Comparative effects of potassium chloride and bicarbonate on thiazide-induced reduction in urinary calcium excretion.

BACKGROUND: The chronic low-grade metabolic acidosis that occurs in various renal disorders and in normal people, and that is related both to dietary net acid load and age-related renal functional decline, may contribute to osteoporosis by increasing urine calcium excretion. Administration of potassium (K) alkali salts neutralizes acid and lowers urine calcium excretion. Urine calcium excretion also can be reduced by the administration of thiazide diuretics, which are often given with supplemental K to avoid hypokalemia. We determined whether the K alkali salt potassium bicarbonate (KHCO3) and

Glycine suppresses kidney calcium oxalate crystal depositions via regulating urinary excretions of oxalate and citrate.

An abnormal urine composition is a key reason for kidney stone formation, but little is known about the roles of small metabolites in the urine during kidney stone formation. Here, we found urine glycine in patients with kidney calcium oxalate (CaOx) stone was significantly lower than that in healthy people via 1 H NMR spectra detection, and investigated the role and underlying mechanism of glycine in the regulation of CaOx stone formation. Our results showed that glycine could significantly attenuate ethylene glycol-induced CaOx crystal depositions in rat kidney via decreasing urine oxalate a

One year follow up of the outcomes of child patients with melamine-related kidney stones in Beijing and surrounding provinces in China.

AIM: To evaluate their prognosis, the damage by melamine on children's kidney and other organs, and its influence on the children's development, was investigated. METHODS: Nine hundred and sixty-two Chinese children were divided into three groups: group A, 265 children diagnosed with melamine-associated urolithiasis; group B, 197 children with a history of melamine-contaminated milk powder consumption but without urolithiasis; and group C, 500 healthy children. They were examined at 0, 1, 3, 6 and 12 months. The factors influencing the prognosis were investigated. The stone discharge was moni

Mutational analysis of idiopathic renal hypouricemia in Korea.

Idiopathic renal hypouricemia is a hereditary disease characterized by abnormally high renal uric acid clearance. Most patients are clinically silent, but acute renal failure (ARF), urolithiasis, or hematuria may develop. A defect in the SLC22A12 gene, which encodes the renal uric acid transporter, URAT1, is the known major cause of this disorder. We performed a mutational analysis of the SLC22A12 gene in five Korean patients with idiopathic renal hypouricemia in this study. Two patients presented with microscopic hematuria, one with uric acid urolithiasis, and one with exercise-induced ARF. O

Lower Urinary Tract Symptoms in Men: A Review.

IMPORTANCE: Up to 40% of men older than 50 years have lower urinary tract symptoms, including urinary urgency, nocturia, and weak urinary stream, due to disorders of the bladder and prostate. These symptoms negatively affect quality of life and may be associated with urinary retention, which can cause kidney insufficiency, bladder calculi, hematuria, and urinary tract infections. OBSERVATIONS: In men, lower urinary tract symptoms can be caused by bladder outlet obstruction secondary to benign prostatic hyperplasia (BPH), an overactive bladder detrusor (a syndrome of urinary urgency and freque

Guidelines for the treatment of acidaemia with THAM.

THAM (trometamol; tris-hydroxymethyl aminomethane) is a biologically inert amino alcohol of low toxicity, which buffers carbon dioxide and acids in vitro and in vivo. At 37 degrees C, the pK (the pH at which the weak conjugate acid or base in the solution is 50% ionised) of THAM is 7.8, making it a more effective buffer than bicarbonate in the physiological range of blood pH. THAM is a proton acceptor with a stoichiometric equivalence of titrating 1 proton per molecule. In vivo, THAM supplements the buffering capacity of the blood bicarbonate system, accepting a proton, generating bicarbonate

Clinical features of Escherichia coli positive calcium oxalate stones and effects of outer membrane vesicles on tubular epithelial injury and crystal adhesion.

BACKGROUND: Although calcium oxalate (CaOx) stones are classically regarded as a metabolic stone type, bacterial factors may also contribute to clinical heterogeneity. However, the biological relevance of bacteriuria in CaOx stone disease remains unclear. We investigated the clinical association between bacteriuria and CaOx stone phenotype and examined whether Escherichia coli-derived outer membrane vesicles (OMVs) could induce epithelial changes relevant to crystal retention. METHODS: We retrospectively analyzed 1,976 patients with urinary stones treated at a single center between 2016 and 2

Causal relationship between basal metabolic rate and kidney stone disease: from discovery in US NHANES to evidence in UK Biobank cohorts.

BACKGROUND: Our previous genome-wide association study (GWAS) suggested a potential negative causal relationship between basal metabolic rate (BMR) and kidney stone disease (KSD) from a genetic standpoint. This study aimed to further investigate their association from a clinical etiological perspective across diverse global populations and to elucidate their dose-response relationship. METHODS: A total of 21 140 adults from the US NHANES (2007-2020) and 289 007 participants without a prior history of KSD from the UK Biobank (2006-2024) were analyzed using cross-sectional and prospective cohor

Association between drinking water type and kidney stone risk in U.S. adults: a cross-sectional analysis of NHANES 2009-2016 data.

PURPOSE: While fluid intake and trace elements are known to influence kidney stone risk, the impact of drinking water type remains unclear. This study aimed to systematically evaluate the association between water source and kidney stone prevalence. METHODS: This analysis included 10 246 adults (≥20 years) from the National Health and Nutrition Examination Survey database (2009-2016). Kidney stone history was self-reported, and drinking water sources were assessed through 24-hour dietary recalls. We employed multivariable-adjusted logistic regression models, adjusting for potential confounders

Influence of Short-Term Consumption of Hericium erinaceus on Serum Biochemical Markers and the Changes of the Gut Microbiota: A Pilot Study.

Hericium erinaceus (H. erinaceus) is widely studied as a medicinal and edible fungus. Recent studies have shown that H. erinaceus has protective effects for diseases, such as inflammatory bowel disease and cancer, which are related to gut microbiota. To investigate the benefits of H. erinaceus intake on gut microbiota and blood indices in adulthood, we recruited 13 healthy adults to consume H. erinaceus powder as a dietary supplement. Blood changes due to H. erinaceus consumption were determined by routine hematological examination and characterized by serum biochemical markers. Microbiota com
