Subcutaneous emphysema of the upper extremity after elbow arthroscopy.

Subcutaneous emphysema of the upper extremity is rare. Crepitation on physical examination and visible gas on radiographs raise the concern of gas gangrene due to gas-producing bacteria. Rapid establishment of a differential diagnosis is necessary to initiate proper treatment. We present a case of subcutaneous emphysema after elbow arthroscopy caused by a noninfectious genesis. A 59-year-old woman with loose bodies in her left elbow due to mild degenerative joint disease and restricted range of motion was offered an elbow arthroscopy with removal of loose bodies and arthrolysis. Postoperativel

Dopamine and carotid body function in the newborn lamb.

The effect of dopamine on the acute ventilatory response to hypoxia was investigated in four newborn lambs studied on the 1st day of postnatal life and eight lambs studied between 7 and 12 days of age. Studies were accomplished during nonrapid-eye-movement sleep in unanesthetized tracheotomized animals. Changes in minute ventilation (VE/kg), tidal volume, and frequency induced by intravenous bolus injection of dopamine (10 micrograms/kg) or saline control were assessed while animals were breathing room air or N2, before and after carotid body denervation (CBD). Dopamine depressed resting venti

A new personal badge monitor for organic vapors.

A new passive monitor for organic vapors has been developed for the determination of the time-weighted average (TWA) concentration of contaminants in air. This personal dosimeter type badge monitor collects the organic vapors through the mechanism of molecular diffusion and adsorption onto an activated carbon collection element. After exposure, the activated carbon is removed from the device and analyzed using gas chromatographic techniques outlined in NIOSH Physical and Chemical Analysis Method (P&CAM) 127. Current NIOSH standards for measuring the concentration of organic vapors in the atmos

Factors influencing radiation exposure during the extracorporeal shock wave lithotripsy.

A prospective evaluation of 89 consecutive sessions of extracorporeal shock wave lithotripsy (ESWL) was undertaken to try and find the best way of minimising the amount of exposure to radiation. Forty-two patients were randomly allocated to undergo ESWL treatment by experienced surgeons (group A), and 47 to undergo the treatment by inexperienced surgeons (group B). The mean calculated entrance radiation exposure was 3.01 rads (group A: 2.64 (0.97) rads, range 1.00-4.48, group B: 3.38 (0.86) rads, range 1.11-5.75). Among factors that influenced radiation exposure, the tissue: air ratio should b

Immediate Effect of Continuous Positive Airway Pressure Therapy on Sleep and Respiration in Patients with Multiple System Atrophy and Sleep-Disordered Breathing.

BACKGROUND: Sleep-disordered breathing (SDB; including stridor and sleep apnea syndromes) is frequent in multiple system atrophy (MSA), but the immediate effect of continuous positive airway pressure (CPAP) therapy is incompletely determined. OBJECTIVE: We sought to evaluate the acute effect and safety of CPAP therapy on SDB and sleep architecture, as well as the clinical characteristics of nonresponders to CPAP therapy. METHODS: The measures of 63 consecutive patients with MSA who underwent a video-polysomnography during two consecutive nights (a first night in ambient air, a second night w

The effect of the COVID-19 lockdown on malaria transmission in South Africa.

BACKGROUND: For a country such as South Africa which is targeting malaria elimination, mobile and migrant populations pose a substantial risk to importation of malaria parasites. It has been hypothesized that halting cross-border movement of mobile and migrant populations will decrease the importation of malaria, however this option is not a politically, operationally, and financially viable prospect. It has social impacts as well, since families live on either side of the border and preventing travel will challenge family ties. Due to the COVID-19 pandemic and closure of ports of entry (land

Three-dimensional eye movement analysis during caloric stimulation used to test vertical semicircular canal function.

HYPOTHESIS AND BACKGROUND: Quantitative caloric testing is considered to be one of the most sensitive parameters in the diagnosis of peripheral vestibular disorders. In the past, because of limitations in the methods, the evaluation of the caloric response was restricted to mainly lateral semicircular canal functions. In this study, the authors tried to extend caloric testing to the function of all semicircular canals by using three-dimensional (3-D) analysis techniques. METHODS: The authors studied in seven normal subjects 3-D eye movement responses to air caloric of the right ear with the s

A computational framework for agent-based assessment of multiple environmental exposures.

BACKGROUND: Agent-based assessment of long-term personal exposure to environmental factors accounts for spatio-temporal variation in exposures along daily activity tracks of individuals. Application up to nationwide study populations requires integration of large data sets on environmental factors, personal behavior, and socio-economic status, as well as propagating uncertainties in these inputs to personal exposure values. OBJECTIVE: To develop and illustrate a methodology and software framework for agent-based personal exposure assessment for large cohorts, including uncertainty assessment.

Nasal continuous positive airway pressure inhibits gastroesophageal refluxes in the preterm lamb.

