DQB1*0602 and DQA1*0102 (DQ1) are better markers than DR2 for narcolepsy in Caucasian and black Americans.

In the present study, we tested 19 Caucasian and 28 Black American narcoleptics for the presence of the human leucocyte antigen (HLA) DQB1*0602 and DQA1*0102 (DQ1) genes using a specific polymerase chain reaction (PCR)-oligotyping technique. A similar technique was also used to identify DRB1*1501 and DRB1*1503 (DR2). Results indicate that all but one Caucasian patient (previously identified) were DRB1*1501 (DR2) and DQB1*0602/DQA1*102 (DQ1) positive. In Black Americans, however, DRB1*1501 (DR2) was a poor marker for narcolepsy. Only 75% of patients were DR2 positive, most of them being DRB1*15

Age-dependent peripheral dissemination of α-synuclein seeds: mechanistic basis for blood-based early detection of Parkinson's disease.

Early diagnosis of Parkinson's disease (PD) before extensive neurodegeneration is a critical unmet need, largely due to the absence of sensitive, minimally or non-invasive biomarkers for the prodromal stage. Here, we primarily use α-synuclein (α-Syn) preformed fibril (PFF)-induced mouse models to investigate the mechanistic basis of peripheral α-Syn dissemination, and additionally perform a small exploratory analysis of a serum-based α-Syn seed amplification assay (SAA) in a prodromal human cohort as proof-of-concept. In the PFF model, misfolded α-Syn propagated broadly throughout the CNS and

Clinical profile of idiopathic normal pressure hydrocephalus with reduced myocardial sympathetic nerve function.

INTRODUCTION: Findings of reduced cardiac sympathetic nerve function and pareidolic illusions have been useful for identifying Lewy body disease (LBD). Therefore, we aimed to investigate whether these findings are associated with a high prevalence of LBD-related symptoms and poor shunt responsiveness in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: We retrospectively analyzed 104 iNPH patients who underwent preoperative 123I-metaiodobenzylguanidine myocardial scintigraphy and the Pareidolia test. We divided the patients into the abnormal cardiac scan (AC) group and t

Lack of genetic evidence for a role of SLC25A46 in alpha-synucleinopathies.

BACKGROUND: The SLC25A46 gene encodes a mitochondrial carrier protein previously implicated in neuropathy and optic atrophy. Biallelic variants in SLC25A46 have been described in patients with Parkinson's disease (PD) with optic atrophy, but the evidence supporting a role in PD remains limited. OBJECTIVE: To assess whether SLC25A46 variants contribute to PD, REM sleep behavior disorder (RBD), or Dementia with Lewy Bodies (DLB). METHODS: We examined common variants using four representative PD genome-wide association studies (GWAS) and an RBD GWAS and applied Summary-data-based Mendelian Rand

Biomarkers of neurodegenerative parkinsonisms: From current clinical to future biological definitions - literature review and our experience.

Over recent decades, the search for sensitive and specific biomarkers of degenerative parkinsonisms has intensified. So also has the number of clinical trials aimed at disease modification and the subsequent need for improved recruitment of participants in the earliest possible stages and with the highest diagnostic certainty. Also increasing in number have been searches for ways to determine target engagement and biological effect, along with tracking of disease progression or its modification. With post mortem neuropathological confirmation remaining the most definite diagnostic category for

Risk of Parkinson's disease in older people with tremor: A population-based prospective study.

BackgroundTremor is prevalent among older adults and is suggested to be associated with the subsequent development of Parkinson's disease (PD). However, prospective evidence for the association is scarce.ObjectiveThis population-based prospective study aimed to estimate PD risk and to explore predictive factors for PD conversion in community-dwelling older people with tremor.MethodsThe study was based on the Beijing Longitudinal Study on Aging (BLSA) II. Tremor was assessed using a validated self-administered method. Participants without parkinsonism and with any tremor were followed for an av

Quantitative measurements of α-synuclein seeds in CSF inform diagnosis of synucleinopathies.

Diagnosing α-synucleinopathies and assessing target engagement in trials is hindered by the lack of reliable biomarkers. Here, we introduce a first-in-kind quantitative, highly sensitive, and disease-specific diagnostic assay, named Seeding Amplification ImmunoAssay (SAIA), developed and validated to detect synucleinopathy-linked disorders. To this end, we used wide range of specimens, including 38 brain homogenates (BH) and 559 cerebrospinal fluid (CSF) samples from subjects with diverse synucleinopathy disorders, non-synucleinopathy diseases, idiopathic REM sleep behavior disorder (iRBD), an

[Classification and Clinical Characteristics of Dementia].

