BACKGROUND: Prosthetic graft infection or the need for reconstructive arterial surgery in septic condition is a challenging situation in vascular surgery. Recent introduction of silver coated polyester graft has meant a new therapeutic option in selecting the type of graft for revascularization. In this study we analyzed the short and midterm outcome of using silver coated grafts in aortic and lower extremity arterial reconstructions (mortality, graft occlusion, graft infection, amputation).
MATERIALS AND METHODS: In a single center retrospective study we implanted 42 silver coated Dacron gra

INTRODUCTION: Cardiac implantable electronic device (CIED) infections are associated with substantial morbidity and mortality, and early recognition with referral for device extraction is crucial for optimal outcomes. General practitioners (GPs) are essential for long-term follow-up, yet their adherence to international guidelines remains poorly defined. This survey aimed to assess GPs' levels of awareness and clinical management of CIED infections.
METHODS: A nationwide survey endorsed by the Electrophysiology and Cardiac Pacing Study Group of the Italian Federation of Cardiology was sent to

BACKGROUND AND OBJECTIVE: Total Knee Arthroplasty (TKA) has proven highly effective in improving quality of life for patients with severe knee conditions. Despite advancements, surgical complications such as periprosthetic joint infections (PJIs) pose risks. The potential predictive value of pre- and post-operative inflammatory markers like neutrophil-to-lymphocyte ratio (NLR), D-dimer, and albumin levels on surgical outcomes is garnering attention. There is a growing interest in leveraging these markers to enhance patient selection and outcome prediction in the context of TKA.Focusing on the

BACKGROUND: Prosthetic joint infection (PJI) is deemed to be the most serious complication following total hip arthroplasty. Obesity and smoking are known risk factors for PJI. However, the influence of these variables on infection free survival, of septic revision hip arthroplasty, is yet to be explored. The aim of this study was to determine the effect of obesity and smoking on the outcome of two-stage prosthetic exchange surgery.
PATIENTS AND METHODS: A consecutive series of 97 hips in 94 patients (69 male, 25 female, mean age 66±12 years), undergoing two-stage revision surgery for hip PJI

BACKGROUND: Depression is common in the general population, and so it is likewise common among patients undergoing THA and TKA. Depression is associated with lower perioperative patient-reported outcomes and an increased risk of postoperative complications. Antidepressants are effective in managing symptoms of depression and may potentially contribute to better functional status and better clinical outcomes after THA and TKA.
QUESTIONS/PURPOSES: We examined (1) whether perioperative depression is associated with all-cause revisions, revisions for aseptic loosening, revisions without infection

BACKGROUND: The prevalence of congenital heart disease (CHD) in the adult population is steadily increasing. A substrate of prosthetic material and residual lesions, constantly evolving as surgical techniques change over time, predispose these patients to the potentially devastating complication of infective endocarditis (IE).
METHODS: We retrospectively reviewed 2935 patients in our adult CHD database for all cases of endocarditis between 1991 and 2016. Incidence, clinical course and predictors of outcomes were analysed.
RESULTS: We document 74 episodes in 62 patients, with an incidence of

Since 2015, the Multicenter Orthopedic Outcomes Network (MOON) has addressed several factors associated with patient outcome after anterior cruciate ligament (ACL) injury. These factors include predictors of high grade knee laxity, the impact of meniscus treatment on joint space narrowing after surgery, graft selection, treating diabetic patients, predictors of clinically significant pain, and outcomes of patients where meniscus tears were left in situ without treatment. The presence of meniscus tears and generalized laxity predict a lax examination under anesthesia prior to surgery. Allograft

Majocchi granuloma is an uncommon deep follicular inflammation caused by dermatophytes and affects immunocompetent and immunocompromised patients. The clinical findings overlap with other skin conditions such bacterial infections and inflammatory skin diseases, thereby delaying correct diagnosis. We describe 2 cases in immunocompetent patients.