Gastroesophageal refluxes (GERs) are universal in newborns and may induce deleterious consequences, especially in preemies. We have previously shown that nasal continuous positive airway pressure (nCPAP) inhibits GERs in full-term lambs, complementing similar results in adult humans. The effect of high-flow nasal cannula (HFNC) on GERs, however, remains unknown. This study aimed to assess the effects of nCPAP and HFNC on GERs in our preterm lamb model. Eleven preterm lambs born 14 days before full-term (∼34 wk of human gestation) were studied. Esophageal pH-impedancemetry and polysomnography r

Medial Wall Fracture and Orbital Emphysema Mimicking Inferior Rectus Entrapment in a Child.

Orbital emphysema is commonly associated with fractures of the orbital floor or medial wall. The air often dissipates spontaneously, but rarely can cause increased intraocular pressure and even loss of vision. Entrapment of the extraocular muscles can also occur with orbital fractures and may require prompt treatment in the pediatric patient due to the risk muscle ischemia. Both conditions can cause diplopia due to restriction of eye movement and differentiation of the two etiologies is important to prevent unnecessary surgical exploration. Identification and prompt management of raised intrao

Field Efficacy of Seven Nighttime Ultra-Low-Volume Spraying Strategies Against Aedes albopictus in Urban Green Spaces - Shenzhen City, Guangdong Province, China, October-November 2025.

INTRODUCTION: Daytime adult mosquito control in urban parks may disrupt public activities and constrain equipment movement. We evaluated whether nighttime ultra-low-volume (ULV) operations could reduce adult Aedes albopictus host-seeking activity in complex vegetation. METHODS: Descriptive field comparisons were conducted at three urban green-space sites in Shenzhen, China. Seven strategies were evaluated: ground ULV-only, unmanned aerial vehicle (UAV)-only, light- or wind-disturbance-assisted UAV, dual-insecticide UAV, and two coordinated ground-air regimens. Adult host-seeking activity was

Does severe and chronic esophageal distension by air (observed in SGB and GB) affect the esophageal mucosa integrity (MNBI).

BACKGROUND: Supragastric belching (SGB) and aerophagia are behavioral disorders characterized by air induced esophageal distension. SGB is known to be associated with Gastro Esophageal Reflux Disease (GERD). Low Mean Nocturnal Baseline Impedance (MNBI) values support GERD diagnosis. We aimed to assess if chronic esophageal distension by air affects the esophageal mucosa integrity by assessing changes in MNBI. METHODS: In a single-center database study, we searched retrospectively for patients with a diagnosis of pathological SGB (n = 146) or aerophagia (n = 34) based on impedance-pH reflux mo

Pulsatile tinnitus: a review of the literature and an unusual case of iatrogenic pneumocephalus causing pulsatile tinnitus.

BACKGROUND: Pulsatile tinnitus is frequently attributed to identifiable and treatable causes, in contrast to the more common subjective non-pulsatile tinnitus. It usually originates from vascular structures as a result of either increased blood flow or lumen stenosis; atherosclerotic carotid or subclavian artery disease; arterial, venous, or arteriovenous malformations, fistulas, or dissection; and paragangliomas. Other causes have also been reported, with often unclear pathophysiology. OBJECTIVE: The aim of this paper is to present a case of pulsatile tinnitus secondary to iatrogenic pneumoc

Environmental pollutant SO₂ exposure affects trophoblast function involving an ER stress pathway.

Sulphur dioxide (SO₂) is a toxic air pollutant with deleterious effects on the respiratory, cardiovascular, endocrine and reproductive systems. SO₂, being freely diffusible, can cross the placenta and reach the fetal system. Previous studies have reported the association between SO₂ exposure and adverse pregnancy and fetal outcomes without much emphasis on its molecular aspects. The present study has investigated the adverse consequences of SO₂ derivatives on the two key trophoblast functions critical for placentation: invasion and fusion. SO₂ exposure inhibited the fusogenic potential of BeWo

Abnormal petrous apex aeration. Review of 12 cases.

OBJECTIVE: Abnormal aeration of the petrous bone, which may be responsible for some clinical symptoms, is an incidental radiological finding. It has been reported that people with abnormal aeration at the petrous bone are more prone to develop complications. In an attempt to make clear the clinical importance of a giant air cell at the petrous apex or abnormal aeration of this region, a prospective analysis has been planned. METHOD: 430 temporal bone CTs taken for diagnostic imaging only between 1992-2000 have been reviewed and 12 cases with petrous air cell were selected. Aeration was measur

Computational fluid dynamics simulation wall model predicting air temperature of the nasal passage for nonhuman primates.

OBJECTIVES: Nasal passages adjust the temperature of inhaled air to reach the required body temperature for the lungs. The nasal regions of primates including humans are believed to have experienced anatomical modifications that are adaptive to effective conditioning of the atmospheric air in the habitat for a given species. Measurements of the nasal temperature are required to understand the air-conditioning performance for a given species. Unfortunately, repeated direct measurements within the nasal passage have been technically precluded in most nonhuman primates. MATERIALS AND METHODS: Co
