[Superselective spinal anesthesia for saphenectomy intervention. Comparison of 2 concentrations of hyperbaric bupivacaine].

BACKGROUND: High concentrations of intrathecal local anaesthetics may be neurotoxic. The purpose of this study was to compare the features of superselective spinal anaesthesia (ASS) obtained with an equivalent dose (5 mg) of 0.5% (available now also in Italy) and 1% hyperbaric bupivacaine. METHODS: A prospective study was carried out on 100 patients undergoing saphenectomy, who were randomized into two groups to receive either 0.5% bupivacaine 1 ml (group A) or 1% bupivacaine 0.5 ml (group B). The anaesthetic solution was injected 1 ml/min at L2-L3 with an atraumatic needle. During dural punc

Unexpected cardiac arrest during spinal anesthesia: a closed claims analysis of predisposing factors.

Fourteen cases of sudden cardiac arrest in healthy patients who received spinal anesthesia were discovered in a preliminary review of 900 closed insurance claims for major anesthetic mishaps. All patients were resuscitated from the intraoperative cardiac arrest, but six suffered such severe neurologic injury that they died in hospital. Of the eight survivors, only one exhibited sufficient neurologic recovery to allow independence in daily self-care. In view of the unexpected nature of the cardiac arrests, as well as the ultimate severity of injury, these cases were analyzed in detail to determ

Leg compression for preventing hypotension after spinal anesthesia in elderly hip fracture patients.

PURPOSE: Hip fracture (HF) is a common geriatric trauma resulting in a substantial rate of mortality and morbidity. Previous studies have shown that the application of leg compression significantly reduced the incidence of perioperative hypotension (PeH) and hypotension-related complication in obstetric surgery. The objective of this study was to evaluate the effect of medical compression stocking for prevention of PeH in elderly patients undergoing HF surgery. METHODS: Sixty patients were randomized in 2 groups: compression stocking (CS) group and no compression stocking (NCS) group (n = 30

Hypobaric unilateral spinal anesthesia with multimodal analgesia enhances recovery in total knee arthroplasty.

BACKGROUND: Total knee arthroplasty (TKA) is an effective treatment for end-stage knee osteoarthritis, but postoperative pain and delayed recovery remain challenges. This study aimed to evaluate the effects of hypobaric unilateral fine-needle spinal anesthesia combined with multimodal analgesia (MMA) on postoperative recovery in TKA patients. METHODS: A randomized controlled triple-blind trial enrolled 118 patients scheduled for TKA between January 2022 and June 2023. Patients were divided into three groups: hypobaric fine-needle spinal anesthesia (Group A, n=40), isobaric fine-needle spinal

Noninvasive Hemodynamic Assessment with Impedance Cardiography During Spinal and Epidural Anesthesia in Obstetrics.

Background/Objectives: Spinal anesthesia (SA) for cesarean section and epidural analgesia (EA) for vaginal delivery induce hemodynamic changes that may compromise maternal and fetal safety. In this observational, hypothesis-generating study, we used impedance cardiography (ICG) to characterize maternal hemodynamic responses to EA for labor versus SA for cesarean delivery and to describe hemodynamic profiles associated with commonly used local anesthetic and vasopressor regimens. Methods: In this observational study, 132 women at term were included (52 with epidural analgesia (EA), 80 with spin

Delayed Onset of Spinal Anesthesia in a Chronic Opioid User Undergoing Cesarean Section.

Delayed onset of spinal anesthesia can pose challenges during cesarean section (CS) in patients with a history of chronic opioid use. We present a case of delayed onset of spinal anesthesia in a patient with chronic opioid use undergoing CS, highlighting the clinical implications and management strategies. Delayed onset of spinal anesthesia in chronic opioid users can result from neurophysiological adaptations and pharmacokinetic interactions in the central nervous system. Previous studies have suggested chronic opioid users display a delayed onset and shorter duration of action of neuraxial b

Predictive factors of epidural rescue in combined spinal-epidural anesthesia for cesarean delivery.

BACKGROUND: Combined spinal-epidural (CSE) is often recommended for cesarean deliveries expected to have prolonged duration. This study aimed to identify factors associated with the need for epidural rescue anesthesia during CSE for cesarean section, to better refine clinical indications for this technique. METHODOLOGY: This retrospective cohort study was conducted from 2020 to 2022 at the maternity of Toulouse University Hospital, France. Data were collected from 230 patients who underwent CSE for cesarean delivery. The mean age was 34.3 ± 5.6 years, 69.1% had previous cesarean sections, and

Acute Cauda Equina Syndrome Following Spinal Anesthesia During Cesarean Section: A Case Report.

This case report aims to discuss the prompt recognition and the possible mechanism of acute cauda equina syndrome (CES) following intrathecal bupivacaine administration for cesarean section. A 30-year-old primigravida developed sudden-onset bilateral lower-extremity weakness, numbness, and paresthesia, along with urinary and bowel incontinence, after an uncomplicated cesarean delivery under spinal anesthesia. Magnetic resonance imaging revealed engorgement and gadolinium enhancement of the cauda equina nerve roots, consistent with a diagnosis of CES. The patient was immediately started on high

Intrathecal Digoxin Administration During Spinal Anesthesia: A Case Report Outlining Recovery and Rehabilitation.

