[Chondroblastoma--current opinion].

Chondroblastoma of bone is rare bone tumor, representing around 1% of benign bone lesions. It is considered a benign lesion, although primary malignant form as well as malignant alteration in the form of chondrosarcoma has been documented. It occurs predominantly in the second decade, more commonly in males. Predilection sites include proximal humeral epiphysis, femoral and tibial condyles, but it can be found in other bones, too (skull, pelvis, posterior vertebral structures, tarsal bones). Radiographically, it appears as an ovoid lesion with thin sclerotic margin, located centrally in the ep

Efficacy, pain, and overall patient satisfaction with pediatric upper arm fracture reduction in the emergency department.

INTRODUCTION: Immediate closed reduction and cast immobilization performed under sedation in the emergency room is the mainstay management for most isolated displaced or angulated upper limb fractures in children. We aimed to determine if this approach is safe, effective and if patients, parents and staff are satisfied with this approach. Our working hypothesis was this management provides a high satisfaction rate. PATIENTS AND METHODS: Between January 2017 and October 2017 we included 118 children presenting with upper arm fractures amenable to closed reduction under our institutional analges

Utilization of axillary brachial plexus block in the postoperative rehabilitation of intra-articular fractures of the distal humerus.

OBJECTIVES: An effective rehabilitation program is essential to prevent joint stiffness and regain range of motion after surgical treatment of intra-articular fractures of the distal humerus. We evaluated the effect of a physiotherapy program on functional results, that involved passive resistive stretching exercises performed under axillary brachial plexus block after radiographic observation of bone union of intra-articular fractures of the distal humerus treated with open reduction and internal fixation. METHODS: The study included 21 patients (7 females, 14 males; mean age 34+/-5 years; r

Fractures of the distal third of the humerus with palsy of the radial nerve: management using minimally-invasive percutaneous plate osteosynthesis.

Fractures of the distal third of the humerus may be complicated by complete lesions of the radial nerve which may be entrapped or compressed by bone fragments. Indirect reduction and internal fixation may result in a permanent nerve lesion. We describe the treatment of these lesions by insertion of a bridge plate using the minimally-invasive percutaneous technique. Six patients were operated on and showed complete functional recovery. Healing of the fractures occurred at a mean of 2.7 months (2 to 3) and complete neurological recovery by a mean of 2.3 months (1 to 5). In one patient infection

Aneurysmal bone cyst in 29 children.

The purpose of this study was to review longitudinally the clinical features, method of treatment, and recurrence rates of 40 cases of aneurysmal bone cyst in children treated at one institution. Twenty-nine patients with histologic confirmation of the diagnosis and minimum follow-up of 2 years were included. Thirteen patients were less than 10 years of age, and 16 were in the second decade of life. The most frequent location of the lesion was the tibia (seven cases). Patients were treated with curettage, curettage and bone grafting, or resection. The overall recurrence rate was 27.5%. Five le

Secular trends in the osteoporotic fractures of the distal humerus in elderly women.

Osteoporosis, with its sequelae of fracture, is a major, continuously increasing threat to the health of the elderly, and therefore reliable epidemiological information is needed for assessment of the fracture development in the future and for effective fracture prevention. However, very little population-based information is available concerning the nationwide numbers, incidences and especially secular trends of osteoporotic fractures other than those occurring at the hip. We determined the current trends in the number and incidence of osteoporotic fractures of the distal humerus in Finnish w

[Does the degree of dislocation correlate with therapy procedure in supracondylar humerus fractures in childhood?].

Hundred and twenty-eight supracondylar fractures of the humerus were studied retrospectively after an follow-up time of 4.3 years (1 to 17.8 years). In 87 cases (68%) the operative procedure was the closed reduction and percutaneous crossed-pin fixation for 19 fractures type III (56%). 22 fractures type II (76%) and 46 fractures type I (85%), whereas 41 fractures were treated by open reduction and crossed-pin fixation. The findings were evaluated according to "Flynn's criteria" leading to the following results: "excellent" 77 times (60.2%), "good" 44 times (34.4%) "fair" 3 times (2.3%) and "po

Vascularized fibula flap onlay for salvage of pathologic fracture of the long bones.

BACKGROUND: Long bone pathologic fractures, especially when radiation-induced, represent a significant and difficult-to-treat entity. The ultimate goal of treatment is limb salvage; however, there are no treatment methods that guarantee bony healing. The vascularized fibula flap has revolutionized bone reconstruction, including that of the long bones. The authors report the results of onlay vascularized fibula flaps for pathologic long bone fractures and the outcomes and complications of this reconstruction method. METHODS: Records of patients who underwent long bone pathologic fracture recon

The effect of shoulder prosthesis stem length on failure due to torsional loading. A biomechanical study in composite humeri.

BACKGROUND: Shoulder arthroplasty is becoming increasingly common. With evolving implant designs, multiple humeral stem options exist for the surgeon to choose from. New stemless and short-stem systems are modular, remove less native bone stock, and better adapt to patient anatomy. It has been suggested that shorter stem implants may be protective against periprosthetic fracture; however, this has not been mechanistically evaluated. Therefore, this study aimed to biomechanically test synthetic humeri with long-stem, short-stem, and stemless arthroplasty components in a torsional manner to eval

Functional outcomes before and after implant removal in patients with posttraumatic shoulder stiffness and healed proximal humerus fractures: does implant material (PEEK vs. titanium) have an impact? - a pilot study.

BACKGROUND: Posttraumatic shoulder stiffness remains a problem after proximal humerus fracture (PHF) despite good healing rates. The aim of this pilot study was to determine whether the implant material and overlying soft tissue have an effect on shoulder range of motion (ROM) before and after implant removal (IR). METHODS: 16 patients (mean age 55.2 ± 15.3 (SD) years; 62.5% female) were included who underwent operative treatment with locking plates of either carbon fiber reinforced Polyetheretherketone (PEEK) (PEEKPower® humeral fracture plate, Arthrex, Naples, Florida, USA, n = 8) or titani

Correlation Between the Deltoid Tuberosity Index and DXA Measurements: Feasibility of Opportunistic Osteoporosis Screening Using Routine Radiograph.

Osteoporosis is a common metabolic bone disease associated with increased fracture risk and reduced quality of life. The deltoid tuberosity index (DTI), derived from humeral radiographs, reflects cortical thickness and may serve as a simple radiographic indicator of reduced bone mass. However, limited data exist regarding the relationship between DTI and dual-energy X-ray absorptiometry (DXA) parameters across different skeletal regions, particularly using routine chest radiographs. This retrospective study included 318 patients (303 females, 15 males) who underwent DXA measurements and radiog
