Mobility impairments in crash-involved older drivers.

OBJECTIVE: To evaluate potential associations of impairments in physical function with motor vehicle crash involvement in older drivers. METHODS: Case participants were randomly selected residents of Mobile County, Alabama, greater or less than 65 years old who had sustained an at-fault motor vehicle crash in 1996. Similarly selected crash-free controls were frequency matched to cases on gender and age. Self-report data on demographic variables, medical conditions, medications, driving exposure, and function were collected by telephone interviewers. RESULTS: Relative to crash-free subjects, …

Neuropathology of occipital horn syndrome.

Occipital horn syndrome, formerly known as Ehlers-Danlos syndrome type IX or X-linked cutis laxa, is an allelic variant of Menkes' syndrome. Although the clinical symptomatology and systemic pathology findings have been well described in occipital horn syndrome, the neuropathology has not previously been reported. A kindred affected by the X-linked occipital horn syndrome is followed at the University of Alabama at Birmingham. A severely mentally retarded dysmorphic man, who died at the age of 26 years, never gained the ability to walk or talk. Among other findings at autopsy, the patient had …

Changing patterns in the diagnosis of hypertrophic pyloric stenosis.

The records of 216 infants who had surgical correction of hypertrophic pyloric stenosis between 1980 and 1984 at the Children's Hospital of Alabama were reviewed. A significant increase in the reliance on upper gastrointestinal roentgenographic series and abdominal sonography for confirmation of the diagnosis of hypertrophic pyloric stenosis was noted in our patients when compared to previous reports. Despite the preoperative presence of a palpable pyloric mass in 192 (89%) of the patients, 174 (81%) had a diagnostic imaging procedure. Similar high rates of imaging studies were noted when the …

Is there an association between pre-existing sleep apnoea and the development of glaucoma?

AIM: To determine if sleep apnoea is associated with an increased risk of developing glaucoma. METHODS: This was a nested case-control study. Patients seen at the Veterans Affairs Medical Center (BVAMC) in Birmingham, Alabama, with newly diagnosed glaucoma (cases) between 1997 through 2001 were selected (n = 667) and age matched with non-glaucomatous controls (n = 6667). Patient information was extracted from the BVAMC data files containing demographic, clinical, and medication information. An index date was assigned to the glaucoma subjects corresponding to the time of diagnosis. Patients wh…

Antimicrobial susceptibility of Edwardsiella ictaluri.

Edwardsiella ictaluri isolates collected over the past 7 yr from outbreaks of disease in fish occurring in different geographic areas were screened against 37 antimicrobial agents. Edwardsiella ictaluri were found to be susceptible to most agents active against gram negative bacteria such as aminoglycosides, cephalosporins, the "newer" penicillins, quinolones, tetracyclines, chloramphenicol, nitrofurantoin, and potentiated sulfonamides. Resistance was observed against colistin, sulfonamides, and several agents regarded as effective for gram positive bacteria. There was no evidence of unusual a

Post-streptococcal glomerulonephritis following pyoderma.

Studies of the epidemiology of acute glomerulonephritis (AGN) following pyoderma reported over the past 15 years have been reviewed. Investigations in Alabama, at Red Lake in Minnesota, and in Trinidad proved of special interest because they contribute new information concerning the natural history of streptococcal skin infections and the role of such infections in AGN. Interesting contrasts between streptococcal infections of the skin and those of the throat are now apparent. Compared with pharyngeal infections, skin infections are more common in young preschool children, are caused by differ…

Building the cancer research workforce through implementation of the RACE21 undergraduate cancer research and education program.

BACKGROUND: Cancer remains the second leading cause of death in the state of Alabama and disproportionately affects communities that are rural, of lower socioeconomic status, and individuals with high-risk, predisposing conditions such as obesity, cardiovascular disease, advanced age, and diabetes. One method to address the increased burden of cancer is to expand the cancer research and clinical workforce. The Roadmap for America's Cancer Explorers for the 21st Century (RACE21) is a National Cancer Institute-funded Youth Enjoy Science program at the University of Alabama at Birmingham (UAB) an…

Using Community Health Advisors to Increase Lung Cancer Screening Awareness in the Black Belt: a Pilot Study.

Disease stage at the time of diagnosis is the most important determinant of prognosis for lung cancer. Despite demonstrated effectiveness of lung cancer screening (LCS) in reducing lung cancer mortality, early detection continues to elude populations with the highest risk for lung cancer death. Consistent with the national rate, current screening rate in Alabama is dismal at 4.2%. While public awareness of LCS may be a likely cause, there are no studies that have thoroughly evaluated current knowledge of LCS within the Deep South. Therefore, we measured LCS knowledge before and after receiving…

Development of an Emergency Department-Based Intervention to Expand Access to Medications for Opioid Use Disorder in a Medicaid Nonexpansion Setting: Protocol for Engagement and Community Collaboration.

BACKGROUND: The opioid epidemic has disproportionately impacted areas in the Appalachian region of the United States. Characterized by persistent Medicaid nonexpansion, higher poverty rates, and health care access challenges, populations residing in these areas of the United States have experienced higher opioid overdose death rates than those in other parts of the country. Jefferson County, Alabama, located in Southern Appalachia, has been especially affected, with overdose rates over 2 times greater than the statewide average (48.8 vs 19.9 overdoses per 10,000 persons). Emergency departments…

CHAMPS+: Protocol for a Randomized Trial of a CHW-Delivered mHealth Intervention to Improve Viral Suppression in the U.S. Deep South.

Viral suppression (VS) disparities persist among persons living with HIV in the U.S. Deep South because of structural barriers and disengagement from care. Community health workers and mobile health (mHealth) interventions may address multilevel challenges, yet real-world evidence is limited. The aim of our study is to evaluate effectiveness and implementation of CHAMPS+, a CHW-delivered mHealth intervention to improve VS. CHAMPS+ is a multisite randomized controlled trial with mixed-method implementation evaluation. A total of 420 virally unsuppressed or out-of-care adults with HIV in Alabama…

Housing as Sanctuary and Constraint: A Qualitative Study of Mental Health Among Older Adults in Government-Assisted Housing.

While government-assisted housing programs primarily aim to reduce financial burden for low-income older adults, depression and anxiety rates remain elevated among residents, suggesting that housing environments shape mental health through pathways beyond economic relief. This study examined how housing environments influence mental health among older adults through a socioecological framework. We conducted semi-structured interviews with 16 older adults (ages 60-86) residing in government-assisted housing in rural Alabama between July and September 2024. Using hybrid thematic and framework an…

Barriers to Medication Adherence and Mild Cognitive Impairment Among African Americans with Persistently Uncontrolled Hypertension: A Cross-sectional Analysis from the Southeastern Collaboration Trial.

BACKGROUND: Mild cognitive impairment (MCI) is frequently undetected and may lead to medication nonadherence. We examined whether more reported barriers to medication adherence could signal the presence of MCI. OBJECTIVE: We developed a count of barriers (0, 1, 2, and ≥3) and determined the independent association between a greater number of barriers and odds of MCI. SETTING: Rural primary care practices in Alabama and North Carolina. PARTICIPANTS: One thousand two hundred seventy-nine participants of the Southeastern Collaboration to Improve Blood Pressure Control trial; all were African A…
