2D-speckle tracking echocardiography contributes to early identification of impaired left ventricular myocardial function in patients with chronic kidney disease.

BACKGROUND/AIMS: Our aim was to investigate left ventricular (LV) physiology and short-axis and long-axis regional LV myocardial function throughout the cardiac cycle with 2D-speckle tracking echocardiography (2D-STE) in patients with chronic kidney disease. METHODS: The study population consisted of 40 maintenance hemodialysis patients (hemodialysis group), 20 uremic patients hospitalized for creation of primary arteriovenous fistula (nondialysis group), and a control group of 20 healthy volunteers. LV regional longitudinal, circumferential and radial peak systolic velocity (Vs); early diast

The role of tubulointerstitial inflammation in the progression of chronic renal failure.

There is compelling evidence that interstitial inflammation plays a central role in the loss of renal function in chronic renal disease. The combined effects of interstitial inflammation, oxidative stress and local angiotensin II activity result in the disruption of glomerulus-tubule continuity, the development of pathogenic hypoxia, the generation of myofibroblasts and fibrosis, and the impairment of the protective autoregulation of glomerular blood flow that leads to glomerulosclerosis. The association between proteinuria and progression of chronic kidney disease is firmly established. Proxi

Accelerated decline of GFR in diabetic nephropathy predicted by interferon release assay to tuberculosis antigens.

BACKGROUND: The QuantiFERON® test (QFT) is a diagnostic tool for active and latent tuberculosis (TB) infections. High rates of positivity to QuantiFERON® have been demonstrated in patients with chronic kidney disease (CKD) and diabetic patients. We performed a pilot study to investigate if QFT positivity in diabetic CKD patients predicted the rate of renal function decline. METHODS: QFT was performed in 38 diabetic patients with CKD 4-5 not on dialysis. The rate of decline in estimated glomerular filtration rate (eGFR) was calculated. RESULTS: 18/38 patients had a positive QFT. Patients with

Clinical significance of early-onset hyperuricemia in renal transplant recipients.

BACKGROUND/AIMS: It is undetermined whether the effect of uric acid (UA) on graft outcome is independent of graft dysfunction. This study was designed to explore whether early-onset hyperuricemia has clinical significance regardless of graft function. METHODS: This study was conducted based on a retrospective chart review. We calculated time-averaged UA and estimated glomerular filtration rate from the values at 3, 6, and 9 months after transplantation. Cardiovascular complications during follow-up and long-term graft survival were assessed according to UA levels and graft function. RESULTS:

Mechanisms of nephronophthisis and related ciliopathies.

An emerging group of human genetic diseases termed 'ciliopathies' are caused by dysfunction of two functionally and physically associated organelles, the centrosome and cilium. These organelles are central to perception of the physical environment through detection of a diverse variety of extracellular signals such as growth factors, chemicals, light and fluid flow. Many of the described ciliopathies display multi-organ involvement, with renal and retina being the most commonly affected. Nephronophthisis is a recessive disorder of the kidney that is the leading cause of end-stage renal failure

Practical genetics for autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is the most common mendelian disorder of the kidney and accounts for ~5% of end-stage renal disease in North America. It is characterized by focal development of renal cysts which increase in number and size with age. Mutations of PKD1 and PKD2 account for most cases. Although the clinical manifestations of both gene types overlap completely, PKD1 is associated with more severe disease than PKD2, with larger kidneys and earlier onset of end-stage renal disease. Furthermore, marked within-family renal disease variability is well documented in

UK Renal Registry 12th Annual Report (December 2009): chapter 11: blood pressure profile of prevalent patients receiving dialysis in the UK in 2008: national and centre-specific analyses.

INTRODUCTION: The UK Renal Registry (UKRR) assesses blood pressure (BP) control annually for patients receiving renal replacement therapy (RRT) at renal centres in England, Wales and Northern Ireland. METHODS: Patients alive and receiving RRT on 31st December 2008 with a BP reading in either the fourth or third quarter of 2008 were included. Summary statistics were calculated for each renal centre, nation and primary renal disease (PRD) category. Longitudinal analyses were performed to assess the long-term impact of treatment modality and PRD on BP control for incident and prevalent patients.

Intravenous iron therapy and risk for progressive loss of kidney function in patients with chronic kidney disease.

BACKGROUND: Intravenous (IV) iron is used in the treatment of anemia in patients with chronic kidney disease (CKD). Several lines of evidence have brought up potential concerns regarding the effect of IV iron on the kidney, specifically the possibility of IV iron leading to renal injury and hastening the progression of CKD. METHODS: We performed a retrospective analysis of 77 patients to assess the rate of change in kidney function prior to and after IV iron infusion. RESULTS: Patients were followed for an average of 21.3 months (range 2-35) prior and 32.8 months (range 2-58) after the singl

Predictors of declining glomerular filtration rate in a population-based chronic kidney disease cohort.

