Region-Based Segmentation of Lymph Node Metastases in Whole-Slide Images of Colorectal Cancer: A Pilot Clinical Study.

BACKGROUND: Digital technologies and artificial intelligence (AI) are transforming medical diagnostics, particularly in pathology. This study presented a two-stage computer vision model designed to detect colorectal cancer metastases in whole slide images (WSIs) of lymph nodes. METHODS: We developed a classification-segmentation pipeline optimized for both accuracy and efficiency. The model was trained on 108 WSIs and evaluated on 554 WSIs collected from two institutions using Leica Aperio AT2 and Hamamatsu NanoZoomer S360 scanners. RESULTS: The classification model achieved a recall of 1.0

Inhalation exposure and particle deposition across 16 nonhuman primate airway models using computational fluid-particle dynamics for allometric extrapolation.

BACKGROUND AND OBJECTIVE: Nonhuman primates are indispensable surrogate models for humans due to their evolutionary proximity; however, ethical concerns and practical constraints are increasingly limiting their use. Computational fluid-particle dynamics (CFPD) offers a promising alternative for inhalation exposure studies; however, previous studies typically used small sample sizes with limited model validation. Therefore, this study aims to elucidate inhalation exposure in monkeys through a large cohort analysis and to identify relationships between humans and monkeys by allometric comparison

State-Space Modeling of Real-Time Visual Biofeedback and Late-Stance Belt-Speed Modulation for Quantifying Error- and Use-Dependent Learning of Gait Propulsion in Individuals Post-Stroke.

This study developed an exploratory, model-constrained state-space model to characterize how real-time visual biofeedback (VB) and late-stance, phase-specific belt modulation (propulsion-facilitating, PF) were associated with improvements in affected-limb propulsion following stroke. Twelve individuals with chronic stroke participated in a single-session experiment conducted on the Adaptive Propulsion Enhancement eXperience (APEX) system, which utilizes an instrumented split-belt treadmill. The system continuously displays the affected-limb anterior ground reaction force (AGRF) while modulatin

High-fidelity postmyocardial infarction ventricular tachycardia simulation for intraprocedure ablation guidance.

BACKGROUND: Postinfarction ventricular tachycardia (VT) arises from reentrant circuits within partially conductive myocardial tissue. Despite antiarrhythmic medication, many patients require ablation therapy, yet recurrence rates remain as high as 50% at 5 years. Virtual-heart arrhythmia ablation targeting (VAAT) methods aim to improve outcomes by simulating VT activity preoperatively, but they are computationally intensive and often lack validation against clinical VT and electrocardiographic (ECG) data. OBJECTIVE: This study aimed to optimize a VAAT pipeline for speed and clinical relevance

Comparative analysis of five AI platforms for mandibular canal segmentation on CBCT images.

OBJECTIVES: Accurate mandibular canal (MC) identification on cone-beam computed tomography (CBCT) is vital to prevent inferior alveolar nerve injury during oral and maxillofacial procedures. Manual segmentation is time-consuming and operator-dependent, while artificial intelligence (AI) offers automated, reproducible alternatives. This study compared the accuracy of automated MC segmentation across five AI platforms using standardized quantitative and qualitative evaluations. METHODS: A total of 120 anonymized CBCT scans (240 MCs) were analyzed using five fully automated AI-based segmentation

An uncertainty-aware vision transformer-BiLSTM Bayesian framework for reliable clinical decision support using chest X-rays.

Despite recent progress in deep leaning for medical image analysis, there are still issues of reliability, interpretability, and uncertainty estimation that must be addressed for the safe adoption of deep learning into clinical practice. Conventional CNN- based models have achieved excellent accuracy but have also typically shown prediction confidence levels that are too extreme, limiting confidence in very high-stakes diagnostic environments. To tar- get these deficiencies, we propose a suitable competence-based uncertainty-aware infrastructure that combines Vision Transformers, BiLSTM modeli

Probing Atrial Substrate With the Triple Extrastimulus Protocol: Impact of Variability on Electrophysiological Metrics.

Pulmonary vein (PV) isolation is often insufficient to prevent recurrence in persistent atrial fibrillation, highlighting the need to characterize non-PV substrates sustaining reentry. Hidden slow-regions, unmasked under premature stimulation, have emerged as potential arrhythmogenic markers. This study integrates high-density clinical mapping with structurally personalized left-atrial (LA) simulations to assess whether in silico models can reproduce conduction changes elicited by a triple-extrastimulus (TE) protocol. Ten anatomically detailed LA models were built from CT imaging and CARTO map

Automatic anatomical classification of esophagogastroduodenoscopy images using deep convolutional neural networks.

The use of convolutional neural networks (CNNs) has dramatically advanced our ability to recognize images with machine learning methods. We aimed to construct a CNN that could recognize the anatomical location of esophagogastroduodenoscopy (EGD) images in an appropriate manner. A CNN-based diagnostic program was constructed based on GoogLeNet architecture, and was trained with 27,335 EGD images that were categorized into four major anatomical locations (larynx, esophagus, stomach and duodenum) and three subsequent sub-classifications for stomach images (upper, middle, and lower regions). The p

Total midgut duplication: a ticking time bomb.

