Aphakic eyes possess considerably less oblique astigmatism than phakic eyes. The possibility that posterior iris displacement after lens extraction accounts for the observed reduction in oblique astigmatism is tested in this study. Oblique astigmatism was calculated for a phakic and an aphakic schematic eye. With no iris displacement little reduction of oblique astigmatism occurred contrary to experimental findings. The presence of cystoid macular edema (CME) after cataract extraction in the pediatric age group could adversely affect rapid visual rehabilitation of the eye by contributing to irreversible amblyopia. A technique is described for implantation of a posterior chamber intraocular lens during keratoplasty for bullous keratopathy when a large capsulotomy and vitreous prolapse are present. Seven cases of pseudophakic or aphakic bullous keratopathy with anterior chamber, iris-fixated, or no intraocular lens in place are reported. A simple clinical predictor of the amount of spectacle convex lens power that will be needed to correct the refractive error of an aphakic patient after cataract surgery is developed based on clinical measures of anterior corneal surface power (keratometer measurements) and axial length. This predictor derived from matrix optics is compared to previous ones developed by Binkhorst and Sanders et al. using different methodologies.

The qualitative photostress test is a simple kinetic test for the evaluation of macular function. The physiological basis of the test is discussed and the results of testing 20 normal patients and 14 postoperative cataract patients are presented. Patients with CME have a severe functional defect that is readily demonstrated with photostress testing.

Corneal swelling response was measured for 12 unilateral aphakic subjects wearing high-plus hydrogel contact lenses on both eyes. Significantly less corneal edema was measured for aphakic eyes than for phakic eyes after 2, 4, and 6 hr of lens wear. These results indicate that the cornea of the aphakic eye responds differently to a given physiologic challenge than does the cornea of the phakic eye. Previous estimates of oxygen permeability required of contact lens materials for extended wear may be artificially high for aphakic eyes.

Twenty-two aphakic eyes in seven females and thirteen males were fitted with a new type of soft contact lens designed for continuous wear. Nine patients were operated on for unilateral traumatic cataract, and eleven patients were operated on for senile cataract. Sixteen patients terminated the study. In all cases the visual acuity with lens was almost equal to the acuity obtained with spectacles. The lenses were well tolerated without any special wearing schedule.
