Lonvoguran Ziclumeran - In Vivo CRISPR Gene Editing in Hereditary Angioedema.

BACKGROUND: Hereditary angioedema is a rare and potentially life-threatening genetic condition characterized by recurrent and debilitating swelling attacks. Lonvoguran ziclumeran (lonvo-z) - an investigational, in vivo gene-editing treatment based on clustered regularly interspaced short palindromic repeats (CRISPR) - is being developed as a single-dose treatment for hereditary angioedema. METHODS: In a phase 3, double-blind trial, we randomly assigned, in a 2:1 ratio, patients who were 16 years of age or older and had hereditary angioedema with C1 inhibitor deficiency to receive a single int…

Transmission of F12-related hereditary angioedema through a sperm donor.

Hereditary angioedema due to F12 pathogenic variants (HAE-FXII) is a rare autosomal dominant disorder characterized by recurrent episodes of angioedema mediated by bradykinin, incomplete penetrance, and marked sex-dependent clinical expression. Most affected individuals carry the recurrent F12 c.983C>A/p.Thr328Lys (T328K) founder variant. Here, we report the first documented transmission of HAE-FXII through sperm donation. The index case was an 18-year-old woman with recurrent estrogen-dependent angioedema, normal C1-inhibitor levels and function, and poor response to conventional antiallergic…

Hereditary angioedema and post traumatic stress disorder: a reciprocal relationship?

BACKGROUND: Hereditary angioedema (HAE) patients endure repeated unpredictable trauma associated with attacks of disfiguring swelling, severe abdominal pain, risk of asphyxiation or experiences with loss of a loved one with HAE. Our study investigated the prevalence of presumptive post-traumatic stress disorder (PTSD) for HAE due to C1 inhibitor deficiency (HAE-C1INH). RESULTS: Participants ≥ 18 with HAE-C1INH were recruited from the US HAE Association: Cohort 1, self-referred; Cohort 2, randomized from the HAEA membership. Participants completed 8 on-line questionnaires: (1) HAE survey; (2) …

Recurrent Intestinal Angioedema with Normal C1-Inhibitor: A Case Report.

Background and Objectives: Angioedema is a non-pitting edema of the submucosal layer which can be acquired or inherited and usually presents with hives. Intestinal angioedema is rare and can mimic other acute gastrointestinal disorders. It is typically associated with a lack or dysfunction of C1-inhibitor, with a small number of cases having normal C1-inhibitor function. We present a rare case of chronic recurrent intestinal angioedema in a patient with normal C1-inhibitor function who did not respond to icatibant therapy. Case presentation: A 56-year-old woman presented with 3 days of abdomin…

Vibratory Angioedema Due to Vigorous Nose Blowing: A Case Report.

Vibratory angioedema is a rare type of chronic inducible angioedema with a variety of triggers reported in the literature. Standard management of vibratory angioedema involves avoidance of known triggers, as well as stepwise therapy starting with second-generation antihistamines. We present a unique case of vibratory angioedema provoked by vigorous nose blowing and improved with treatment of chronic rhinitis. To our knowledge, this is the first reported case of vibratory angioedema associated with this specific trigger. Recognition of nose blowing as a potential precipitant may have meaningful…

Nonepisodic angioedema with eosinophilia following a rose-thorn prick injury.

BACKGROUND: Nonepisodic angioedema with eosinophilia (NEAE) is a condition marked by angioedema and significant eosinophilia and often linked with atopic dermatitis. It predominantly affects young Asian women and occurs more frequently in the autumn and winter. Despite over 100 reported cases, its etiology and pathogenesis remain unclear. CASE PRESENTATION: A 23-year-old Japanese female florist presented with acute arm swelling following rose-thorn pricks to her hands and fingers in spring. One week later, she developed progressive symmetrical non-pitting edema in her lower legs and a 3 kg we…

Nonepisodic angioedema with eosinophilia subsequent to acute parvovirus B19 infection.

