Integrin expression on monocytes and lymphocytes in unstable angina short term effects of atorvastatin.

Inflammatory reactions in coronary plaques play an important role in the pathogenesis of acute atherothrombotic events. The most powerful class of lipid-lowering drugs available-statins (3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors)--have additional actions, unrelated to cholesterol reduction, including anti-inflammatory and immunomodulatory properties. This study sought to determine if atorvastatin affects monocyte and lymphocyte activation in patients with unstable angina and mild primary hypercholesterolemia. Following a 4-weeks hypolipemiant-free baseline period, 22 patients-

[Six-month's clinical course of patients with unstable angina admitted at the emergency room].

OBJECTIVE: to know the mortality rate after six months in patients with unstable angina. MATERIAL AND METHODS: A representative sample of patients who were admitted at the emergency room with unstable angina diagnosis during a period of two years was selected. The presence of angina pectoris, heart attack and death was registered during their stay at the emergency room. A six-month follow-up took place to evaluate the frequency of angina, heart attack and death related to ischemic acute coronary syndrome, as well as the determination of the relative risk of factors associated to mortality. Association of endothelin-1 with transient myocardial ischemia in patients with unstable angina pectoris.

In this study we assessed whether serum endothelin-1 levels were associated with indexes of disease severity in unstable angina, including troponin I, C-reactive protein, and transient myocardial ischemia. Endothelin-1 levels were higher in patients who had transient myocardial ischemia and in those who had 3-vessel disease on angiography but were not significantly correlated with levels of C-reactive protein and troponin I.

The incremental value of troponin-I testing in patients with intermediate risk unstable angina.

BACKGROUND: Classification of patients with unstable angina (UA) by Agency for Health Care Policy and Research (AHCPR) guidelines in the emergency department reliably stratifies risk of death or myocardial infarction (MI) for triage to outpatient evaluation (low-risk), hospitalization (high-risk), or additional testing (intermediate-risk). Cardiac troponin-I elevation may identify patients at higher risk, but the incremental value may vary with AHCPR clinical risk. HYPOTHESIS: The objective of this study was to determine whether cardiac troponin-I had any additional value beyond triage based

Concentrations of interleukins, interferon, and C-reactive protein in stable and unstable angina pectoris.

We analyzed the concentrations of interleukins (IL)-6, IL-10, IL-12, and IL-18, interferon (IFN)-gamma, and high-sensitivity C-reactive protein (hsCRP) in 40 patients with unstable angina (UAP), 39 patients with stable angina (SAP), and 52 age- and gender-matched controls. Compared with the control group, IL-12 concentrations were significantly higher in both the SAP and UAP groups, especially in the UAP group, and the IL-18 concentrations tended to be higher in the UAP group. Conversely, IL-10 concentrations were significantly lower in the SAP and UAP groups. Both IL-6 and hsCRP concentration

[Pulmonary embolism in a patient with unstable angina treated with surgical revascularisation--a case report].

A case of a 58-year-old female patient with unstable angina is presented. Two weeks earlier the patient suffered from acute myocardial infarction treated with thrombolysis. The patient underwent surgical revascularisation which was complicated by acute pulmonary embolism. Repeated surgery and inspection of pulmonary arteries revealed the presence of thrombus which was successfully removed. The post-operative course was uneventful. The causes and treatment of pulmonary embolism complicating coronary artery by-pass grafting are discussed.

Effects of acute hormone therapy on recurrent ischemia in postmenopausal women with unstable angina.

OBJECTIVES: We tested whether acute hormone therapy reduces ambulatory electrocardiographic ischemia in postmenopausal (PMP) women with unstable angina (UA). BACKGROUND: Endothelial dysfunction contributes to the pathophysiology of UA. Acute estrogen administration improves endothelial function in PMP women with coronary artery disease and increases coronary artery blood flow. METHODS: Two hundred ninety-three PMP women with UA (mean age 69.7 years), treated with standard anti-ischemic therapy, were enrolled within 24 h of symptom onset. In a double-blind fashion, subjects were randomized to

Early and late mortality after surgery for unstable angina in relation to Braunwald class.