BACKGROUND/AIM: A high percentage of patients with chronic kidney disease has nonprogressive disease. Classification with respect to rate of change in glomerular filtration rate (GFR) would permit a more targeted approach to these patients. The aim of this study was to study predictors of rate of change in GFR in a population-based cohort. METHODS: All prevalent patients with chronic kidney disease stage 3 in a well-defined population in Northern Europe between 1994 and 2003 were included. Clinical and biochemical data were collected from primary care and hospital medical records. GFR was est

An update and practical guide to renal stone management.

Renal stone disease covers kidney and lower urinary tract stones caused by a variety of conditions, including metabolic and inherited disorders, and anatomical defects with or without chronic urinary infection. Most cases are idiopathic, in which there is undoubtedly a genetic predisposition, but where environmental and lifestyle factors play an important role. Indeed, it is becoming apparent that renal stone disease is often part of a larger 'metabolic picture' commonly associated with type 2 diabetes, obesity, dyslipidaemia, and hypertension. Renal stone disease is a growing problem in the U

Moderate chronic kidney disease in women is associated with fracture occurrence independently of osteoporosis.

BACKGROUND: Chronic kidney disease (CKD) is associated with increased risk of fragility fracture but whether this is independent of osteoporosis is unclear. METHODS: We conducted a retrospective cross-sectional study of 1,702 female patients referred for dual-energy X-ray absorptiometry (DXA) scanning (Lunar IDXA) between September 2006 and April 2007. Estimated glomerular filtration rate (eGFR; ml/min/1.73 m(2)) by Modification of Diet in Renal Disease was calculated within 1 year (median interval 4 weeks) of the DXA scan. The independent association of self-reported fracture occurrence with

Increased ACE and decreased ACE2 expression in kidneys from patients with IgA nephropathy.

BACKGROUND: Angiotensin-converting enzyme (ACE)2 forms angiotensin-1-7 which may protect kidney in a counterregulatory manner to angiotensin II. Recent studies revealed increased ACE and decreased ACE2 expression in kidneys of patients with diabetic nephropathy. However, these changes may not be specific for diabetic nephropathy. We studied ACE and ACE2 expression in patients with IgA nephropathy. METHODS: Renal ACE and ACE2 expression was assessed by immunohistochemistry and in situ hybridization in 30 patients with IgA nephropathy and 21 healthy controls. Correlation between ACE and ACE2 ex

Rituximab in ANCA-associated vasculitis: fad or fact?

A number of case reports and case series of patients with Wegener's granulomatosis, microscopic polyangiitis and Churg-Strauss syndrome have supported the use of rituximab (RTX) for the treatment of refractory ANCA-associated vasculitis (AAV). Whether B cell depletion with RTX could replace cyclophosphamide as a first-line therapy for patients with severe AAV remains to be proven. Two studies, recently published in the New England Journal of Medicine, have examined the efficacy of RTX in inducing remission in patients with severe AAV.

High ferritin, but not hepcidin, is associated with a poor immune response to an influenza vaccine in hemodialysis patients.

BACKGROUND: The immune response to vaccination in hemodialysis (HD) patients can be influenced by disorders of iron metabolism, iron overload or chronic inflammatory state. Elevated levels of hepcidin are considered a new marker of iron metabolism impairment and anemia of inflammation in HD patients. METHODS: We studied the effects of hepcidin, other markers of iron status and intravenous iron (Fe(iv)) on the response to an influenza vaccine (Influvac(R) subunit 2008/2009) in 40 HD patients. The immune response of HD patients was compared with that of 46 controls without renal disease accordi

Influence of obesity on progression of non-diabetic chronic kidney disease: a retrospective cohort study.

BACKGROUND: There is increasing awareness of the impact of obesity on chronic diseases including chronic kidney disease (CKD). Until recently, a limited number of epidemiologic studies have examined the association between obesity and CKD. We conducted a retrospective cohort study to evaluate whether obesity impacts on the rate of non-diabetic CKD progression. METHODS: The medical records of 125 non-diabetic CKD patients in the Sheffield Kidney Institute, Sheffield, UK, who have been followed-up for around 10 years, were reviewed. Various socio-demographic, clinical and biochemical parameters

Impact of vitamin D dose on biochemical parameters in patients with secondary hyperparathyroidism receiving cinacalcet.

BACKGROUND/AIMS: The calcimimetic cinacalcet (Mimpara/Sensipar) simultaneously lowers parathyroid hormone (PTH), phosphorus (P) and calcium (Ca) levels in patients with secondary hyperparathyroidism. The OPTIMA study demonstrated that cinacalcet and adjusted doses of vitamin D maximized control of these parameters. This post-hoc analysis of OPTIMA data assessed the impact of reducing or increasing the dose of concomitant vitamin D on PTH, P and Ca in patients receiving cinacalcet. METHODS: Dialysis patients with mean baseline intact PTH (iPTH) 300-800 pg/ml (31.8-84.8 pM) received doses of ci
