new treatment impacts disease progression and functional outcomes. This paper focuses on new statistical approaches for analyzing data generated over time in a small sample, sequential, multiple assignment, randomized trial (snSMART), with an application to DMD. In addition, the use of external control data can enhance the statistical and operational efficiency in rare

Estimating the risk of cancer with and without a screening history.

A probability method to estimate cancer risk for asymptomatic individuals for the rest of life was developed based on one's current age and screening history using the disease progressive model. The risk is a function of the transition probability density from the disease-free to the preclinical state, the sojourn time in the preclinical state and the screening sensitivity if one had a screening history with negative results. The method can be applied to any chronic disease. As an example, the method was applied to estimate women's breast cancer risk using parameters estimated from the Health

Frequentist Grouped Weighted Quantile Sum Regression for Correlated Chemical Mixtures.

As individuals are exposed to a myriad of potentially harmful pollutants every day, it is important to determine which actors have the greatest influence on health outcomes. However, jointly modeling the associations of multiple pollutant exposures is often hindered by the presence of highly correlated chemicals originating from a common source. A popular approach to analyzing associations between a disease outcome and several highly correlated exposures is Weighted Quantile Sum Regression (WQSR) modeling. WQSR provides increased stability in estimating model parameters but requires data split