BACKGROUND: The prognostic value of the preoperative Braunwald class of unstable angina to predict early and long-term outcome after urgent coronary artery bypass grafting (CABG) has not been studied previously. METHODS: Deaths were recorded after all primary isolated CABG performed for unstable (n = 992) and stable (n = 5376) angina pectoris during 1980-1995. Severity of symptoms in the unstable patients was classified according to Braunwald. RESULTS: Death within 30 days of surgery occurred in 4.6% of the patients having unstable angina and in 1.6% of those with stable angina. Early mortal

C-reactive protein is a marker for a complex culprit lesion anatomy in unstable angina.

BACKGROUND: The putative theory is that the clinical syndrome of unstable angina is caused by rupture of the atherosclerotic plaque with superimposed thrombus formation. It is characterized by angiographically complex coronary lesions in the majority of patients. HYPOTHESIS: This study aimed at assessing the correlation between C-reactive protein (CRP) and the complexity of culprit coronary lesions in unstable angina. METHODS: We identified culprit lesion complexity in 96 patients with unstable angina and normal creatine kinase (CK) and CK-MB mass. Serum concentrations of CRP (N < 5.0 mg/l)

Assessment of weight-based versus standard dosing of heparin in patients with unstable angina.

OBJECTIVE: To assess whether a weight-based dosing regimen (80 u/kg + 18 u/kg/h) or a standard-fixed dose regimen (5000 u + 1000 u/h) of heparin is more appropriate in patients with unstable angina (UA). METHOD: A drug use evaluation was conducted. Patient data for all patients weighing less than 100 kg who were in the coronary care unit of a Veterans Administration Hospital and who received heparin for UA (>24 h) over a 7-month period were included. For the first 4 months, patients were given standard heparin dosing and in the final 3 months patients were given heparin based on weight. The p

Serum levels of the antiinflammatory cytokine interleukin-10 are decreased in patients with unstable angina.

BACKGROUND: Proinflammatory cytokines play a role in acute coronary events. However, the potential role of antiinflammatory cytokines in the modulation of the atherosclerotic process remains unknown. Interleukin (IL)-10, which is expressed in human atherosclerotic plaques, has potent deactivating properties in macrophages and T cells. The aim of this study was to assess whether serum concentrations of IL-10 differed between patients with unstable and stable angina pectoris. METHODS AND RESULTS: A total of 95 patients with angina pectoris and angiographically documented coronary artery disease

Enzymatic oxidant and antioxidants of human blood platelets in unstable angina and myocardial infarction.

In ischaemic heart conditions we report a remarkable increase in platelet xanthine oxidase activity and rise in the levels of malondialdehyde (MDA) with concomitant decrease in the activities of free radical scavenging enzymes - superoxide dismutase, catalase, glutathione peroxidase and glutathione reductase. The increased levels of free radical generating system and MDA and lowered levels of free radical scavenging systems seem to have critical role in ischaemic heart conditions.

Plasma protein acute-phase response in unstable angina is not induced by ischemic injury.

BACKGROUND: Elevated levels of C-reactive protein (CRP) are associated with an unfavorable clinical outcome in patients with unstable angina. To determine whether ischemia-reperfusion injury causes this acute-phase response, we studied the temporal relation between plasma levels of CRP and ischemic episodes in 48 patients with unstable angina and 20 control patients with active variant angina, in which severe myocardial ischemia is caused by occlusive coronary artery spasm. METHODS AND RESULTS: Blood samples were taken on admission and subsequently at 24, 48, 72, and 96 hours. All patients un

The use of beta-blockers in unstable angina: comparison with calcium channel blocking agents.

The pathophysiology of unstable angina comprises critical narrowing of large coronary vessels by advanced atherosclerotic plaques, thrombus formation, and vasospastic reactions. Therapeutic goals aim primarily at an increase of flow and include vasodilation, thrombolytic strategies, and antithrombotic procedures (anticoagulation, antiaggregation). In the complex system of myocardial blood flow regulation, under normal and ischemic conditions, coronary vasomotor tone plays an important role. Especially in patients prone to vasospastic reactions, coronary vessels can be constricted by blockade o
