Effect of sub-anesthetic dose of esketamine on oculocardiac reflex of pediatric strabismus surgery in total intravenous anesthesia: study protocol for a randomized controlled trial.

BACKGROUND: The oculocardiac reflex (OCR) is the most common complication in pediatric strabismus surgery, and its main clinical manifestation is bradycardia. Esketamine can activate the sympathetic nervous system, leading to an indirect cardiovascular stimulation effect, which results in increased blood pressure and a faster heart rate. The aim of this study is to explore whether a sub-anesthetic dose of esketamine can effectively inhibit the oculocardiac reflex during strabismus surgery. METHOD: This prospective, single-center, randomized, double-blinded trial will enroll 204 pediatric pati

Effectiveness of intracuff alkalinized lidocaine associated with intravenous dexamethasone in reducing laryngotracheal morbidity in children undergoing general anesthesia for tonsillectomy: a randomized controlled trial.

BACKGROUND: Postoperative sore throat is one of the main postoperative complaints in patients undergoing tonsillectomy. As the primary outcome, we aimed to determine whether endotracheal tube cuffs filled with alkalinized lidocaine are associated with a lower incidence of postoperative sore throat and anesthesia emergence phenomena in children undergoing tonsillectomy or adenotonsillectomy. We also assessed the potential additional benefits of IV dexamethasone in reducing postoperative laryngotracheal morbidity. METHODS: This is a clinical prospective, randomized, controlled trial. Patients w

Effects of Volatile Versus Intravenous Anesthesia on Cerebral Vasospasm in Open Surgical and Endovascular Procedures for Aneurysmal Subarachnoid Hemorrhage: A Systematic Review and Meta-Analysis.

The selection of anesthetic agents during aneurysm repair may have an impact on the prognosis of patients with aneurysmal subarachnoid hemorrhage (SAH). In this systematic review and meta-analysis, we compared the effects of volatile-based anesthesia with those of total intravenous anesthesia (TIVA) on perioperative outcomes in SAH patients. A comprehensive literature search was performed in PubMed, EMBASE, Web of Science, and the Cochrane Library through December 2024, yielding 9 studies (comprising 4 randomized controlled trials [RCTs] and 5 cohort studies) with a total of 1459 participants.

Effects of surgery and propofol-remifentanil total intravenous anesthesia on cerebrospinal fluid biomarkers of inflammation, Alzheimer's disease, and neuronal injury in humans: a cohort study.

BACKGROUND: Surgery and anesthesia have been linked to postoperative cognitive disturbance and increased risk of Alzheimer's disease. It is not clear by which mechanisms this increased risk for cognitive disease is mediated. Further, amyloid β production has been suggested to depend on the sleep-wake cycle and neuronal activity. The aim of the present study was to examine if cerebrospinal fluid (CSF) concentrations of a number of biomarkers for Alzheimer's disease-related processes, including amyloid β, neuronal injury, and inflammation, changed over time during intravenous anesthesia in surgi

Comparative efficacy of clonidine versus magnesium sulfate as an adjunct to lignocaine in intravenous regional anesthesia for postoperative analgesia: A prospective, randomized, double-blind study.

BACKGROUND AND AIMS: Intravenous regional anesthesia (IVRA) is a very good technique to be used in unstable patients. Various adjuvants have been added, but till date, there is no ideal adjuvant. Clonidine is one of the most widely used adjuvants in IVRA. However, it has many side effects. Hence, the search continues for a better adjuvant. The aim of the present study was to compare the efficacy of clonidine versus MgSO4 as an adjunct to lignocaine in IVRA for postoperative analgesia and to compare their side effect profile. MATERIAL AND METHODS: This prospective double-blind randomized contr

Complex Oncologic Surgery in a Nonagenarian With Severe Aortic Stenosis: Integration of Enhanced Recovery After Surgery Protocol, Total Intravenous Anesthesia, and Advanced Hemodynamic Monitoring.

The number of elderly patients presenting for major surgery is steadily increasing, and perioperative management in this population remains challenging because of frailty, multiple comorbidities, and reduced physiological reserve. Severe aortic stenosis represents a particularly high-risk condition due to fixed stroke volume and sensitivity to even brief episodes of hypotension or tachycardia. We describe the perioperative course of a 92-year-old woman who underwent combined right radical nephroureterectomy, cystectomy, and total hysterectomy with bilateral salpingo-oophorectomy, using a multi

Comparison of intraoperative hemodynamic parameters between propofol- and remimazolam-based total intravenous anesthesia in patients undergoing robotic gynecologic surgery: a randomized controlled trial protocol.

INTRODUCTION: Propofol-based total intravenous anesthesia (TIVA) with remifentanil is widely used for general anesthesia but is frequently associated with hypotension and bradycardia. Remimazolam, an ultra-short-acting benzodiazepine, provides rapid onset and offset, with the additional advantage of reversal by flumazenil, and may result in less hemodynamic depression. Previous randomized studies have largely focused on the induction phase or the incidence of hypotension, with limited data on continuous intraoperative hemodynamic changes during laparoscopic gynecologic surgery. The study is de

Comparative effectiveness of intravenous remifentanil, epidural anesthesia, and a 2-step analgesic approach for external cephalic version: a large prospective single-center cohort study.

BACKGROUND: Among the analgesic strategies used during external cephalic version, neuraxial anesthesia is the only approach that has consistently demonstrated higher vaginal delivery rates. However, neuraxial analgesia encompasses a heterogeneous range of techniques, and higher complication rates have been reported compared with other approaches. OBJECTIVE: This study aimed to compare the success of external cephalic version, modes of delivery, maternal pain, and complications using 3 strategies: intravenous analgesia with remifentanil, epidural anesthesia, and a stepwise approach in which ep

Spontaneous Respiration Using Intravenous Anesthesia and High-Flow Nasal Oxygen (STRIVE-Hi) for Prolonged Rigid Bronchoscopy in a Patient With Functional Single-Lung Physiology.

Rigid bronchoscopy in patients with functional single-lung physiology presents a considerable anesthetic challenge because interruption of spontaneous ventilation may rapidly result in severe hypoxemia. Although spontaneous respiration using intravenous anesthesia and high-flow nasal oxygen (STRIVE-Hi) has been described as a technique that facilitates prolonged apneic oxygenation, evidence regarding its application to preserve spontaneous ventilation during prolonged rigid bronchoscopy remains limited. A 55-year-old man with complete obstruction of the left main bronchus secondary to post-int

Intraoperative immunomodulatory effects of sevoflurane versus total intravenous anesthesia with propofol in bariatric surgery (the OBESITA trial): study protocol for a randomized controlled pilot trial.

BACKGROUND: Obesity is associated with a chronic systemic inflammatory process. Volatile or intravenous anesthetic agents may modulate immune function, and may do so differentially in obesity. However, no study has evaluated whether these potential immunomodulatory effects differ according to type of anesthesia in obese patients undergoing laparoscopic bariatric surgery. METHODS/DESIGN: The OBESITA trial is a prospective, nonblinded, single-center, randomized, controlled clinical pilot trial. The trial will include 48 patients with a body mass index ≥ 35 kg/m2, scheduled for laparoscopic bari
