Intramedullary Screw Fixation of Metacarpal Fractures Results in Excellent Functional Outcomes: A Literature Review.

BACKGROUND: Intramedullary fixation of a comminuted subcapital metacarpal fracture using a headless compression screw was first described in 2010. The purpose of this study was to identify and evaluate all studies reporting clinical outcomes of intramedullary screw fixation of metacarpal fractures. METHODS: A comprehensive search of the literature was performed under Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines to identify all relevant studies. Outcome measures included mean follow-up, evidence of radiographic union, functional outcomes, and quality-of-life mea

Novel TNXB Variants in Two Italian Patients with Classical-Like Ehlers-Danlos Syndrome.

TNXB-related classical-like Ehlers-Danlos syndrome (TNXB-clEDS) is an ultrarare type of Ehlers-Danlos syndrome due to biallelic null variants in TNXB, encoding tenascin-X. Less than 30 individuals have been reported to date, mostly of Dutch origin and showing a phenotype resembling classical Ehlers-Danlos syndrome without atrophic scarring. TNXB-clEDS is likely underdiagnosed due to the complex structure of the TNXB locus, a fact that complicates diagnostic molecular testing. Here, we report two unrelated Italian women with TNXB-clEDS due to compound heterozygosity for null alleles in TNXB. Bo

Discharged but not dissatisfied: outcomes and satisfaction of patients discharged from the Edinburgh Trauma Triage Clinic.

AIMS: The Edinburgh Trauma Triage Clinic (TTC) streamlines outpatient care through consultant-led 'virtual' triage of referrals and the direct discharge of minor fractures from the Emergency Department. We compared the patient outcomes for simple fractures of the radial head, little finger metacarpal, and fifth metatarsal before and after the implementation of the TTC. PATIENTS AND METHODS: A total of 628 patients who had sustained these injuries over a one-year period were identified. There were 337 patients in the pre-TTC group and 289 in the post-TTC group. The Disabilities of the Arm, Sho

Biomechanical comparison of 3 methods of scapholunate ligament reconstruction.

PURPOSE: To compare the biomechanical characteristics of 3 methods of scapholunate (SL) ligament reconstruction, including 1 that provides a biological central axis tether. METHODS: Twelve fresh-frozen cadaver limbs were mounted on a jig that allowed for wrist and finger motion by tendon loading. The specimens were randomized to receive the SL axis method (SLAM) reconstruction, the Blatt capsulodesis (BC), or the modified Brunelli tenodesis (MBT). Fluoroscopic images were taken to measure the SL interval and SL angle in various positions. The specimens were evaluated in 4 states: intact, with

Ethanol sclerotherapy for venous malformation.

BACKGROUND: Ethanol sclerotherapy (ES) is the preferred treatment for venous malformation (VM) with surgery playing an adjunctive role. Results of ES, however, are not well documented in the literature. METHODS: VM patients were identified from our vascular anomalies database from 1996 to 2011. After treatment completion, patients completed questionnaires evaluating symptoms and their severity and effect on appearance, function and overall quality of life (QoL), before and >6 months after treatment, using visual analogue scales of 0-10. Patients rated their overall satisfaction with ES using

Expression pattern of zinc-finger transcription factor Odd-skipped related 2 in murine development and neonatal stage.

The Odd-skipped gene, first identified as a Drosophila pair-rule zinc-finger transcription factor, plays an important role in Drosophila development. The mammalian homolog, Odd-skipped related 2 (Osr2), regulates limb, tooth, and kidney development in mouse embryos. However, the detailed expression pattern of Osr2 during neonatal development remains unclear. In this study, we investigated Osr2 expression patterns in mouse neonatal and embryo tissues using qPCR and in situ hybridization methods. First, we examined the tissue distribution of Osr2 by qPCR, and found it to be highly expressed in t

Supernumerary digital flexion creases: an additional clinical manifestation of Alagille syndrome.

Alagille syndrome (AGS; OMIM 118450) is a complex dominantly inherited multisystem disorder involving the liver, heart, eyes, facies, skeleton, and other systems. Criteria for the clinical diagnosis have been established as the presence of bile duct paucity on liver biopsy in association with three of five major clinical findings (cholestasis, butterfly vertebrae, posterior embryotoxon, congenital heart disease, and facial features). Jagged1 has been identified as the AGS disease gene. Jagged1 is a large gene, with no mutational hot spots, making molecular testing difficult at this time. Other

Deforming arthropathy or lupus and rhupus hands in systemic lupus erythematosus.

OBJECTIVE: Although deforming arthropathy in systemic lupus erythematosus (SLE) is characterised by a number of manifestations, definitive criteria for the different forms have not yet been established. To define deforming arthropathy and its different types a study was undertaken of 176 SLE patients. METHODS: Using as criterion any deviation from any of the metacarpus finger axes 17 patients (16 women, one man) were identified with clinical deforming arthropathy. These patients were evaluated according to a standardised protocol that covered all known characteristics of deforming arthropathy

Mandibular advancement with dental appliances in obstructive sleep apnoea.

