The role of policy in community pharmacies' response to injection-drug use: results of a nationwide Canadian survey.

The response rate to this survey reflects the salience of the topic and the professional concern about and interest in issues presented by HIV. The HIV/AIDS epidemic has presented pharmacists with one of the greatest challenges to their professional training, ethics, and practice. It further expedites a current re-examination that is occurring among community pharmacies concerning their roles in community health practices. In response to HIV there have been dramatic and unprecedented changes in pharmacy policy and practices. Clearly, some community pharmacies have led the way and influenced po

Proceedings of the Annual Scientific Meeting of the International Medical Society of Paraplegia held at Stoke Mandeville from 28-30 July 1977 (Part II). Fracture dislocation of the cervical spine: a critique of current management in the United States.

The author has been asked to review the hospital records and X-rays of 12 patients from various regions of the United States who have sustained fracture dislocations of the cervical spine. In no instance was the initial care considered to be appropriate. No patient was significantly improved by treatment although only 17% had apparent complete transverse cord syndromes on admission. Sixty-seven per cent of the patients became worse. In the latter group there were three patients who were admitted with either no neurological deficit or only minimal pyramidal signs. All of these three patients be

Development and validation of machine learning models for obstructive sleep apnea risk stratification in high-risk pregnant women.

BACKGROUND: Obstructive sleep apnea (OSA) affects approximately 15% of pregnancies and is associated with adverse maternal and fetal outcomes. Although polysomnography (PSG) is the diagnostic gold standard, increasing clinical demand creates substantial bottlenecks in manual PSG scoring and specialist review. Conventional screening tools often show limited discriminative ability in high-risk referred populations. Therefore, optimized risk stratification models are needed to streamline clinical workflows, prioritize diagnostic resource allocation, and facilitate timely intervention for high-ris

Effectiveness and Outcomes of Cast Immobilization in Adults With Scaphoid Waist Fractures Compared With Surgical Fixation: A Systematic Review and Meta-Analysis.

The optimal management of acute, minimally displaced, or undisplaced scaphoid waist fractures is a clinical equipoise. Although cast immobilization is effective, it requires prolonged wrist restriction. Early surgical fixation aims to expedite recovery but introduces operative risks and incurs higher costs. This systematic review and meta-analysis synthesizes contemporary, high-quality evidence comparing the effectiveness of these two management strategies. Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a systematic search of PubMed, MEDLI

Determination of Modified Waldenström Staging in Legg-Calvé-Perthes Disease Using Deep Learning.

BACKGROUND: Age and disease stage are critical factors in guiding management in Legg-Calvé-Perthes disease (LCPD). The modified Waldenström system is the most common staging system for LCPD but has only moderate inter-rater reliability. Deep learning (DL) has gained popularity in orthopaedics due to its potential to improve consistency/efficiency in determination of image-based classifications. The current study aimed to develop a DL model for determination of modified Waldenström staging from standard hip radiographs. METHODS: Paired anteroposterior (AP) and frog-lateral hip radiographs from

Systematic Review of Hybrid Nanosystems as Emerging Tools in Targeted Cancer Therapy and Future Aspects.

INTRODUCTION: Hybrid Nanosystems (HNSs) are a type of advanced drug carrier that can be made from various materials, including organic compounds, inorganic particles, lipids, polymers, and biomimetic components. This combined framework facilitates functionalities such as medication distribution, imaging support, and modification of therapeutic responses. This renders them highly promising for oncological therapy. The main goal of this study was to identify studies on HNSs that investigated their development, underlying characteristics, and therapeutic efficacy. METHODS: A comprehensive review

Reassessing the clock in cauda equina syndrome: a systematic review and meta-analysis of surgical timing and outcomes.

BACKGROUND CONTEXT: Despite universal advocacy for urgent decompression in cauda equina syndrome (CES), the literature remains inconsistent regarding the optimal surgical timing. Heterogeneous diagnostic criteria, variable timing definitions, and a mix of clinical cohorts and national datasets have contributed to ongoing debate over 24- and 48-hour thresholds. PURPOSE: To evaluate whether timing of decompression influences urinary, neurological, functional, and inpatient outcomes in CES, and to identify clinically meaningful temporal thresholds. STUDY DESIGN/SETTING: Systematic review and me

Telephone and 'Straight-to-Test' Model in Triaging Urgent Suspected Cancer Referrals in a Head and Neck Clinic.

BACKGROUND: In the face of challenging times characterized by high referral volumes, limited resources, and service constraints, our Head and Neck Cancer service at the Broomfield Hospital (Mid and South Essex NHS Foundation Trust) adopted a telephone triage system to streamline patient management and prioritise high-risk cases. To improve efficiency and expedite cancer pathways, we implemented a telephone triage and 'straight-to-test' model (STT), whereby high-risk patients are referred directly for diagnostic imaging following telephone triage, bypassing an initial face-to-face consultation.