This case report highlights the rehabilitation outcomes of a 24-year-old female who received inadvertent intrathecal digoxin during a routine cesarean section, leading to encephalomeningitis, vestibulocochlear neuritis, and incomplete paraplegia. Despite initial neurological deficits, the patient demonstrated significant improvement in both cognition and functional mobility during a one-month inpatient rehabilitation program, ultimately achieving ambulation with assistive devices. This case underscores the potential for rehabilitation of neurological sequela following accidental intrathecal di

Postoperative complications of spinal vs general anesthesia in elderly patients undergoing hip hemiarthroplasty.

INTRODUCTION: Elderly patients with hip fractures are at high risk for mortality due to postsurgical complications. Hip hemiarthroplasty is a routine procedure done in elderly patients for surgical repair  of femoral neck fractures. Both general and spinal anesthesia can be used in elderly patients undergoing hemiarthroplasty. Rates of postoperative complications among the two anesthetic choices have not been directly compared. In this study, we compare the rates of postoperative complications in elderly patients (age greater than 70) undergoing hip hemiarthroplasty for femoral neck fractures

A Second Prone Dose Algorithm for Patients Undergoing Spinal Anesthesia During Thoracolumbar Surgeries.

BACKGROUND: Spinal anesthesia is safe and effective in lumbar surgeries, with numerous advantages over general anesthesia (GA). Nevertheless, 1 major concern preventing the widespread adoption of this anesthetic modality in spine surgeries is the potential for intraprocedural anesthetic failure, resulting in the need to convert to GA intraoperatively. OBJECTIVE: To present a novel additional prone dose algorithm for when a first spinal dose fails to achieve the necessary effect. METHODS: A total of 422 consecutive patients undergoing simple and complex thoracolumbar surgeries under spinal an

Spinal anesthesia in contemporary and complex lumbar spine surgery: experience with 343 cases.

OBJECTIVE: Spinal anesthesia (SA) is an alternative to general anesthesia (GA) for lumbar spine surgery, including complex instrumented fusion, although there are relatively few outcome data available. The authors discuss their experience using SA in a modern complex lumbar spine surgery practice to describe its utility and implementation. METHODS: Data from patients receiving SA for lumbar spine surgery by one surgeon from March 2017 to December 2020 were collected via a retrospective chart review. Cases were divided into nonfusion and fusion procedure categories and analyzed for demographic

Postoperative Analgesic Effectiveness of Quadratus Lumborum Block for Cesarean Delivery under Spinal Anesthesia.

BACKGROUND: Spinal morphine is the mainstay of postcesarean analgesia. Quadratus lumborum block has recently been proposed as an adjunct or alternative to spinal morphine. The authors evaluated the analgesic effectiveness of quadratus lumborum block in cesarean delivery with and without spinal morphine. METHODS: Randomized trials evaluating quadratus lumborum block benefits in elective cesarean delivery under spinal anesthesia were sought. Three comparisons were considered: spinal morphine versus spinal morphine and quadratus lumborum block; spinal morphine versus quadratus lumborum block; an

Cauda equina syndrome following combined spinal-epidural anesthesia with levobupivacaine for cesarean section.

Cauda equina syndrome (CES) is a rare but possible complication of neuroaxial anesthesia. Damage to the nerve roots may occur due to compression, inflammation, stretching, direct trauma, spinal ischemia or neurotoxicity, usually with lidocaine or bupivacaine. We describe a case of a 33-year-old patient that underwent an uneventful cesarean section with a combined spinal-epidural technique anesthesia, with levobupivacaine. 48 h after the procedure, she presented diminished muscular strength and abolished osteotendinous reflexes in the left lower limb, limited flexion of the right hallux, urinar

Differences in intraoperative hemodynamics between spinal and general anesthesia in infants undergoing pyloromyotomy.

BACKGROUND: Hypotension and bradycardia are known side effects of spinal anesthesia in pregnant women undergoing cesarean section and adults undergoing other surgical procedures. Whether children experience similar hemodynamic changes is unclear. AIMS: The purpose of this study is to evaluate hemodynamic effects of spinal anesthesia compared to general anesthesia in a cohort of healthy infants. METHODS: The University of Vermont Medical Center almost exclusively used spinal anesthesia for infant pyloromyotomy surgery between 2008 and 2013, while Columbia University Medical Center relied on g

Torsion of left main bronchus during general anesthesia for posterior instrumented spinal fusion.

Pulmonary function tests such as flow-volume loops and reconstructive radiological imaging may aid the detection of large airway obstruction prior to corrective surgery for severe scoliosis. Intraoperative use of halo-gravity traction may help to reduce the severity of the scoliosis, and thus the extrinsic compression or torsion of the airways.