Nonepisodic angioedema with eosinophilia (NEAE) is characterised by a single episode of angioedema localised to the extremities and peripheral eosinophilia. While NEAE can develop in response to infection or vaccination, NEAE associated with acute parvovirus B19 (B19V) infection is rare. We describe the case of a young woman with NEAE that developed during acute B19V infection. She presented with 1-week history of pruritus and polyarthralgia in the extremities, followed by the development of peripheral oedema, and was positive for anti-B19V IgM antibody. Her arthralgia improved within 2 weeks …

Hereditary angioedema (HAE) in children and adolescents: New treatment options.

Modern management of hereditary angioedema (HAE) due to reduced C1 inhibitor (C1-INH) function or concentration (HAE-C1-INH) focuses on individualized therapeutic strategies to address the specific needs of children and adolescents as well as the severity of the disease. Psychosocial factors such as the burden of disease and therapy on quality of life and participation play an important role. New medications have already significantly improved the prognosis and health related quality of life in HAE patients, but not all of these therapies have yet been approved for children. Further treatment …

An Incidental Finding of Eagle Syndrome Post-Losartan-Induced Angioedema.

Eagle syndrome is a condition that can present with a wide range of clinical manifestations, including orofacial pain, altered sensation, dysphagia, tinnitus, and ear pain, and is caused by the abnormal elongation of the styloid process or the mineralization of the stylohyoid ligament. We present a case of an incidental finding of Eagle syndrome in a 48-year-old African American patient with losartan-induced angioedema. The patient complained of a foreign body sensation in his throat and mild dysphagia, and a computed tomography scan of his neck showed ossification of bilateral stylohyoid liga…

Could it be hereditary angioedema?-Perspectives from different medical specialties.

Hereditary angioedema (HAE) is a rare autosomal dominant disease, with patients often suffering with associated symptoms for many years before receiving a correct diagnosis. The symptoms greatly impact a patient's quality of life (QoL) and include excruciating abdominal pain and angioedema of the skin and submucosa. Angioedema of the larynx represents a significant mortality risk in undiagnosed patients, and a large proportion of patients with HAE receive incorrect diagnoses and undergo unnecessary surgery. HAE-specific treatments can control and prevent acute life-threatening episodes, in add…

Trigeminal Zoster with drug-induced labial angioedema leading to necrosis.

INTRODUCTION: Zoster is caused by the reactivation of a dormant viral infection, and is characterized by painful, vesicular lesions along a dermatome. Neuritic pain associated with zoster can be treated with anticonvulsant medications. CASE REPORT: An immunocompetent adult physician developed prominent zoster lesions in the trigeminal nerve distribution. Treatment included antiviral therapy for the acute infection, and pharmacotherapy for neuritic pain. Pharmacotherapy included several anticonvulsant agents, with labial angioedema developing after initiation of oxcarbazepine. DISCUSSION: The…

Managing Chronic Urticaria and Recurrent Angioedema Differently with Advancing Age.

Angioedema and urticaria affect people of all ages. Accurate diagnosis and optimum management is essential for healthy aging. Older people continue to experience mast cell-mediated urticaria and angioedema, with a higher prevalence of autoimmune and a lower prevalence of autoallergic disease. Bradykinin-mediated angioedemas are more common in the elderly because of their association with angiotensin-converting enzyme inhibitor (ACEI) treatment. Acquired C1-inhibitor deficiency, another bradykinin-mediated angioedema, occurs predominantly in older people, whereas hereditary angioedema due to C1…

Solifenacin-induced acute urticaria and angioedema: a rare adverse effect.

Antimuscarinics are first-line medication for management of overactive bladder with solifenacin being commonly prescribed. Angioedema is the swelling of mucosa and submucosal tissue. There are no published case reports of drug-induced angioedema involving solifenacin. We report a case of a 41-year-old man with spinal cord injury who presented with oedema of face, lips, tongue and associated pruritic urticaria after taking 5 mg of solifenacin. All other possible causes including food allergy, insect bite, hereditary angioedema, use of NSAIDs, ACE inhibitors and antibiotics were ruled out. The t…
