The sprained ankle: current therapy.

The sprained ankle is one of the most common musculoskeletal injuries. Three treatments for severe acute sprain are advocated: immobilization, protected mobilization, and primary surgical repair. These injuries are cared for by different medical and paramedical specialties, each with varying skills and interests. In order to define the current preferred treatment, a survey was randomly distributed to 500 physicians. These data indicate that the treatment is generally conservative: cast immobilization was the preferred treatment for the moderate and severe sprain. These data also demonstrated t…

The hidden adolescent ankle fracture.

Ankle injuries are common in adolescence, seldom initially seen by orthopaedists, and often treated primarily by emergency department personnel. It is often not appreciated that the distal tibial growth plate fuses in a mediolateral direction, leaving it open and vulnerable to growth plate injuries for a longer period during adolescence. Fractures of the lateral plafond of the tibia are often not appreciated due to overlap of the fibula. A review of the injuries at the Winnipeg Children's Hospital has revealed these are not as uncommon as often supposed. A 3 year experience yielded 26 patients…

Operative treatment of ankle fractures.

In an unselected prospectively planned series of 611 ankle fractures 25% were of the AO (Weber) type A, 56% type B and 13% type C; 4% were impact fractures. The fractures were also classified according to Lauge Hansen's system, which was considered more complicated and not suitable for planning of operative treatment. Lauge Hansen's theory of the mechanism of the supination-eversion (SE) injury is questioned--outward rotation does not seem to be obligatory for the typical SE injury. 345 fractures were operated on, and 327 (95%) of them were followed up 1-6 years after operation. The range of m…

A comparative study of modern total ankle replacement and ankle arthrodesis for ankle osteoarthritis at different follow-up times: a systematic review and meta-analysis.

PURPOSE: Total ankle replacement (TAR) or ankle arthrodesis (AA) is the main surgical treatment for end-stage ankle osteoarthritis. However, the therapeutic effect of the two surgical procedures at different follow-up times remains controversial. The purpose of this meta-analysis is to compare the short-term, medium-term, and long-term safety and efficiency of the two modern surgical treatments. METHODS: We conducted a comprehensive search in PubMed, EMBASE, Cochrane library databases, Web of Science, and Scopus. The main results were the patient's reported outcome measure (PROM) score, satis…

Valgus ankle deformity is associated with increased mechanical complications and revision following total ankle arthroplasty.

BACKGROUND: Coronal plane deformities of the ankle, including varus and valgus malalignment, present technical challenges in total ankle arthroplasty (TAA) and may affect implant performance, although their impact on short-term outcomes remains uncertain. PURPOSE: To determine whether preoperative ankle varus or valgus deformity is associated with increased 2-year mechanical or implant-related complications following primary TAA. STUDY DESIGN: Retrospective cohort study using a national clinical database. METHODS: A retrospective analysis was conducted using the TriNetX Research Network, id…

Tibiocalcaneal Augmentation of Deltoid Ligament Repair Improves Ankle Stability: A Robotic Investigation of Ankle Biomechanics.

BACKGROUND: The deltoid ligament can tear in association with an ankle fracture or a syndesmotic injury or independently. Previous biomechanical research has shown that the optimal surgical treatment method for an anterior deltoid tear is repair with tibiocalcaneal augmentation. However, tibiocalcaneal augmentation is technically challenging to perform, as there is a risk of damage to neurovascular structures in the medial ankle. PURPOSE: To determine if tibiotalar augmentation is sufficient to stabilize the ankle in the setting of a deltoid tear. STUDY DESIGN: Controlled laboratory study. …

comparison of postoperative gait parameters after total ankle arthroplasty and ankle fusion: A systematic review.

Ankle pathology, such as severe arthritis, often necessitates surgical intervention to restore mobility and alleviate pain. Two commonly performed procedures for end-stage ankle disease are ankle fusion (AF) and total ankle arthroplasty (TAA). This review aims to compare the impact of AF and TAA on postoperative gait parameters. An extensive search in PubMed, Scopus, and Web of Science electronic databases was conducted with the use of the keywords ("ankle arthrodesis" OR "ankle fusion") AND ("ankle replacement" OR "ankle arthroplasty") AND "gait." Clinical studies in terms of postoperative ga…

Ankle-Foot-Orthosis "Hermes" Compensates Pathological Ankle Stiffness of Chronic Stroke-A Proof of Concept.

Individuals with an upper motor neuron syndrome, e.g., stroke survivors, may have a pathological increase of passive ankle stiffness due to spasticity, that impairs ankle function and activities such as walking. To improve mobility, walking aids such as ankle-foot orthoses and orthopaedic shoes are prescribed. However, these walking aids generally limit the range of motion (ROM) of the foot and may therewith negatively influence activities that require a larger ROM. Here we present a new ankle-foot orthosis "Hermes", and its first experimental results from four hemiparetic chronic stroke patie…
