OBJECTIVE: This study aimed to observe the distribution of individuals with ABO discrepancies among Han Chinese patients in Nantong, China, and to analyze the molecular mechanisms of weak ABO expression.
METHODS: ABO group tests were conducted on patient samples collected in three Tertiary hospitals in Nantong from January 1, 2021 to March 31, 2025. Discrepancies were confirmed by other serological methods. If the discrepancy was confirmed, ABO genotype was determined to detect which alleles were present.
RESULTS: In this study, 412,513 patients were routinely typed for ABO, among whom 1465

Matching donor and recipient blood groups based on red blood cell (RBC) surface ABO glycans and antibodies in plasma is crucial to avoid potentially fatal reactions during transfusions. Enzymatic conversion of RBC glycans to the universal group O is an attractive solution to simplify blood logistics and prevent ABO-mismatched transfusions. The gut symbiont Akkermansia muciniphila can degrade mucin O-glycans including ABO epitopes. Here we biochemically evaluated 23 Akkermansia glycosyl hydrolases and identified exoglycosidase combinations which efficiently transformed both A and B antigens and

Savin et al. report a retrospective analysis of ABO-incompatible red cell transfusions in the United Kingdom, France and Germany. In spite of varied strategies for bedside preventative measures, all three countries continue to identify preventable errors leading to ABO-incompatible red cell transfusions. This report highlights the ongoing challenge of human error as a factor in the persistence of preventable mistransfusions. Commentary on: Savin et al. Frequencies and causes of ABO-incompatible red cell transfusions in France, Germany and the United Kingdom. Br J Haematol 2025; 206:726-734.

Kidney transplant candidates are blood group incompatible with roughly one out of three potential living donors. We compared outcomes after ABO-incompatible (ABOi) kidney transplantation with matched ABO-compatible (ABOc) living and deceased donor transplantation and analyzed different induction regimens. We performed a retrospective study with propensity matching and compared patient and death-censored graft survival after ABOi versus ABOc living donor and deceased donor kidney transplantation in a nationwide registry from 2006 till 2019. 296 ABOi were compared with 1184 center and propensity

BACKGROUND: Liver transplantation usually requires blood transfusion, and a red blood cell (RBC) antibody screen is essential for the prevention of a hemolytic reaction. Since proper ABO-compatible grafts are lacking, ABO-incompatible living donor liver transplantation (ABO-i LDLT) with desensitization is a feasible therapy. Desensitization includes intravenous rituximab injection and plasmapheresis before surgery.
CASE: A 60-year-old female was diagnosed with hepatitis B virus-related hepatocellular carcinoma and planned for ABO-i LDLT. She tested positive in a RBC antibody screen over two y

OBJECTIVES: ABO fetomaternal red blood cell incompatibility (ABO FMI) induces an immune hemolysis after fetal transfer of hemolyzing maternal anti-A or anti-B. ABO hemolytic disease (ABO HD) remains the most frequent cause of severe and early jaundice in newborns. High levels of unconjugated hyperbilirubinemia may induce acute and chronic neurological complications. Severe hyperbilirubinemia can be prevented by first-line phototherapy (PT) treatment, but exchange transfusion (ET) is required if treatment is not effective, even if ET is linked with high hemodynamic, infectious, gastrointestinal

Since 1989 we have been collecting dry-platelets on a routine basis. Dry-platelets are those collected along with 25-30 ml of contaminating plasma cell with separators such as the Amicus, AS 104 and the Excel Pro. Platelets are resuspended in non plasma media for storage and for at least 60 hours their viability and functionality are not impaired. In this article we report on two hemolytic crises determined by two O Rh D + units of single donor platelets (SPD) taken from the same donor in a double-apheresis session. The two split units were administered to two A Rh D + patients suffering from

BACKGROUND: Hemolytic transfusion reaction is considered a rare complication of platelet transfusion. If minor ABO incompatibility exists (donor antibody directed against recipient's red cells [plasma-incompatible platelets]), however, the antibodies present in the plasma of platelets may cause acute hemolysis. A retrospective study was performed to identify possible hemolysis related to the transfusion of plasma-incompatible apheresis platelets.
STUDY DESIGN AND METHODS: Acute hemolysis due to low-titer anti-A present in group O apheresis platelets transfused to a group A patient is reported

The course and the treatment of the patient M.F., who after kidney transplantation, produced autoreactive antierythrocyte antibodies is described. The patient M.F. was B, Rh positive and the kidney donor was 0, Rh positive. On the 12th posttransplantation day severe hemolysis, anemia and bilirubinemia were observed. Beside anti-A, autoreactive anti-B antibodies were discovered in the B group patient and the direct antiglobulin test (DAT) was positive. The patient was treated with several transfusions of 0 blood group washed red cells, immunosuppressive drugs (Cyclosporin, Methylprednisolone) a

The effect of traditional blue-light and riboflavin combined with blue-light was compared in newborns affected by ABO incompatibility, admitted for exchange transfusion. During the period of preparation for the intervention 14 patients were treated with blue light alone and 14 patients with riboflavin combined with phototherapy. A single dose of 10 mg/kg riboflavin was administered intravenously. The duration of treatment was three hours in both groups. The effect of phototherapy was markedly enhanced by the additional riboflavin, by the end of the 3-hour period a significant fall of serum bil

BACKGROUND: The ABO blood group system is vital for red blood cell antigens. Weakened expression of ABO antigens often leads to misidentification of blood types, challenging transfusion safety.
MATERIALS AND METHODS: Blood samples from 44 cases underwent serological analysis using microcolumn agglutination and tube tests. To investigate genetic variations, we sequenced the promoter region and exons 1-7 of the ABO gene. We also examined the relationship between ABO haplotypes and the presence of rare alleles. We used PolyPhen-2 to predict the structure and function of glycosyltransferase B (GT

Allogeneic hematopoietic stem cell transplantation (HSCT) is a curative option for a variety of malignant and non-malignant hematological and congenital diseases. Due to the fact that the human leukocyte antigen system is inherited independently of the blood group system, approximately 40-50% of all HSCTs are performed across the ABO blood group barrier. The expected immune-hematological consequences after transplantation of an ABO-mismatched stem cell graft are immediate and delayed hemolytic complications due to presence of isohemagglutinins or passenger lymphocyte syndrome. The risks of the

Immune haemolysis following renal transplantation has been reported and known causes include infection, medication and metabolic disturbances (1,2). Autoimmune haemolysis after renal transplantation secondary to ABO minor mismatch is an uncommon but important cause that should be considered in the differential diagnosis of post-transplantation haemolysis. A case of haemolytic anaemia caused by graft versus host antibody formation is presented. We suggest that direct Coomb's test should be done as a routine in all cases of ABO mismatch renal transplantation and red cells compatible with both do
