Recurrence of trigeminal neuralgia due to an acquired arachnoid cyst.

We present the case of a 55-year-old female with pain recurrence after microvascular decompression for trigeminal neuralgia due to development of an arachnoid cyst. Radioimaging studies were inconclusive for vascular compression but showed evidence of fifth nerve distortion. The patient underwent surgical re-exploration, and a cystic lesion of thickened arachnoid containing cerebrospinal fluid was identified and excised. Postoperatively, the patient obtained pain relief. Arachnoid cyst formation may be a possible reason for pain recurrence after microvascular decompression for trigeminal neura

Occlusive hydrocephalus caused by a fourth ventricle arachnoid cyst.

The case history of a woman with occlusive hydrocephalus caused by a fourth ventricle cyst is presented. She had slowly progressive complaints and symptoms--concentration and memory disturbances, low-tempered mood, then slight dizziness, loss of appetite and progressive headache--transitorily misinterpreted for signs of depression. She had been treated by psychotherapy and antidepressants for months. Since she did not improve she was referred to a psychiatric hospital. The rapidly progressing neurological syndrome with worsening headache, gait disturbance and vomiting was finally identified an

Giant arachnoid granulation in a patient with benign intracranial hypertension.

We report magnetic resonance (MR), computed tomography (CT) and angiographic imaging of an unusual giant arachnoid granulation in the superior sagittal sinus in a man with headache and vertigo. Intrasinus pressure measurements revealed a significant pressure gradient across the lesion. MR imaging is useful to identify giant arachnoid granulation and dural sinus thrombosis, whereas dural sinus pressure measurement in certain cases of giant arachnoid granulations can be used to evaluate the lesion as the cause of the patient's symptoms.

Magnetic resonance cisternography for preoperative evaluation of arachnoid cysts.

INTRODUCTION: With a high likelihood of clinical improvement and low rates of complications, minimally invasive neuroendoscopic surgery is becoming the treatment of choice for symptomatic or growing arachnoid cysts. In neuroendoscopic surgery, visualization of anatomical landmarks is essential in achieving successful fenestration without complications. Because of the restricted visual field in neuroendoscopic surgery, preoperative anatomical assessment is very helpful. Magnetic resonance cisternography (MRC) with high spatial resolution and contrast, using for example 3-D Fourier transformatio

Arachnoid cyst within the fourth ventricle: a case report.

The authors report a case of fourth ventricular arachnoid cyst that presented clinically with the criteria of normal pressure hydrocephalus. Only a few cases of intraventricular arachnoid cyst have been recorded in the literature. In our case, a posterior approach was used via a midline suboccipital craniectomy and the cyst was excised.

[De novo arachnoid cyst treated with a cystoperitoneal shunt].

INTRODUCTION: Arachnoid cysts (AC) are benign pathological structures that are typically found during childhood, and represent approximately 1% of all intracranial lesions. Unlike cases of secondary cysts with a known causation, which can be attributed to inflammatory processes or traumatic brain injuries, the causation of the primary cysts is unknown. In the cases reported to date, the cysts present from the first imaging study conducted, which is why they are considered to be of a prenatal origin. CASE REPORT: A boy who, at the age of 5 months, is found to have an AC of considerable dimensi

Endoscopic surgery for large posterior fossa arachnoid cysts.

The authors report two cases of large arachnoid cysts of the posterior fossa treated by endoscopic surgery. One patient underwent a successful endoscopic cyst fenestration by burr hole approach after several procedures of shunt revision. In another an endoscope-assisted microsurgical intervention was necessary. Lateral (cerebellar or cerebellopontine angle) cysts, as two reported cases, may be treated through a lateral retromastoid approach by fenestration into the prepontine cistern and eventually into the cisterna magna. We advise to start the operation through a burr hole and to try to real

Spinal arachnoid cyst without neural tube defect.

Symptomatic arachnoid cysts of the spine are rare lesions in the pediatric age group. Although most commonly occurring in association with neural tube defects, such as myelomeningocele and diastematomyelia, in some cases the cysts appear in children without spinal anomalies. We describe a 12-month-old girl with lumbar intradural arachnoid cyst with progressive weakness of the lower limbs. There was full recovery after fenestration of the cyst.

Arachnoid telangiectasia causing meningeal fibrosis and secondary syringomyelia.

A case of syringomyelia secondary to arachnoiditis associated with arachnoid telangiectasia is reported in a female patient with no other stigmata of hereditary haemorrhagic telangiectasia. Such a case has not been reported before. She underwent surgical decompression of the spinal cord with successful outcome.

Delayed postoperative CSF rhinorrhea of intrasellar arachnoid cyst.

CSF rhinorrhea due to a transsphenoidal approach usually follows accidental or intentional arachnoid opening. We report a patient with an intrasellar arachnoid cyst, who developed delayed onset of CSF rhinorrhea. A sixty-two-year-old man presented with bitemporal type visual field defect for the last 3 years. With the diagnosis of arachnoid cyst or Rathke's cleft cyst, based on MRI findings of intra-and supra-sellar cyst with CSF intensity, he successfully underwent transsphenoidal surgery without evidence of intra-operative CSF leakage. He developed CSF rhinorrhea one week later. This needed

[A case of schizophrenia-like psychosis in a patient with arachnoid cyst].

The aim of this report was the presentation of the extremely rare case of the arachnoid cyst of the brain, with only psychiatric manifestation. According to the literature and own experience the authors conclude, that the treatment of these patients would be interdisciplinary, both neurosurgical and psychiatric.

Microsurgical anatomy of the supratentorial arachnoidal trabecular membranes and cisterns.

We examined the microsurgical anatomy of the supratentorial subarachnoid cisterns with a surgical microscope in 20 brains prepared using the immersion technique. The adult brains were immersed in Ringer's solution and air was injected into the subarachnoid cisterns while the brains remained submerged in solution. We identified nine trabecular membranes that limit the 15 cisterns. We specifically looked at the anatomical relationship between the supratentorial trabecular membranes and cisterns to their corresponding vessels and cranial nerves. The cistern divisions and the dispositions of trabe

Arachnoid cyst of the lateral ventricle manifesting positional psychosis.

A 30-year-old woman had an arachnoid cyst in the trigone of the right lateral ventricle 5 years before she developed episodic auditory and visual hallucinations as well as delusions of persecution. The psychotic episodes tended to occur after the patient had lain in bed for 1 to 2 hours. After craniotomy and wide excision of the cystic membrane, draining the cystic fluid to the lateral ventricle, the psychotic episodes subsided in a follow-up period of 6 months. We believe that when the patient was recumbent, the trigone cyst blocked the temporal horn further, caused local ischemia, and trigge
