Retrieval of Broken Guidewire from the Hip Joint Protruding inside the Pelvic Cavity: A Rare Case Report.

Hardware breakage in the form of guide wire or drill bit is a devastating complication particularly if occurs around hip joint during cephalo-medullary nailing. It should be removed on urgent basis as it can migrate to the pelvic cavity and damages adjacent neuro-vascular bundle and visceral organ immediately as well joint arthritis later on. There are very few cases report available in the literature with retrieval techniques by using disc forceps, pituitary forceps, oversized reamers, arthrotomy with joint dislocation and through ilioinguinal approaches occasionally. We are presenting an int

Ilizarov fixator as salvage procedure after frustrating arthrodesis using intramedullary nailing - is there a chance of consolidation?

INTRODUCTION: Arthrodesis of the tibiotalar and subtalar joints is a salvage procedure that has been used successfully for years. Treatment options include internal procedures and external procedures. Retrograde intramedullary nailing is considered a safe procedure with a high degree of stability and comfort. Nevertheless, there are cases in which this internal arthrodesis fails and another procedure must be considered. Ilizarov fixator treatment could be a solution for those patients in whom intramedullary nailing has failed. Even if it means another surgical revision - is it possible to fina

Bilateral total hip replacement in osteogenesis imperfecta with hyperplastic callus.

Osteogenesis imperfecta is a rare inherited disorder of connective tissue which may present with recurrent fractures which are prone to nonunion and malunion resulting in deformity. Some patients develop osteoarthritis of the hip. Formation of hyperplastic callus after recurrent fractures may deform the shape of the femur and preclude the use of standard implants at joint replacement. Replacement can thus be technically demanding. We present a case of bilateral hip replacement in a patient with osteogenesis imperfecta and hyperplastic callus which was treated by the use of long femoral allogra

Narrative Review on the Management of Neck of Femur Fractures in People Living with HIV: Challenges, Complications, and Long-Term Outcomes.

Neck of femur (NOF) fractures are a critical orthopaedic emergency with a high morbidity and mortality prevalence, particularly in people living with Human Immunodeficiency Virus (PLWHIV). A combination of HIV infection, combined antiretroviral therapy (cART), and compromised bone health further increases the risk of fragility fractures. Additionally, HIV-related immune dysfunction, cART-induced osteoporosis, and perioperative infection risks further pose challenges in ongoing surgical management. Despite the rising global prevalence of PLWHIV, no specific guidelines exist for the perioperativ

An anatomic classification for heterotopic ossification about the hip.

STUDY DESIGN: Retrospective cohort. SUMMARY OF BACKGROUND DATA: Heterotopic ossification (HO) about the hip is a debilitating condition that can occur after fixation for acetabular fractures, total hip replacement, or polytrauma with closed head injuries. No classification exists that informs surgical treatment. PURPOSE: To establish a classification system for HO about the hip by reviewing a consecutive series of HO at a single institution. It was hypothesized that HO about the hip could be grouped into a novel classification scheme based upon the location and involved structures of the hip

Use of elastic stable intramedullary nailing for treating unstable forearm fractures in children.

BACKGROUND: The majority of the pediatric diaphyseal forearm fractures are treated with closed reduction and the application of an above elbow cast for a varying period, depending on the age of the child. Some forearm diaphyseal fractures require osteosynthesis mainly because of the presence of an unacceptable angular deformity, fracture instability, presence of an open injury or failure of conservative management in the form of redisplacement in the cast after manipulation. METHODS: We present our experience with elastic stable intramedullary nailing (ESIN) for displaced and unstable pediatr

[Management of fractures in the severely injured--influence of the principle of "damage control orthopaedic surgery"].

There have been substantial changes in the management of multiply injured patients over the last decades. In the 1950s and 60s, perioperative care was limited and the surgical techniques were not well developed. It was therefore discussed that a patient might be "too sick to operate" and the general recommendation was to postpone surgical care of the extremities, until a patient had stabilized. In addition this recommendation was maintained by fears of the "fat embolism syndrome", which was considered to be directly related to fat and intramedullary contents released from the fracture site lead

Time to union in ballistic trauma lower extremity diaphyseal fractures treated with intramedullary nailing.

