OBJECTIVE: Insufficient information exists about the ability of hemiparetic patients to adjust reach extent during early recovery from stroke. Further knowledge may suggest guidance for therapy intervention. The objective of this study was to investigate the ability of hemiparetic subjects to adjust reach extent within 6 months after stroke.
DESIGN: Repeated-measures design experiment with two factors-group and target position.
SETTING: Physiotherapy department.
PARTICIPANTS: Nine hemiparetic and nine age- and gender-matched healthy subjects.

An abnormal communication between an artery and a vein is known as arteriovenous malformation (AVM) or arteriovenous fistula (AVF). The AVM or the AVF might be congenital in origin or even acquired. Hand and arm problems are frequent in flautists. Severity is greatest in professional players, which can result in being unable to work. Based on a biomechanical analysis a support for the right hand was designed, which can be easily clipped on to the flute. Tests by professional flautists demonstrated that the right hand thumb support prevented rotation of the flute about its longitudinal axis. The latter reduces tension in forearm, wrist and fingers. This is the most important result of the study, which led to the introduction of the right hand thumb rest on the market.

Driving with an arm protruding through a window can result in limb threatening injury. A series of seven upper limb injuries sustained during motor vehicle accidents (MVAs) with a limb protruding from the vehicle window is described. The severity and complexity of these injuries are related to the ultimate functional recovery for the limb. This study serves to highlight the severity of injury and morbidity following these modalities of trauma, which although rarely fatal, carry extensive consequences and could be easily prevented with appropriate education programmes and legislation.

Sport-specific upper extremity strain, mostly unilateral, during growth may lead to adaptations in soft tissue and bone. We investigated 51 male professional handball players between 18 and 39 years of age (average, 27 years), 39 right-handed and 12 left-handed. A combination injury to the forearm, consisting of Monteggia- and Galeazzi lesion is described. After osteosyntheses of the fractures the dislocation of the head of radius and ulna were reduced spontaneously because of restauration of correct axes and length. Because of a simultaneous fracture of the coronoid process an above elbow plaster cast was supplied. Healing took place with free motion of all joints.

The stabilising effects on the glenohumeral joint of each of the rotator-cuff muscles and of the biceps were studied with the arm in abduction and external rotation in 13 cadaver shoulders. We present our experience with the use of the posterior arm fasciocutaneous flap in the reconstruction of 22 axillae in 17 patients. These patients all suffered from long-standing, severe hidradenitis suppurativa and this flap was used to reconstruct the axilla after surgical excision of the whole hair-bearing area of the axilla and the underlying axillary disease. Only a small number of cases of compartment syndrome in the upper arm has been reported in the literature. The authors have reviewed 14 patients with 14 cases of compartment syndrome treated at their institution from 1980 to 1988. In the majority of cases in this series, compartment syndrome was caused by blunt, high-energy trauma. We report on a patient with deficiency of distal 6p and compare the clinical and cytogenetic findings in this child with those of three previously reported patients who had similar deletions. Distal del(6p) appears to be associated with a relatively non-specific phenotype, with the possible exception of unusual congenital eye findings. This apparent association of congenital eye defects with distal del(6p) was supported by comparison with patients having other deletions of chromosome 6, particularly those with ring chromosome 6.

A case of an interstitial deletion of chromosome 13, identified as 46,XY,del(13)(q22q31), is reported in a child with psychomotor retardation, prominent low-set ears, epicanthus, hypertelorism, broad nasal bridge, hypoplastic fifth fingers and abnormal dermatoglyphics. This patient is compared to others in the literature with a similar deletion.

The paper discusses the histories and results of examination of 200 cases of lymphedema of the upper extremity developing after radical surgery for breast cancer. Forty percent of patients had suffered erysipelas, its frequency being higher in a group of pronounced edema. The incidence of erysipelas dropped 5.7 times following application of pneumatic compression, suggesting the use of the procedure for erysipelas by stimulating lymph drainage.