The purpose of this study was to compare the effects of a modified Herbst appliance (mHA) and a muscle relaxation appliance (MR) on nocturnal breathing and body movement activity in patients with obstructive sleep apnoea syndrome (OSAS). To increase the airway space posterior to the tongue base without severely affecting the craniomandibular joint, the mHA was adjusted to anchor the mandible at 50% of maximum protrusion. MR producing an occlusal coverage but no protrusion served as a control appliance. All-night static charge-sensitive bed (SCSB) and finger oximeter recordings were done to six

Tuft resorption in patients with psoriatic arthritis.

OBJECTIVES: Tuft resorption (TR) is an important radiographic feature of psoriatic arthritis (PsA). We aimed to define the prevalence of TR in patients with PsA, the clinical and radiographic features associated with it, and the risk factors for its occurrence. METHODS: We included patients with PsA followed at our prospective observational cohort. We defined TR as resorptive changes in the terminal tufts of the fingers or toes. We used generalized estimating equations to characterize clinical and radiographic features cross-sectionally associated with TR. We used multivariate Cox regression

A Complex Intrachromosomal Rearrangement Disrupting IRF6 in a Family with Popliteal Pterygium and Van der Woude Syndromes.

Clefts of the lip and/or palate (CL/P) are considered the most common form of congenital anomalies occurring either in isolation or in association with other clinical features. Van der woude syndrome (VWS) is associated with about 2% of all CL/P cases and is further characterized by having lower lip pits. Popliteal pterygium syndrome (PPS) is a more severe form of VWS, normally characterized by orofacial clefts, lower lip pits, skin webbing, skeletal anomalies and syndactyly of toes and fingers. Both syndromes are inherited in an autosomal dominant manner, usually caused by heterozygous mutati

Proximal Interphalangeal Joint Adipofascial Flap Resurfacing Improves the Active Motion of the Proximal Interphalangeal Joint after Contracture Release.

BACKGROUND: Posttraumatic proximal interphalangeal (PIP) joint contractures of the digits are common and are associated with impaired hand function. However, relapse is common after surgical release of PIP joint contractures. This article presents a novel treatment strategy with a PIP joint adipofascial flap to resurface the joint after release, and compares patients with similar joint contracture release who did and did not undergo resurfacing with a PIP joint adipofascial flap. METHODS: From January of 2010 to January of 2018, 10 patients received single-digit PIP joint flexion contracture

Sudden multiple fractures in a patient with sarcoidosis in multiple organs.

A 30-year-old man who incidentally fractured his right olecranon and other multiple phalanges was admitted to our hospital. He had a 2-year history of uveitis and bilateral hilar lymphadenopathy (BHL), and pulmonary sarcoidosis was diagnosed from transbronchial lung biopsy. Right elbow arthrodesis was performed, and biopsied specimens showed non-caseating epithelioid cell granuloma, suggesting osseous sarcoidosis. He was discharged uneventfully without further treatment, but BHL had progressed with the appearance of lung parenchymal lesions 3 months later. At that time, involvement of other or

The prevalence of the extensor digitorum communis tendon and its insertion variants: a systematic review and meta-analysis.

The tendons of the Extensor Digitorum Communis (EDC) are frequently injured in hand trauma. Dislocation and spontaneous rupture can also occur during the course of wrist osteoarthritis and rheumatoid arthritis. The EDC exhibits many variations including splitting of its individual slips to the medial four fingers or their absence. The aim of this systematic review is to assemble evidence about the prevalence of the EDC and its variants on the dorsum of the hand. Twenty-four cadaveric studies met the inclusion criteria, providing data from a total of 2,005 hands. Meta-analysis yielded the follo

Nail dystrophy in psoriatic arthritis presenting as nail onychomycosis: a case report.

Dystrophic nail changes without a skin lesion is a rare form of psoriasis. We describe a patient that presented with a chronic nail onychomycosis and a destructive thumb interphalangeal joint which was initially considered as onychomycotic joint infection, and was later diagnosed as having dystrophic nail changes in psoriatic arthritis and coexistent onychomycosis. The nail changes were completely normalized after methotrexate treatment.

Immediate active short arc motion following extensor tendon repair.

Postoperative management of the extensor tendon injury has been altered over the past 15 years from treatment with 4 to 6 weeks of immobilization to controlled passive motion and, now, to active SAM with minimal tension and wrist tenodesis programs. The concepts of immediate motion are supported biochemically in experimental studies, and biomechanically through excursion studies, mathematical analysis of tendon excursion and force application, and study of repair tensile strengths. The results cited in this article and those reported by others demonstrate that early motion for extensor tendons
