Hierarchical deep Q-network for anomaly detection and interpretation in multidimensional time-series physical fitness data.

This paper addresses the critical challenge of achieving both accuracy and interpretability in anomaly detection within complex multidimensional time-series physical fitness data. We design and implement an innovative framework based on a Hierarchical Deep Q-Network (HDQN) that effectively processes and learns the inherent multi-scale and multi-granularity characteristics of physical fitness data. The introduction of a hierarchical structure enables the model to capture subtle patterns at different levels of abstraction, from low-level sensor fluctuations to high-level behavioral patterns. The

From Single Foods to Whole Diets: A Critical Framework for Protein Quality Assessment Accounting for Complementarity and Matrix Effects.

Accurate protein quality assessment is fundamental to food science and nutrition policy, yet the two dominant metrics, the Protein Digestibility-Corrected Amino Acid Score (PDCAAS) and the Digestible Indispensable Amino Acid Score (DIAAS), evaluate foods in isolation, identifying a single limiting indispensable amino acid (AA) per source. Although PDCAAS and DIAAS are broadly applicable, their single-food approach is especially limiting for plant-based proteins, where AA complementarity and food-matrix complexity are most pronounced. This single-food approach systematically underestimates prot

Weight of evidence in safety pharmacology guidance: A case for explicit probabilistic reasoning in the revision of ICH S7A.

ICH S7A (2000) defined the foundational framework for safety pharmacology studies supporting the registration of human pharmaceuticals. Overall, the guidance applied a risk-based, rational scope combined with a prescriptive framework of core battery endpoints collected under GLP guidelines that were required prior to first-in-human testing. Because of the evolving landscape of pharmaceutical modalities, nonclinical methodologies, and regulatory science over the last two decades, a critical examination of its relationship to weight-of-evidence (WoE) and risk-based reasoning is warranted. The ce

Enhanced Recovery After Surgery in Lower-Extremity Sarcoma Reconstruction: A Scoping Review of Reported Components and Outcomes.

BACKGROUND: Oncologic lower-extremity sarcoma surgery with flap reconstruction is associated with a high risk of postoperative morbidity. Enhanced Recovery After Surgery (ERAS) protocols have been implemented in various surgical fields to improve postoperative outcomes, but ERAS protocols have not yet been adapted specifically for lower-extremity sarcoma surgery. This scoping review maps ERAS-reported components in the literature on lower-extremity sarcoma reconstruction to inform future protocol development. METHODS: We searched PubMed/MEDLINE, Embase, Web of Science, and Cochrane Library fr

Coagulation disorders and thromboprophylaxis in pancreatic cancer: a review of current evidence and clinical challenges.

Pancreatic cancer is one of the most thrombogenic malignancies. Venous thromboembolism (VTE) is a frequent and very severe complication. The risk of thrombosis and postoperative hemorrhage, especially post-pancreatectomy, makes thromboprophylaxis difficult to implement in clinical practice depending on what component dominates. A review was conducted using PubMed, Scopus, Web of Science, Elsevier, and Google Scholar databases. Studies published in English focusing on VTE pathophysiology, prevention, and treatment in pancreatic cancer were included. The evidence was summarized descriptively. Tw

An analytical quality by design-guided method development for the quantification of N-nitroso-fluoxetine to support mitigation strategies for a fluoxetine drug product.

This work focused on N-nitroso-fluoxetine, a N-nitrosamine drug substance-related impurity (NDSRI) formed in fluoxetine-containing products via nitrosation pathways. N-nitroso-fluoxetine was previously detected above the established safety threshold of 100 ng/day, leading to temporary withdrawal of a marketed product in Belgium. An Analytical Quality by Design (AQbD) approach supported the development of a reversed-phase LC-(ESI+)-MS/MS method. Design of experiments identified aqueous mobile phase pH and gradient time as critical method parameters affecting detection or quantification limits a

Protocol for Evaluating The Effect of Pelvic Physiotherapy on Uterine Blood Flow: A Single-Center, Two-Arm, Parallel, Randomized, Clinical Trial.

OBJECTIVE: Repeated implantation failure (RIF) is a major challenge in in vitro fertilization (IVF). Insufficient uterine blood flow and reduced endometrial thickness are important factors contributing to implantation failure. Pelvic physiotherapy, including electrotherapy, manual therapy, and exercise therapy, has shown potential to enhance endometrial thickness and improve local blood flow through pelvic floor exercises. Transfer of Energy Capacitive and Resistive (TECAR) therapy, a novel physiotherapy modality, promotes vasodilation, tissue oxygenation, and blood circulation. This study pre

Developing Scenario-Based Strategies for Health, Climate, and Environmental Preparedness: The One Health, One Earth Approach.

Climate change amplifies many threats to human health. Despite advances in understanding climate change dynamics and impacts, there remains a critical gap in translating scientific knowledge into equitable, and community-driven health interventions. The inaugural One Earth, One Health workshop sought to explore this gap through human-centered design exercises involving interdisciplinary researchers from climate and Earth sciences, engineering, epidemiology, microbiology, and environmental health. Although participants did not co-develop solutions with affected communities, they used stakeholde

Prognostic value of different biomarkers in bladder cancer: protocol for an umbrella review of systematic reviews and meta-analyses.

INTRODUCTION: Bladder cancer (BC), the second most common urinary cancer, is classified as either non-muscle-invasive BC or muscle-invasive BC. Although numerous studies have identified potential prognostic biomarkers, none have yet been implemented in clinical practice. A comprehensive umbrella review comparing all prognostic biomarkers in BC remains absent. Thus, we aim to synthesise evidence from systematic reviews and meta-analyses on prognostic factors in patients with BC. METHODS AND ANALYSIS: We will search PubMed/MEDLINE, Scopus, Web of Science (WOS), EMBASE, Cochrane Database of Syst

Prevalence of human brucellosis in Ethiopia: systematic review and meta-analysis.

Brucellosis is a significant public health challenge in Ethiopia, which is characterized by a high prevalence among humans. The disease is primarily transmitted through the consumption of unpasteurized dairy products, direct contact with infected animals, or inhalation of contaminated aerosols. In addition to its impact on human health, brucellosis imposes substantial economic burdens. However, existing epidemiological studies provide inconsistent prevalence estimates, underscoring the need for a more comprehensive and systematic analysis of the disease's burden and associated risk factors in
