Pathological fractures of the proximal femur due to solitary bone cyst: classification, methods of treatment.

Fractures are a very important issue in a child's orthopedic pathology. Neglected a good amount of time, being considered "not too serious", or "rare", having better and faster healing methods and not leaving sequels, like in the case of adults, a child's fractures remain an important chapter of traumatology in general. Because of the raising prevalence of child osteoarticular traumas, as well as new less invasive treatment methods, this theme is always to date. The paper analyzes particular cases of bone fractures that appeared due to minor traumas, on bones with a high brittleness, localized

[Descending intramedullary nailing for the treatment of displaced supracondylar humeral fractures in children].

BACKGROUND: Supracondylar humeral fractures are the most common lesions in childhood. Severely displaced fractures are commonly treated by crossed K-wire osteosynthesis. Such stabilized fractures require a cast and often involve postoperative complications such as iatrogenic lesions of the n. ulnaris and secondary displacements, sometimes leading to a consecutive cubitus varus. This study analyzed possible advantages of elastic stabile intramedullary nailing (ESIN) with postoperative release of motion. PATIENTS AND METHOD: All children aged 1-14 years suffering from a supracondylar humeral fra

[The interposition nail in diaphyseal bone defects and arthrodesis nail in failed knee-joint replacement].

Our clinical experience with different constructions of interposition and arthrodesis nails, used in 3 and 12 patients respectively, is presented. It has shown that cemented intramedullary nailing for the stabilization of long bones and the knee joint using an approach from the site of the defect is a useful additional means in reconstructive bone and joint surgery.

Preventing varus collapse in proximal humerus fracture fixation: 90-90 dual plating versus endosteal fibular allograft strut.

INTRODUCTION: Screw cut out and varus collapse are the most common complication of locked plate fixation of proximal humerus fractures. The purpose of this study was to compare dual plating and endosteal fibular allograft struts as augmentation strategies to prevent varus collapse. MATERIALS AND METHODS: A trapezoidal osteotomy was created at the metaphysis to create a 2-part proximal humerus model in 18 paired shoulder specimens. Each specimen was assigned to group A, B, or C and was fixed with either a lateral locking plate, a lateral locking plate and anterior one-third tubular plate in an

Locked Intramedullary Nailing versus Compression Plating for Stable Ulna Fractures: A Comparative Study.

BACKGROUND: Isolated ulna shaft fractures (USFs) are a relatively uncommon, but significant, injury. For unstable USF treatment, open reduction and internal fixation (ORIF) is the gold standard, while for stable USFs several procedures were described. The aim of this study is to compare the outcomes in patients with stable USFs treated by either ORIF or intramedullary nail (IMN). METHODS: According to their surgical treatment, 23 eligible USF-affected patients were divided into ORIF (14 subjects) and IMN (nine subjects) groups. The subjects underwent postoperative clinical follow-up at 1, 3,

Surgical management of angulated femoral intramedullary nails associated with closed fractures: A systematic review of the literature.

BACKGROUND: Femoral nail bending is a rare complication of intramedullary (IM) fixation of femoral diaphyseal fractures. Published literature regarding this injury pattern has thus far been limited to case reports or case series, thus no universally accepted surgical treatment strategy has been developed. METHODS: A systematic review was conducted using the Pubmed/MEDLINE and Scopus/EMBASE databases. A standardized template was used to extract data including author, year of publication, patient demographics, degree of angulation, mechanism of injury, time since initial procedure to reinjury,

Radial neck fracture in children: anatomic and functional results of Metaizeau technique.

Fractures of the radial neck accounts for 1% of all childhood fractures and 5% to 10% of childhood traumatic lesions involving the elbow. Intramedullary percutaneous nail reduction (Metaizeau technique) is considered the most effective surgical technique. The purpose of this study was to identify the main clinical features of radial neck fracture in children and to evaluate the anatomical and functional results of the Metaizeau technique. In this retrospective study, we evaluated 22 patients under the age of 16 who were treated for radial neck fracture at the orthopedic and trauma surgery depa

Three-part humeral head fractures treated with a definite construct of blocked threaded wires: finite element and parametric optimization analysis.

BACKGROUND: Mini open reduction and percutaneous fixation of three-part humeral head fracture with blocked threaded wires has demonstrated functional results similar to locking plates or intramedullary nails but with significantly lower major complication rate. In the context of three-part humeral head fractures, we performed a parametric optimization through a finite element analysis of a recently published construct to verify if the encouraging clinical results can be supported by a more rigorous investigation from a mechanical viewpoint. MATERIALS AND METHODS: The 2-dimensional geometry of

The efficacy of continuous-flow cryo and cyclic compression therapy after hip fracture surgery on postoperative pain: design of a prospective, open-label, parallel, multicenter, randomized controlled, clinical trial.

BACKGROUND: The number of hip fractures and resulting post-surgical outcome are a major public health concern and the incidence is expected to increase significantly. The acute recovery phase after hip fracture surgery in elder patients is often complicated by severe pain, high morphine consumption, perioperative blood loss with subsequent transfusion and delirium. Postoperative continuous-flow cryocompression therapy is suggested to minimize these complications and to attenuate the inflammatory reaction that the traumatic fracture and subsequent surgical trauma encompass. Based on a pilot stu

A Comparative Study Evaluating the Outcomes of Primary Cemented Bipolar Hemiarthroplasty Versus Proximal Femoral Nail Osteosynthesis for Unstable Intertrochanteric Femur Fractures in the Elderly Patient.

