Channelrhodopsin fluorescent tag replacement for clinical translation of optogenetic hearing restoration.

Sensory restoration by optogenetic neurostimulation provides a promising future alternative to current electrical stimulation approaches. So far, channelrhodopsins (ChRs) typically contain a C-terminal fluorescent protein (FP) tag for visualization that potentially poses an additional risk for clinical translation. Previous work indicated a reduction of optogenetic stimulation efficacy upon FP removal. Here, we further optimized the fast-gating, red-light-activated ChR f-Chrimson to achieve efficient optogenetic stimulation in the absence of the C-terminal FP. Upon FP removal, we observed a ma

The role of oxidative stress in the susceptibility of noise-impaired cochleae to synaptic loss induced by intracochlear electrical stimulation.

Intracochlear electrical stimulation (ES) generated by cochlear implants (CIs) is used to activate auditory nerves to restore hearing perception in deaf subjects and those with residual hearing who use electroacoustic stimulation (EAS) technology. Approximately 1/3 of EAS recipients experience loss of residual hearing a few months after ES activation, but the underlying mechanism is unknown. Clinical evidence indicates that the loss is related to the previous history of noise-induced hearing loss (NIHL). In this report, we investigated the impact of intracochlear ES on oxidative stress levels

Status of early hearing detection and intervention in South Korea: a nationwide population-based study of national infant health checkup.

The aim of this study was to evaluate the status of early hearing detection and intervention after newborn hearing screening (NHS) in South Korea. A retrospective review of Korean national health insurance service data of all infants receiving the 4-month old national infant health checkup between 2010 and 2016 from a nationwide population-based database was conducted. Based on the results of the NHS-administered hearing questionnaires as part of the national infant health checkup, individuals were classified into "pass" (1,730,615 infants) or "refer" (10,941 infants) groups. Next, an analysis

Increasing the expression level of ChR2 enhances the optogenetic excitability of cochlear neurons.

Optogenetics comprise a promising alternative to electrical stimulation for characterization of neural circuits and for the next generation of neural prostheses. Optogenetic stimulation relies on expression of photosensitive microbial proteins in animal cells to initiate a flow of ions into the cells in response to visible light. Here, we generated a novel transgenic mouse model in which we studied the optogenetic activation of spiral ganglion neurons, the primary afferent neurons of the auditory system, and showed a strong optogenetic response, with a similar amplitude as the acoustically evo

Laser therapy for hair removal on grafts and flaps.

The diversity and utility of laser procedures have increased over the recent years and nowadays, applications for medical and cosmetic reasons have increased considerably. Problematic intraoral and cutaneous hirsutisms have been described as a consequence of complex reconstruction usually after oncology surgery. We present three patients in whom hair removal laser was performed on grafts and flaps in different compromised anatomical areas: oral cavity, penis, and auricular pavilion. All three patients were men; in two of them the hairy graft was a consequence after oncologic surgery reconstruc

The combination of vestibular impairment and congenital sensorineural hearing loss predisposes patients to ocular anomalies, including Usher syndrome.

The co-occurrence of hearing impairment and visual dysfunction is devastating. Most deaf-blind etiologies are genetically determined, the commonest being Usher syndrome (USH). While studies of the congenitally deaf population reveal a variable degree of visual problems, there are no effective ophthalmic screening guidelines. We hypothesized that children with congenital sensorineural hearing loss (SNHL) and vestibular impairment were at an increased risk of having USH. A retrospective chart review of 33 cochlear implants recipients for severe to profound SNHL and measured vestibular dysfunctio

On the balance of envelope and temporal fine structure in the encoding of speech in the early auditory system.

There is much debate on how the spectrotemporal modulations of speech (or its spectrogram) are encoded in the responses of the auditory nerve, and whether speech intelligibility is best conveyed via the "envelope" (E) or "temporal fine-structure" (TFS) of the neural responses. Wide use of vocoders to resolve this question has commonly assumed that manipulating the amplitude-modulation and frequency-modulation components of the vocoded signal alters the relative importance of E or TFS encoding on the nerve, thus facilitating assessment of their relative importance to intelligibility. Here we ar

Cortical oscillations predict auditory grouping in listeners with various hearing configurations.

Auditory grouping relies on the ability to bind tones with coherent spectral features over time to form auditory objects. Sensorineural hearing loss (SNHL) degrades spectral resolution, and the extent of this degradation varies with the hearing configuration. However, it remains unclear how SNHL impacts auditory grouping and whether different hearing configurations affect this ability. This study investigated task performance and cortical activity during auditory object detection in four groups with different hearing configurations: Twenty normal-hearing (NH) listeners, seventeen bilateral hea

[Management of vestibular schwannomas].

Vestibular schwannomas are benign neoplasms originating from the Schwann cells of the vestibular vestibular nerve of the vestibulocochlear nerve, and rarely from the pars cochlearis. These are tumors that are in contact with the nerve but do not bind the fibers. Benign neoplasms of the Schwann cells of the auditory and equilibrium nerves can also occur primarily in the inner ear and are referred to as intralabyrinthine schwannomas (ILS). Vestibular schwannomas represent 6-7 % of all intracranial and 90 % of cerebellopontine angle tumors. Bilateral occurrence occurs in < 5 % of cases, and then

The relationship between language development and behaviour problems in children with hearing loss.

BACKGROUND: There are well-replicated findings that link poor development on a range of communication skills with increased behavioural problems. This paper examines this relationship in children with hearing loss. METHOD: One hundred and twenty children with hearing loss (67 boys, 53 girls) and 63 hearing children (37 boys, 26 girls) with a mean age of 8 years from eight districts in Southern England were assessed for receptive and expressive language skills. The relationships between these measures and an aggregate of parent- and teacher-reported behaviour problems in the children were inve

Dysmorphism of the middle ear: case report.

Although there are numerous publications in the literature describing the wide range of diagnosis, classifications and treatment of malformations of the hearing apparatus, even more variations can be found in clinical practice. Indeed, each individual case is unique as far as concerns pathogenesis, clinical course and treatment. The case reported herein describes a 12-year-old boy affected by cranio-facial dysmorphism and monolateral conductive hearing loss in the right ear: followed from radiological diagnosis--carried out to study a malformation of the ear pinna--to surgical treatment.

Bipolar recording of the cochlear nerve action potentials during cerebellopontine angle surgery.

A technique of bipolar recording of the cochlear nerve compound action potentials (CAPs) to make identification and preservation of the cochlear nerve easier during cerebellopontine angle surgery is described. Four patients, aged 32 to 65 years, with acoustic tumors smaller than 2 mm and serviceable hearing, participated in the study. To investigate the resolution power of bipolar CAP recordings, a pilot study had been performed in a group of four patients submitted to vestibular neurectomy. Two silver electrodes insulated by Teflon up to the exposed ends were utilized for bipolar recording of

Small Molecule Therapeutic Approaches With a Greater Focus on Hearing Loss: A Review.

BACKGROUND AND AIMS: Hearing loss (HL) is one of the most common sensory impairments worldwide and has substantial effects on quality of life and daily functioning. Although cochlear implants and hearing aids have markedly improved the management of hearing impairment, these devices do not directly address the underlying pathogenic mechanisms of HL. This review aims to summarize the pathophysiology and classification of HL and to discuss the potential therapeutic applications of small molecules (SMs) in its management. METHODS: This review focuses on the major pathological mechanisms involved
