Comparison of Hybrid Abdominal Flap versus Homodigital Neurovascular Island Flap for Fingertip Amputation.

Amputation at the level of the distal phalanx is a common hand injury and is normally treated with replantation. However, if the level of injury is distal or the vessels of the stump have been crushed by injury, replantation cannot be a viable option. The aim of this study is to evaluate the functional and aesthetic outcomes of the "hybrid abdominal flap", which consists of a random-pattern abdominal skin flap and an autologous bone graft. A retrospective analysis was performed on fingertip amputation patients from March 2019 to April 2021. The patients were managed by either hybrid abdominal

The reorganization of the motor system in primates after the loss of a limb.

As a result of an extensive search, we were able to obtain a limited number of macaque monkeys, New World squirrel monkeys, and prosimian galagos years after they had received a therapeutic amputation of a forelimb or hindlimb as a treatment for injury. Forelimb or hindlimb regions of primary motor cortex, M1, were stimulated in these primates with microelectrodes using amounts of current just sufficient to evoke a movement. Results were compared to those obtained from normal primates or from M1 contralateral to the intact limb in the same primates. In all cases, more sites in M1 contralateral

Prosthesis applications and challenges in children with earthquake-related amputations.

BACKGROUND: Amputations are among the most important traumatic injuries caused by earthquakes. However, data on amputee children and prosthesis application is quite limited in the literature. The aim of the study is to evaluate the injury-related data, stump problems, prosthesis application, difficulties and complications experienced with prosthesis during follow-up of children with 2023 Kahramanmaraş earthquake-related limb loss. PATIENTS AND METHODS: Sociodemographic and injury-related data, pre-amputation and post-amputation interventions, prosthesis application, current prosthetic problem

Syme's amputation: a retrospective review of 10 cases.

Amputation at the level of the ankle joint is a valuable, yet underused procedure for a variety of conditions affecting the foot and ankle. The procedure can provide a comfortable and durable stump that allows a lower extremity amputee to function with minimal disability. The indications, surgical technique, and postoperative prosthetic considerations are reviewed and discussed. Emphasis is placed on the preoperative considerations deemed essential for a successful outcome based on the senior author's experience of 10 cases performed for a wide variety of pathologies. Three case illustrations

[Amputation and prosthesis attachment of the lower extremities].

Approximately 61,000 amputations are performed in Germany per year. In most cases the lower limbs are affected. The reasons for amputations are arteriosclerosis, diabetes mellitus, severe infections, tumors and complex trauma to the extremities. A decision must be made concerning whether a salvage procedure or amputation is appropriate, specially after trauma. In cases where the need for amputation is clear, the site of amputation needs to be planned in advance with the aim of creating a stump which allows sufficient prosthetic attachment. Adjuvant pain therapy is mandatory, especially in orde

Traumatic amputations. A vietnam experience.

Ninety-two acute combat amputations were performed in 70 casualities of the Vietnam conflict. In the 67 surviving patients with 83 stumps, primary closure was done in 24 limbs (30%) and delayed closure in 11 more. Closed cases were observed 2 weeks and no failures noted. In selected patients early suture of war amputations can be practiced with a satisfactory result.

Traumatic forequarter amputation.

Traumatic forequarter amputation is a rare and devastating injury in which the arm, scapula, clavicle, and pectoral muscles are avulsed from the body by a tremendous force. The case of a man who survived this injury is presented. Replantation at this level has not been successful: treatment recommended consists of control of hemorrhage, wound debridement, ligation of vessels and nerves, coverage of their exposed stumps with muscle, unless excess contamination is present; electrical or cosmetic prostheses can be fitted later.

Symmetrical peripheral gangrene reconstruction after lower limb amputation.

Symmetrical peripheral gangrene (SPG) is a complication of septicemia, characterized by progressive skin lesions, which can result in wide necrosis of all the extremities. Severe necrosis is often responsible of disfiguring consequences. It is important to limit the amputation and to provide a stable and functional soft tissue coverage of the stumps. Limited amputation and free flap reconstruction may help to prevent tissue loss and to provide an effective prosthetization and a prompt rehabilitation. We present a clinical case of a 60-year-old male admitted in our department for lower limbs ne

Stump and phantom limb pain.

Recent literature suggests that phantom pain and stump pain have closely related physiologic mechanisms and that treatments frequently overlap. Decreased blood flow in the residual limb is related to burning and tingling phantom and stump pain, whereas spasms in major muscles of the residual limb precede cramping phantom and stump pain. There is little support for psychological mechanisms underlying the vast majority of chronic phantom and stump pain problems, but these mechanisms can exacerbate both acute and chronic pain. It is critically important to educate patients about the process of am

An effective method of skin traction in A-K guillotine amputation.