Performance of Artificial Intelligence-Based Models for Epiretinal Membrane Diagnosis: A Systematic Review and Meta-Analysis.

TOPIC: Epiretinal membrane (ERM) can impair central vision by forming a pre-retinal fibrous layer on the inner retina. Artificial intelligence (AI)-based tools may streamline ERM diagnosis, but their overall performance and factors affecting accuracy require evaluation. CLINICAL RELEVANCE: With an aging population, ERM prevalence is expected to rise, placing increased demands on clinical resources. Early detection via AI models could expedite diagnosis, reduce subjective errors, and guide timely surgical intervention. This systematic review and meta-analysis evaluates the pooled diagnostic pe

Evaluating the Oncologic and Safety Outcomes of High-Dose Palliative Radiation Treatment with 30 Grays in Five Fractions.

Introduction In select tumor sites, symptom palliation and local control can be improved through delivering higher biological equivalent doses (BED) of radiotherapy. However, not all patients are suitable candidates for stereotactic body radiation therapy (SBRT). The 30 Grays in five fractions (30/5) regimen is a conformal, hypofractionated regimen that offers a higher BED compared to conventional palliative radiotherapy. It adheres to the same organ-at-risk constraints as five-fraction SBRT. The planning and set-up processes are optimized to expedite treatment delivery compared to SBRT. This

Comparative Study of the Effectiveness of Double Priming Agents in Cervical Ripening and Induction of Labor in Nulliparas.

BACKGROUND: Induction of labor is the stimulation of contractions before the spontaneous onset of labor with or without ruptured membranes. The labor induction in the presence of an unfavorable cervix is associated with an increased likelihood of prolonged labor and increased incidence of chorioamnionitis and caesarean section. Consequently, performing cervical ripening before conventional methods of induction has become standard practice. Combined techniques have been proposed to improve the success rate of labor induction and to enhance the progression of labor by leveraging the synergistic

Association of candidate surrogate endpoints with overall survival in advanced biliary tract cancer.

BACKGROUND & AIMS: Surrogate endpoints are increasingly used in biliary tract cancer (BTC) trials. While this may expedite drug approval and decrease costs, surrogate endpoints may not always correlate with an overall survival (OS) advantage. We aimed to explore the association of progression-free survival (PFS), objective response rate (ORR) and disease control rate (DCR) with OS at the trial- and patient-level. METHODS: For the trial-level analysis, we performed a systematic review of Pubmed/MEDLINE, Embase, Cochrane, clinicaltrials.gov and conference proceedings for phase II-III trials in

Dual Role of Quercetin in Promoting Early Wound Healing via Inhibiting Inflammatory Factors and Attenuating Scar Formation by Suppressing Myofibroblast Differentiation.

BACKGROUND: Quercetin, a naturally occurring flavonoid, possesses anti-inflammatory properties and has emerged as a potential modulator of tissue repair. Impaired wound healing and pathological scarring are often driven by excessive inflammation and dysregulated myofibroblast differentiation. Current therapeutic approaches, however, frequently fall short in simultaneously addressing these intertwined challenges. This study investigates whether quercetin can provide a bifunctional therapeutic advantage by promoting early wound closure through inflammation resolution and suppressing scar formati

Research Domain Criteria (RDoC) Constructs of Acute and Potential Threat Differentially Associate With Pediatric Anxiety.

OBJECTIVE: To address problems with the current categorical diagnostic classification system for mental health, the National Institute of Mental Health created the Research Domain Criteria (RDoC) initiative, which encourages researchers to link dimensionally assessed psychological constructs with transdiagnostic psychopathology to expedite discovery of biological mechanisms and clinical translation. Two constructs that were theorized by the RDoC to be pertinent to anxiety symptoms across categorical anxiety disorders and the spectrum of severity are acute threat (AT; fear of a present danger)

Arthroscopic versus open fixation for medial malleolus fractures improves trauma response and bone healing.

Medial malleolus fracture is traditionally performed via an open approach. However, significant drawbacks such as exfoliated soft tissue injury, infection, prolonged recovery periods have been reported. Recently, arthroscopy-assisted reduction percutaneous internal fixation (ARPF) demonstrates advantages in minimizing soft tissue damage, improving joint visualization, and enhancing postoperative recovery, however, robust comparative evidence remains sparse on the efficacy of ARPF versus open reduction and internal fixation (ORIF) in treating medial malleolus fractures. The current study provid

Correlation between clinical and sonographic assessment of the fetal head station in the second stage of labor: a prospective observational study.

BACKGROUND: A correct estimation of the head level is required to evaluate labor progression and decide how to best expedite birth - i.e. instrumental vs cesarean delivery - if dystocia or fetal distress are diagnosed during the second stage. However, evidence from the literature has shown that the clinical diagnosis of the fetal station is mostly imprecise. Intrapartum transperineal sonography has been proposed as a tool allowing a reliable and reproducible evaluation of the fetal head station. OBJECTIVE: The aim of this study was to investigate the correlation and agreement of the clinical