A day-old neonate presented with bowel obstruction and an abdominal mass. Exploratory laparotomy revealed complete tubular midgut duplication from duodeno-jejunal junction up to terminal ileum which was communicating with the ileum distally. At the proximal end, another 5×5 cm duplication cyst was identified and excised. Postoperatively, complaints were relieved. During follow-up, 99m-Tc-pertechnetate-SPECT scan showed ectopic gastric mucosa in lower abdomen and in the right hemithorax. CECT-chest showed a 3×3 cm foregut duplication cyst, but there were no respiratory symptoms. While being pla

Cardiac ischemia-insights from computational models.

BACKGROUND: Complementary to clinical and experimental studies, computational cardiac modeling serves to obtain a comprehensive understanding of the cardiovascular system in order to analyze dysfunction, evaluate existing, and develop novel treatment strategies. OBJECTIVES: We describe the basics of multiscale computational modeling of cardiac electrophysiology from the molecular ion channel to the whole body scale. By modeling cardiac ischemia, we illustrate how in silico experiments can contribute to our understanding of how the pathophysiological mechanisms translate into changes observed

Sensitivity of reentrant driver localization to electrophysiological parameter variability in image-based computational models of persistent atrial fibrillation sustained by a fibrotic substrate.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia, causing morbidity and mortality in millions worldwide. The atria of patients with persistent AF (PsAF) are characterized by the presence of extensive and distributed atrial fibrosis, which facilitates the formation of persistent reentrant drivers (RDs, i.e., spiral waves), which promote fibrillatory activity. Targeted catheter ablation of RD-harboring tissues has shown promise as a clinical treatment for PsAF, but the outcomes remain sub-par. Personalized computational modeling has been proposed as a means of non-invasiv

Heart rate variability (HRV) during virtual reality immersion.

The goal of the study was assessment of the hour-long training involving handling virtual environment (sVR) and watching a stereoscopic 3D movie on the mechanisms of autonomic heart rate (HR) regulation among the subjects who were not predisposed to motion sickness. In order to exclude predispositions to motion sickness, all the participants (n=19) underwent a Coriolis test. During an exposure to 3D and sVR the ECG signal was continuously recorded using the Holter method. For the twelve consecutive 5-min epochs of ECG signal, the analysis of heart rate variability (HRV) in time and frequency d

Fracture of bony lamellae within the frontal recess after balloon catheter dilatation.

BACKGROUND: The precise manner in which balloon catheter dilatation (BCD) alters the dimensions of the frontal recess and frontal sinus outflow tract is not well characterized. The purpose of this investigation was to determine whether BCD within the frontal recess is associated with reproducible patterns of fracture in bony lamellae, to characterize changes between pre- and postintervention measurements of the frontal sinus outflow tract, and to compare the degree of change seen with endoscopic Draf I dissection. METHODS: Eight cadaver heads underwent pre- and postintervention endoscopic vis

Real-time fMRI feedback training may improve chronic tinnitus.

OBJECTIVES: Tinnitus consists of a more or less constant aversive tone or noise and is associated with excess auditory activation. Transient distortion of this activation (repetitive transcranial magnetic stimulation, rTMS) may improve tinnitus. Recently proposed operant training in real-time functional magnetic resonance imaging (rtfMRI) neurofeedback allows voluntary modification of specific circumscribed neuronal activations. Combining these observations, we investigated whether patients suffering from tinnitus can (1) learn to voluntarily reduce activation of the auditory system by rtfMRI

Implantable cardioverter defibrillator algorithms: status review in terms of computational cost.

In recent decades, implantable cardioverter defibrillators (ICDs) have improved substantially, becoming the treatment of choice for patients at high risk of life-threatening arrhythmias. Nevertheless, inappropriate shock therapy for non-ventricular arrhythmias is still a problem. Extending the ICD battery lifetime demands very low power consumption, which is obtained at very low microprocessor clock frequencies. Currently, some high-performance algorithms remain beyond the computational capabilities of ICDs. Future ICDs with higher computing power will permit the implementation of computationa

Imaging in CNS lupus.

The diagnosis of neuropsychiatric systemic lupus erythematosus (NPSLE) is complex not only on account of the heterogeneous nature of neurological presentation but also because of the difficulty of differentiating lupus-related pathology from other neuropsychiatric diseases. Magnetic resonance imaging (MRI) remains the gold standard for the non-invasive assessment of NPSLE but there are problems, both with sensitivity and specificity. Both T(2) quantitation and the use of gadolinium have shown promise in differentiating acute from chronic lesions. Nonetheless, the lack of sensitivity of convent

National plan for reliable tuberculosis laboratory services using a systems approach. Recommendations from CDC and the Association of Public Health Laboratories Task Force on Tuberculosis Laboratory Services.

Since the mid-1990s, public health laboratories have improved tuberculosis (TB) test performance, which has contributed to the resumption of the decline in TB incidence in the United States. However, to eliminate TB in the United States, further improvements are needed in laboratory services to support TB treatment, prevention, and control. A critical step is the development of an integrated system that ensures prompt and reliable laboratory testing and flow of information among laboratorians, clinicians, and TB-control officials. Challenges to developing such a system include 1) establishing