BACKGROUND: The current study sought to demonstrate the advantages offered by fluorine 18-fluorodeoxyglucose ((18)F-FDG) positron emission tomography/computed tomography (PET/CT) in patients supported with continuous-flow left ventricular assist devices (CF-LVADs) in detecting infection and the consequent effect on clinical decisions.
METHODS: Between April 2009 and September 2013, 40 PET examinations were performed in 31 patients (78.1% men; mean age, 51.0 ± 14.9 years) supported with a CF-LVAD. In group A (19 examinations in 14 patients), PET/CT was performed to detect infectious focus in p

PURPOSE: To carry out a statistical analysis on the significant risk factors for deep late infection (prosthetic joint infection, PJI) in patients with a knee arthroplasty (TKA).
METHODS: A retrospective observational case-control study was conducted on a case series of 32 consecutive knee infections, using an analysis of all the risk factors reported in the literature. A control series of 100 randomly selected patients operated in the same Department of a University General Hospital during the same period of time, with no sign of deep infection in their knee arthroplasty during follow-up. BACKGROUND: In recent years arthroscopic interventions of the hip joint have become increasingly more frequent. An advantage of the procedure is that open surgery can be avoided in many cases by using minimally invasive procedures. The spectrum of indications is becoming increasingly broader and more differentiated. Complications are rare.
MATERIAL AND METHODS: According to the types of complications 13,154 cases from institutions having conducted approximately 470 hip arthroscopies per year between 2008 and 2012 were collated, observed and analysed. Intraoperative or postoperative complicati

BACKGROUND: Current guidelines for treatment of Candida osteoarticular infections (COAIs) recommend a prolonged course of antifungal therapy (AFT) of 6-12 months. Based upon strategies developed at the Hospital for Special Surgery (HSS), we hypothesized that the duration of antifungal therapy may be substantially reduced for management of COAI.
METHODS: This was a retrospective chart review of cases of COAI treated at the HSS for the past 14 years. COAI was documented by open biopsy and direct culture in all cases. The mean (95% confidence interval, CI) duration of documented follow-up was 39

BACKGROUND: Thin patients with thoracic pacemakers and automatic implantable cardioverter-defibrillators often have minimal tissue over the devices, with erosion through the surface a major concern. This erosion can lead to device infection and need for removal, or primary device infection can, in turn, lead to erosion. Even worse is exposure and infection of the leads to the heart, with fatalities having occurred. Pressure symptoms, as with shoulder seatbelt straps, can occur, and the visible deformity may be objectionable.
METHODS: To correct these problems without device removal, we used a

BACKGROUND: It is accepted in rhinoplasty that complications are more common with alloplastic implants than with autografts. There is little guidance in the literature on how to deal with the cosmetic and/or functional problems that follow alloplastic implant rejection. The conventional advice has been to remove the allograft and not place any graft at the same time. The present article presents our experience treating allograft rejection and immediately repairing any structural defect with autografts.
OBJECTIVE: To demonstrate that immediate nasal reconstruction using autogenous cartilage is

BACKGROUND: Transcutaneous osseointegration following amputation (TOFA) is an alternative to standard prosthesis by directly anchoring a metal implant to the skeletal residuum. However, there is a paucity of data concerning whether TOFA is safe and reasonable for patients with lower limb amputation to manage complications of sepsis. The primary aim of this study was to record adverse post-TOFA events for nine patients (15 limbs) whose index amputations were performed to manage sepsis-related complications. The secondary aim was to compare the pre- and post-TOFA mobility and quality of life (Qo

INTRODUCTION: The use of an aortic bioprosthesis is on the rise in younger patients with severe aortic stenosis despite the risk of accelerated structural valve degeneration (SVD). In the search for an optimal valve substitute that would not be prone to SVD, the INSPIRIS bioprosthesis represents a promising solution to lowering the risk of SVD. Here, we report the 1-year outcomes of the INSPIRIS RESILIA aortic bioprosthesis in a population of young patients who underwent aortic valve replacement.
METHODS: In this prospective single-center study, we included all consecutive patients receiving

BACKGROUND AND AIMS: Transvenous lead extraction (TLE) is increasingly performed for malfunction or infection of cardiac implantable electronic devices (CIED). Active mechanical rotational sheaths facilitate extraction and are available in short and long sizes, but it is unclear how many patients short active mechanical extraction sheaths (SAMES) are sufficient for TLE, and in which patients they may facilitate TLE. This study aimed to evaluate the incidence and predictors of successful lead extraction using only SAMES.
METHODS: We retrospectively analyzed patients who underwent TLE at the Un