In modern orthopedic surgery, there are still indications for the guillotine amputation of the thigh. Immediate postoperative management by effective skin traction can promote rapid healing and eliminate the need for revision surgery. Revision surgery necessitates further shortening of the stump; skin grafting likewise can be avoided as a secondary procedure. Most skin tractions for above-the-knee amputations loosen within 24 hours. The suggested method of applying skin traction on A-K amputations, if applied properly, will be effective for periods ranging from five to seven days.

Anterior cerebral artery amputation and salvage repair of internal carotid artery tear: technical case report.

OBJECTIVE: We describe a novel technique used to repair an unanticipated tear of the internal carotid artery (ICA) requiring anterior cerebral artery (ACA) amputation to allow primary repair of the arteriotomy. CLINICAL PRESENTATION: A 59-year-old woman underwent an orbitozygomatic craniotomy to treat a large, suprasellar, thyroid-stimulating hormone-secreting adenoma. During resection, an incidental ICA tear occurred opposite the exit of the middle cerebral artery. TECHNIQUE: After an ICA tear on the wall opposite the middle cerebral artery occurred, clips were placed on the ICA, middle cer

Unilateral granuloma annulare in association with pyoderma gangrenosum and chronic lymphocytic leukemia.

Granuloma annulare (GA) is a fairly common inflammatory skin condition with a range of clinical subtypes. We describe an unusual case of unilateral GA confined to the thigh on a previously amputated limb. A man in his 80s with a past medical history of below-knee amputation of the left leg owing to severe leg ulcers from pyoderma gangrenosum, chronic lymphocytic leukemia, and dyslipidemia developed a slowly spreading eruption on the distal stump spreading proximally. On physical examination, he had numerous non-scaly violaceous papules and annular plaques from the stump to the lateral, medial,

Salvage of a below-the-knee amputation with a fillet of foot free flap: a 15-year follow-up.

In the setting of traumatic injury, obtaining suitable coverage to salvage a below-the-knee amputation has been performed by using a variety of tissue flaps, including the fillet of foot free flap. This flap is a free tissue transfer that utilizes the soft tissues of the foot for stump coverage. In this report, we present a 15-year follow-up of a patient with a fillet of foot free flap to demonstrate its applicability and long-term efficacy. We also discuss technical aspects for performing this procedure. In this follow-up, we have found the flap to be extremely durable, even when placed in th

Transcutaneous electrical nerve stimulation after major amputation.

We studied the effect of transcutaneous electrical nerve stimulation (TENS) on stump healing and postoperative and late phantom pain after major amputations of the lower limb. A total of 51 patients were randomised to one of three postoperative treatment regimens: sham TENS and chlorpromazine medication, sham TENS only, and active low frequency TENS. There were fewer re-amputations and more rapid stump healing among below-knee amputees who had received active TENS. Sham TENS had a considerable placebo effect on pain. There were, however, no significant differences in the analgesic requirements

[Amputation below the fracture site in pseudarthrosis complicated by infection or vascular lesion].

In cases of fractures of the shaft of a bone complicated by non-union, infection and vascular and/or neurological lesions, amputation may be the ultimate possible treatment. In some cases, an amputation at the level of the fracture site would produce too short a stump for a normal prosthesis. The authors consider that, in this type of lesion, it may be wise to preserve some of the distal portion of the bone below the site of the fracture, even in cases of infected non-union. They performed this technique in seven cases, the distal fragment of the bone being covered by an osteomyoplasty, infect

A new treatment of painful amputation neuroma: a preliminary report.

Two cases of painful amputation neuroma treated by a new method are described. A vascularized island transfer from a less important finger was used, but the accompanying nerve was divided and its distal end sutured to the proximal end of the nerve in the recipient finger, after excision of the neuroma. Four years and two years later, no clinical evidence of neuroma was present at either amputation stump or donor site.

Predictive Value of Preoperative Inflammatory and Nutritional Indices for Revision Surgery After Major Lower Extremity Amputation in Patients With Wagner Stage 5 Diabetic Foot.

BACKGROUND Major lower extremity amputation is a severe complication of diabetic foot disease, leading to high morbidity and repeat surgeries. Although systemic inflammatory and nutritional markers have been studied as prognostic factors in diabetic foot ulcers, their role in predicting revision surgery after major amputation is unclear. This study aims to evaluate the link between preoperative inflammatory and nutritional indices and revision surgery after major lower extremity amputation. MATERIAL AND METHODS This multicenter retrospective study reviewed medical records of patients with Wagn

Brachial plexus neuroma mimicking local recurrence of malignant mesenchymal tumor after forequarter amputation.

A stump neuroma is an attempt for the repair of a nerve following amputation. This article presents a case of a 60-year-old woman who was suspected of having a local recurrence on the chest wall following amputation of the left arm for a malignant mesenchymal tumor. The tumor did not show any invasion to adjacent structures thereby any necessity for chest wall resection and reconstruction. The patient underwent local excision of the tumor with reamputation of the branches of the lower brachial plexus, subclavian artery, and proximal one third of the clavicle. The tumor was eventually diagnosed