BACKGROUND: Time to union in civilian firearm injuries is variable and not well described in the literature. This study measures the time to union for femoral and tibial shaft fractures treated with an intramedullary nail (IMN). Time to union is compared across open, closed, or ballistic trauma (BT) fractures. The goal of the study is to answer the questions: (1) Do tibial shaft fractures heal in specific temporal patterns based on mechanism of injury and (2) Do these temporal healing patterns apply to both the tibia and femur? PATIENTS AND METHODS: Included patients had tibial or femoral sha

Outcomes of antegrade straight interlocking intramedullary nail combined with autogenous iliac crest bone graft in the treatment of nonunions of humeral surgical neck fractures at a minimum 2 years' follow-up.

ObjectiveNonunions of humeral surgical neck fractures (HSNF) are rare and complex problems. This study aimed to evaluate the clinical and functional outcomes of antegrade straight interlocking intramedullary nail (ASIIN) combined with autogenous iliac crest bone graft (ICBG) for treating nonunions of HSNF and to analyze the impact of initial treatment on nonunion repair outcomes.MethodsWe conducted a retrospective analysis of patients with nonunions of HSNF treated with ASIIN combined with autogenous ICBG at our institution. Patients were divided into two groups based on initial treatment: pri

Percutaneous versus subcutaneous titanium elastic nail tips: complication profiles by age group in pediatric forearm shaft fractures.

BACKGROUND: Titanium elastic nailing (TEN) is widely used for the treatment of unstable pediatric forearm fractures. Although subcutaneous nail tips remain the standard technique, superficial (exposed) nail tip placement has been described as an alternative strategy to facilitate implant removal. However, evidence regarding complication profiles associated with different TEN tip management techniques remains limited, particularly with respect to age-related differences. This study addresses three critical gaps: (1) comparative complication rates between superficial and subcutaneous TEN placeme

Comparison of mechanical and clinical outcomes between cement-augmented and conventional cephalomedullary nailing in osteoporotic trochanteric fractures: a propensity score-matched cohort study.

BACKGROUND: Trochanteric fractures in the older population are challenging to treat due to osteoporotic bone and high risk of fixation failure. Cement augmentation (CA) of cephalomedullary fixation has been proposed to enhance implant anchorage and reduce complications. This study compared mechanical failure rates and clinical outcomes between cement-augmented and conventional cephalomedullary nailing in osteoporotic trochanteric fractures. METHODS: We performed a retrospective comparative study of patients with trochanteric fractures treated with either a CA or non-CA cephalomedullary nail f

Nail Versus Plate for Distal Femur Fracture: A Propensity-matched Analysis.

INTRODUCTION: Distal femur fractures are commonly treated with retrograde intramedullary nails or lateral locked plates. Although enthusiasm is growing for dual-implant constructs, most fractures are still treated with a single implant, though it remains unclear whether one implant type offers superior outcomes. This study compared revision surgery rates as well as clinical and radiographic outcomes between matched cohorts of distal femur fractures treated with intramedullary nails or lateral plates. METHODS: This multicenter retrospective study included adult patients with OTA/AO 33A or 33C

Long-term Outcomes of Intramedullary Nails in Osteogenesis Imperfecta: Fewer Surgeries and Longer Survival Times With Telescoping Rods in Patients With Over Ten Years Follow-up.

OBJECTIVE: Intramedullary rodding of lower extremity long bones in patients with osteogenesis imperfecta (OI) is a widely accepted technique for fracture treatment and prevention. Previous small studies with short to medium-term follow-up have shown that telescoping rods have longer survival times compared with static rods. However, there are no long-term studies (10 or more years) longitudinally evaluating the results of these procedures. The purpose of this study was to compare the rod survival duration and number of procedures in patients treated with static versus telescoping rods with 10