OBJECTIVE: The objective of this study was to compare the outcomes of primary cemented bipolar hemiarthroplasty with proximal femoral nail (PFN) osteosynthesis as a management modality in unstable fractures of the intertrochanteric femur. INTRODUCTION: Intertrochanteric fractures also referred to as extracapsular fractures occur in the area between the greater and lesser trochanters. Osteosynthesis is a prompt and efficient method for managing stable intertrochanteric fractures. Utilizing osteosynthesis procedures other than intramedullary fixation to address unstable intertrochanteric fractu

Position statement: management of proximal humerus fractures.

We sought to compare outcomes and reoperation rates for the surgical treatment of proximal humerus fractures (excluding head-splitting fractures, fracture-dislocations, and isolated greater-tuberosity fractures) in men and women older than 60 years. We searched MEDLINE, Embase, and Cochrane through to Feb. 1, 2022, and included all English-language randomized trials comparing operative versus nonoperative treatment; open reduction and internal fixation (ORIF) with locking plate versus intramedullary nail; arthroplasty versus ORIF; and reverse shoulder arthroplasty versus hemiarthroplasty. Lower limb inequality treatment with subtrochanteric femoral shortening osteotomy fixed with intramedullary nail.

BACKGROUND: The aim of the study is to analyze the results of treating the lower limb inequality with subtrochanteric femoral shortening osteotomy fixed with interlocking intramedullary nail. The analyzed material includes 14 patients aged 16 to 36 (mean 25), in whom femoral shortening osteotomy fixed with interlocking nail was performed. In all patients the indication for this therapeutic method was lower limb inequality caused by femur shortening and accompanied by contraindication for femoral lengthening procedure. Limb length inequality ranged from 1.5 to 6.0 cm (mean 3.8). The follow-up p

Leg lengthening with a motorized nail in adolescents : an alternative to external fixators?

UNLABELLED: Leg lengthening by external fixation is associated with various difficulties. We evaluated eight adolescent patients who underwent leg lengthening with a motorized intramedullary lengthening device. We asked whether this method could reduce the time of hospitalization and rehabilitation and whether the incidence of complications commonly associated with external fixators could be reduced. We compared our preliminary results with those from other reports, with a focus on leg length achieved, time of rehabilitation, and rate of complications. The average leg-length discrepancy was 3.

Adult respiratory distress syndrome, pneumonia, and mortality following thoracic injury and a femoral fracture treated either with intramedullary nailing with reaming or with a plate. A comparative study.

Multiply injured patients (an Injury Severity Score of 17 points or more) who were admitted to one of two level-I regional trauma centers between 1983 and 1994 because of a fracture of the femoral shaft with a thoracic injury (an Abbreviated Injury Scale score of 2 points or more) or without a thoracic injury were studied retrospectively. The patient populations and the protocols for the treatment of trauma were similar at the two centers; however, the centers differed with regard to the technique that was used for acute stabilization of the fracture of the femoral shaft. [Malpractice in the treatment of supracondylar humeral fractures in children - experience of the arbitration office of the Northern German Medical Boards].

INTRODUCTION: Arbitration offices ("Schlichtungsstellen") in Germany are expert panels for the extrajudicial resolution of malpractice claims. The performance of arbitration panel proceedings ("Schlichtungsverfahren") is based on the German medical and insurance jurisdiction. In Germany, and in the United States likewise, malpractice claims involving children concern in most cases fracture treatment followed by appendicitis. Out of 242 panel proceedings with the background of fracture treatment in children malpractice was confirmed in 144 cases (60%). The overall ratio: number of confirmed mal

Locking plate fixation in distal metaphyseal tibial fractures: series of 79 patients.

Open reduction and internal fixation in distal tibial fractures jeopardises fracture fragment vascularity and often results in soft tissue complications. Minimally invasive osteosynthesis, if possible, offers the best possible option as it permits adequate fixation in a biological manner. Seventy-nine consecutive adult patients with distal tibial fractures, including one patient with a bilateral fracture of the distal tibia, treated with locking plates, were retrospectively reviewed. The 4.5-mm limited-contact locking compression plate (LC-LCP) was used in 33 fractures, the metaphyseal LCP in

Retrospective Cohort Study Comparing Rates of Symptomatic Implants Requiring Removal between Standard versus Low-Profile Tibial Nail Interlocking Screws.

This study aimed to compare the rates of symptomatic implants requiring removal between intramedullary tibia nails using standard (SD) versus low-profile (LP) screws. We hypothesize that LP screws will demonstrate lower rates of symptomatic screw removal compared with SD interlocking screws with no difference in secondary outcomes. We performed a retrospective cohort study of all patients 18-89 years old who underwent IMN of tibia fractures from November 2018 to February 2023 at a single, academic level one trauma center. The primary outcome analyzed was symptomatic screws requiring removal. S

Burying flexible intramedullary nails improves implant survival and reduces tip-related complications in nonambulatory children with cerebral palsy.

Closed diaphyseal femoral fractures in children with cerebral palsy classified as Gross Motor Function Classification System (GMFCS) levels IV/V are frequently managed with flexible intramedullary nails (FIN). This study aimed to evaluate the survival of FIN with distal ends buried within the distal femoral metaphyseal cortex. Secondary outcomes included the occurrence of complications, especially those requiring reoperation. Children were divided into two groups based on the location of the distal nail. In group A, the nail ends were buried within the distal femoral metaphyseal cortex. In gro