Community-based surveys conducted in Japan investigating the prevalence of dementia and its underlying causes revealed that dementia of Alzheimer's type (DAT) is the most common, followed by vascular dementia (VaD), dementia with Lewy bodies (DLB), mixed dementia, and other conditions including frontotemporal lobar degeneration (FTLD). Accurate differential diagnosis of these disorders requires clarification of their clinical characteristics. The initial symptoms of DAT typically include recent memory loss, episodic memory impairment, and temporal disorientation. Behavioral and psychological s

Brainstem changes causing reversible RBD in patients with spontaneous intracranial hypotension: a longitudinal neuroimaging study.

OBJECTIVE: This study aimed to investigate the prevalence of REM sleep behavior disorder (RBD) in patients with spontaneous intracranial hypotension (SIH) and longitudinally assess the effects of epidural blood patch (EBP) treatment on brainstem structures using a neuroimaging approach. METHODS: Twenty-two participants (10 SIH patients and 12 controls) underwent 3-Tesla Magnetic Resonance Imaging (MRI) scans. Midbrain and pons areas were measured on T1-weighted scans at baseline in both groups and three months after the first EBP in SIH patients to determine any MRI structural changes. The RB

Lack of Efficacy of JNJ-18038683 on Cognitive Impairment in Patients With Stable Bipolar Disorder.

BACKGROUND: The serotonin type 7 (5-HT7) receptor is one of 14 5-HT receptors. It has received attention for its possible role in mood disorders and cognition. The 5-HT7 receptor antagonist, JNJ-18038683, has been reported to be effective in rodent models of depression and REM sleep. Also, 5-HT7 receptor blockade has been postulated to be a key component of cognitive enhancement in a number of drugs. Bipolar disorder (BD) usually endures cognitive impairment (CI); however, no treatment for CI in BD has been approved. This study aimed to evaluate the efficacy of JNJ-18038683 to improve the CI o

A Review on the Clinical Diagnosis of Multiple System Atrophy.

Multiple system atrophy (MSA) is a rare, adult-onset, progressive neurodegenerative disorder with major diagnostic challenges. Aiming for a better diagnostic accuracy particularly at early disease stages, novel Movement Disorder Society criteria for the diagnosis of MSA (MDS MSA criteria) have been recently developed. They introduce a neuropathologically established MSA category and three levels of clinical diagnostic certainty including clinically established MSA, clinically probable MSA, and the research category of possible prodromal MSA. The diagnosis of clinically established and clinical

Prefrontal network dysfunctions in rapid eye movement sleep behavior disorder.

INTRODUCTION: Resting-state functional connectivity magnetic resonance imaging (rsfcMRI) of rapid eye movement (REM) sleep behavior disorder (RBD) may provide an early biomarker of α-synucleinopathy. However, few rsfcMRI studies have examined cognitive networks. To elucidate brain network changes in RBD, we performed rsfcMRI in patients with polysomnography-confirmed RBD and healthy controls (HCs), with a sufficiently large sample size in each group. METHODS: We analyzed rsfcMRI data from 50 RBD patients and 70 age-matched HCs. Although RBD patients showed no motor signs, some exhibited auton

Features in essential tremor and the development of Parkinson's disease vs. parkinsonism.

BACKGROUND: Patients with essential tremor (ET) are at higher risk to develop Parkinson's disease (PD). Recent studies suggest that propranolol (common treatment for ET) can augment pathologic expression of alpha-synuclein. We studied features associated with the development of Parkinson's disease with antecedant essential tremor (ET-PD) compared with ET-plus with parkinsonism (PK). DESIGN: Retrospective case series from a tertiary movement disorders center including patients with ET and PD, found to have ET-PD or ET-plus (PK). RESULTS: We analyzed two groups: (1) ET-plus (PK) (n = 33) and (

Functional neuroimaging and clinical features of drug naive patients with de novo Parkinson's disease and probable RBD.

INTRODUCTION: The association between Parkinson Disease (PD) and REM sleep behavior disorder (RBD) has been related to a specific, malignant clinical phenotype. Definite RBD diagnosis requires video-polysomnography that is often unfeasible. A malignant clinical PD-RBD phenotype could be expected also in PD patients with probable RBD. Aim of this cross-sectional study was to evaluate whether a more severe neuropsychological and functional neuroimaging phenotype can be identified in PD patients with probable RBD. METHODS: Thirty-eight de novo, drug naïve PD patients underwent a first-line clini

Increased plasma level of tumor necrosis factor α in patients with narcolepsy in Taiwan.

BACKGROUND: Narcolepsy is a neuropsychiatric disorder characterized by excessive daytime sleepiness, cataplexy, hypnagogic hallucinations, and abnormal rapid eye movement (REM) sleep. Tumor necrosis factor α (TNF α) and its cognate receptors have been reported to be involved in the pathophysiology of narcolepsy in addition to the HLA antigen system. Our study aimed to determine if the TNF-α system was associated with narcolepsy in our patients. METHODS: We first measured the plasma level of TNF α in 56 narcoleptic patients and 53 control subjects using a highly sensitive enzyme-linked immunos
