Progress towards development of a vaccine for amebiasis.

The application of molecular biologic techniques over the past decade has seen a tremendous growth in our knowledge of the biology of Entamoeba histolytica, the causative agent of amebic dysentery and amebic liver abscess. This approach has also led to the identification and structural characterization of three amebic antigens, the serine-rich Entamoeba histolytica protein (SREHP), the 170-kDa subunit of the Gal/GalNAc binding lectin, and the 29-kDa cysteine-rich protein, which all show promise as recombinant antigen-based vaccines to prevent amebiasis. In recent studies, an immunogenic dodeca…

[The diagnosis and surgical treatment of intestinal amebiasis].

Ninety-one patients with intestinal amebiasis (IA) were studied. All had been long suffering from IA due to the fact that it had not been diagnosed in time. Indications for emergency surgery were profuse hemorrhage, perforation of amebic ulcers, gangrene, and toxic dilatation of the large intestine. Planned operations were made in pseudopolyposis, strictures of the large intestine. Surgical treatment was performed along with specific therapy. Seven patients died due to late diagnosis, ineffective therapy and profound systemic and regional changes caused by these factors. It is necessary to dif…

New concepts regarding the pathogenesis of amebiasis.

Our understanding of the pathogenesis of amebiasis has progressed significantly since Dr. Braude's description of the major clinical syndromes of amebic liver abscess. His hypothesis that invasive amebic strains must be resistant to complement-mediated lysis has been confirmed. We have also shown that Entamoeba histolytica activates complement by a unique mechanism, i.e., cleavage of C3 by an extracellular cysteine proteinase. Cysteine proteinases are important virulence factors encoded by at least three genes; one gene, acp1, is unique to invasive strains. The amebic cysteine proteinases are …

[Amebiasis in Transcaucasia and in mountainous desert locales].

The history of the amebiasis investigation in Transcaucasia is presented. 16 cases of amebiasis are described. Data for 1982-1988 on the amebiasis in the moorland are given (41 fatal case).

Hepatopulmonary amebiasis--a review of 40 cases.

Forty cases of hepatopulmonary amebiasis secondary to hepatic amebiasis are reported. In an endemic area of amebiasis, this entity should always be borne in mind, as the frequent reports of hepatopulmonary amebiasis gives evidence of the magnitude of the problem. Diagnosis is mainly based on a palpable tender liver and/or lower intercostal tenderness, the fluoroscopic and radiological findings, and therapeutic response to dehydroemetine.

[Epidemiological data on amebiasis in New Caledonia].

The present study permits to set up the following conclusions: A) The hygienic level of New-Caledonia and Dependences, concerning both running water feeding and installation of water-closets inside housings, is extremely disparate according to the parishes. On that account, the hygienic level is low on the East Coast of the "Grande-Terre" and in the islands. Most of European inhabitants are found in the district of Noumea whereas Melanesians live in tribes mainly on the East Coast and in the islands. Therefore, there is a real difference of hygienic level between the two main ethnic groups.

Massive necrosis and perforation of the colon in amebiasis.

A successfully treated case of acute fulminating necrotizing amebic colitis characterized by signs of toxemia, septicemia and peritonitis is reported. Early diagnosis and staged surgical procedures apparently lower the mortality. Intensive antiamebic therapy should be instituted as soon as amebiasis is confirmed, otherwise surgical therapy is likely to fail.
