Transoral Endoscopic Coblation Tongue Base Surgery in Obstructive Sleep Apnea: Resection versus Ablation.

BACKGROUND: A new transoral tongue base surgical procedure for the treatment of snoring and obstructive sleep apnea (OSA) is described. It is named "Robo-Cob" technique because it is similar to transoral robotic surgery (TORS) but it is performed by means of coblation technology in order to resect the tongue base in countries where TORS is not an available option for such benign conditions. METHODS: In this prospective, randomized, controlled trial, the new Robo-Cob technique was carried out in 25 adult OSA patients with confirmed tongue base hypertrophy at preoperative drug-induced sedation

Are urological procedures in tetraplegic patients safely performed without anesthesia? a report of three cases.

BACKGROUND: Some tetraplegic patients may wish to undergo urological procedures without anaesthesia, but these patients can develop autonomic dysreflexia if cystoscopy and vesical lithotripsy are performed without anaesthesia. CASE PRESENTATION: We describe three tetraplegic patients, who developed autonomic dysreflexia when cystoscopy and laser lithotripsy were carried out without anesthesia. In two patients, who declined anaesthesia, blood pressure increased to more than 200/110 mmHg during cystoscopy. One of these patients developed severe bleeding from bladder mucosa and lithotripsy was ab

Zero-fluoroscopy catheter ablation for arrhythmias in pregnancy: validation of a standardized approach with 5-year follow-up.

BACKGROUND: Increasing experience with zero- (ZF) or near-zero fluoroscopy catheter ablation (CA) supports the implementation of an early fluoroless approach for recurrent, symptomatic arrhythmias during pregnancy. AIM: This study evaluated the feasibility, efficacy, and safety of CA using a standardized ZF approach during pregnancy. METHODS: Data were derived from a large, prospective multicenter registry (ELEKTRO-RARE-A-CAREgistry). Between 2012 and 2019, more than 2,655 CA procedures were performed in women using an intention-to-treat ZF approach. The procedures were performed using (1) f

Marriage abdominoplasty expands the mini-abdominoplasty concept.

The marriage of aggressive superwet liposculpture of the abdomen and adjacent anatomic regions with a modification of well-established open surgical techniques to address skin excess and perform muscle plication was used to treat 29 patients presenting for aesthetic abdominal contouring over the past 3 1/2 years. The charts of 57 patients who had aesthetic contouring procedures on the abdomen performed from December of 1994 to July of 1998 were retrospectively reviewed. Fifteen patients underwent suction lipectomy alone, 13 patients were treated with conventional abdominoplasty, and 29 underwe

Awake Tracheostomy in the Sitting Position for a Predicted Difficult Airway in Ankylosing Spondylitis With Inhalation Injury: A Case Report.

BACKGROUND: Airway management in patients with inhalation injury is particularly challenging because of rapidly progressive airway edema. This difficulty is further compounded in patients with ankylosing spondylitis, in whom cervical spine rigidity and deformity severely restrict neck mobility and may prevent standard positioning for airway interventions. In such situations, conventional airway management techniques may fail, necessitating alternative strategies. CASE PRESENTATION: We report the case of a 48-year-old man with inhalation injury following a flame burn who also had long-standing

Evaluation of the Efficacy of Bupivacaine (0.5%) alone or with Clonidine (1μg/kg) Versus Control in a Single Level Paravertebral Blockin Patients Undergoing PCNL Procedure.

INTRODUCTION: Percutaneous Nephrolithotomy (PCNL) is a widely used procedure to remove complex upper tract renal calculi by means of a nephroscope. Although less invasive, PCNL is associated with significant pain owing to soft tissue injury. Most of these patients have mild to moderately compromised renal function. An anaesthesia plan that reduces intraoperative requirement of anaesthetics, analgesics, muscle relaxants and postoperative requirement of systemic analgesics is essential. Paravertebral Block (PVB) in combination with general anaesthesia may be an ideal technique for achieving all

REASONS FOR PERFORMING STUDY: There are no consistently safe and effective methods for the treatment of trigeminal-mediated headshaking in horses. In affected horses, the trigeminal nerve is sensitised, appearing to result in neuropathic pain. Percutaneous electrical nerve stimulation (PENS) therapy is a minimally invasive neuromodulatory treatment used in people to manage neuropathic pain. OBJECTIVES: To determine whether PENS therapy is safe, tolerated and effective for the management of trigeminal-mediated headshaking in horses. STUDY DESIGN: Descriptive case series. METHODS: Seven horse

Impact of healthcare system strain on the implementation of ICU sedation practices and encephalopathy burden during the early COVID-19 pandemic.

The COVID-19 pandemic posed unprecedented challenges to healthcare systems worldwide, particularly in managing critically ill patients requiring mechanical ventilation early in the pandemic. Surging patient volumes strained hospital resources and complicated the implementation of standard-of-care intensive care unit (ICU) practices, including sedation management. The objective of this study was to evaluate the impact of an evidence-based ICU sedation bundle during the early COVID-19 pandemic. The bundle was designed by a multi-disciplinary collaborative to reinforce best clinical practices rel

Proportional assist ventilation feasibility in the early stage of respiratory failure: a prospective randomized multicenter trial.

BACKGROUND: Proportional assist ventilation (PAV+) is an assisted ventilator mode usually applied during weaning. We aimed to determine the feasibility of using PAV+ in the early phase of acute respiratory failure compared to volume-assist control ventilation (V-ACV) in order to shorten the length of mechanical ventilation (MV). METHODS: We conducted a prospective randomized trial comparing high-assistance PAV+ (gain 80%) vs. V-ACV in four university hospital Intensive Care Units. Patients were included based on a previous pilot trial. Length of MV was the main objective. Secondary objectives

Neuromuscular electrical stimulation prevents muscle wasting in critically ill comatose patients.

Fully sedated patients, being treated in the intensive care unit (ICU), experience substantial skeletal muscle loss. Consequently, survival rate is reduced and full recovery after awakening is compromised. Neuromuscular electrical stimulation (NMES) represents an effective method to stimulate muscle protein synthesis and alleviate muscle disuse atrophy in healthy subjects. We investigated the efficacy of twice-daily NMES to alleviate muscle loss in six fully sedated ICU patients admitted for acute critical illness [n=3 males, n=3 females; age 63 ± 6 y; APACHE II (Acute Physiology and Chronic H
