Where does a man stand on issues of assisted reproduction, surrogacy, artificial insemination within lesbian relationships and posthumous conception?

This article takes issues more commonly associated with women: assisted reproduction, surrogacy, artificial insemination within lesbian relationships and posthumous conception and considers what role a man may play in this domain. To this end the article commences with a short scenario of possible life events then traverses the current legal position to examine the extent that the law either facilitates or frustrates a man's desire and ability to fulfill the role of either father or parent in the above situations. The article considers the issues of assisted reproduction technology in both a r

Integrating experimental evolution and transcriptomics reveals the phenotypic and molecular architecture of the pace-of-life syndrome.

Phenotypic traits often covary through shared genetic and environmental influences, shaping the evolution of integrated life-history strategies. In particular, correlations between behavioral and life-history traits form the core of the pace-of-life syndrome (POLS) hypothesis, yet direct experimental tests of its evolutionary and molecular foundations remain limited. Here, we combined experimental evolution and transcriptomic analyses to investigate the phenotypic and molecular architecture of POLS in the red flour beetle Tribolium castaneum. Using replicate lines artificially selected for inc

Ocular surface disease following LASIK and cataract surgery: a review of their interrelated complications.

BACKGROUND: Ocular surface disease is a multifactorial condition that is very commonly caused by dry eye disease (DED). Ophthalmic procedures intended to improve visual outcomes, laser-assisted in situ keratomileusis (LASIK) and cataract surgery, can paradoxically cause or exacerbate underlying ocular surface disease. This results in worsening vision and quality of life. AREAS COVERED: This review examines the pathophysiological mechanisms contributing to ocular surface disease development following LASIK and cataract surgery. Both procedures are associated with the transection of corneal ner

[Determination of the life expectancy of patients undergoing continuous endocardial stimulation].

Using an actuarial analysis, the authors studied the time of survival in 120 patients with continuous endocardiac electrostimulation. There were 37 lethal outcomes by the end of the fourth year of the follow-up, with 31 of these patients aging from 65 to 100 years. The study of the survival rate in different age groups showed that the highest survival rate was in the group below 55 years and the lowest in the group of patients aged 75 to 100 years. The study of survival in different age groups is important for the prognosis of the necessary amount of cardiostimulators with energy sources varyi

Congenital central hypoventilation syndrome: four families.

BACKGROUND: Congenital central hypoventilation syndrome (CCHS) is a rare condition that usually presents soon after birth and is potentially life-shortening if not treated. The defining abnormality is hypoventilation during sleep which requires life-long treatment with artificial ventilation. This syndrome may also be associated with generalised dysfunction of the autonomic nervous system and a sub-group with associated Hirschsprung's disease. The genetic basis of CCHS has been identified as mutations in the PHOX2B gene. METHODS: We present four families, three with autosomal dominant inherit

The Indiana experience with artificial urinary sphincters in children and young adults.

PURPOSE: We reviewed a 22-year single institutional experience with the artificial urinary sphincter in children and adolescents. To our knowledge this report represents the largest series in the world in children. MATERIALS AND METHODS: Between 1980 and 2002, 142 patients underwent implantation of an artificial urinary sphincter, of whom 93 males and 41 females with a median age of 10 years (range 3 to 39) were available for analysis. A total of 59 patients initially received an AMS 742/792 (American Medical Systems, Inc., Minnetonka, Minnesota) artificial urinary sphincter, of whom 33 were

Ultraviolet radiation: a hazard to children and adolescents.

Sunlight sustains life on earth. Sunlight is essential for vitamin D synthesis in the skin. The sun's ultraviolet rays can be hazardous, however, because excessive exposure causes skin cancer and other adverse health effects. Skin cancer is a major public health problem; more than 2 million new cases are diagnosed in the United States each year. Ultraviolet radiation (UVR) causes the 3 major forms of skin cancer: basal cell carcinoma; squamous cell carcinoma; and cutaneous malignant melanoma. Exposure to UVR from sunlight and artificial sources early in life elevates the risk of developing ski

[Obstetrician-gynecologist's attitudes toward legal and voluntary interruption of pregnancy].

Objective In 2006, Colombia decriminalized legal and voluntary interruption of pregnancy (LVIP) in situations in which: the mother's life or health is in danger, fetal life-threatening malformations exist, or when the pregnancy is the result of rape, non-consensual artificial insemination, or incest. Difficulties reported in the introduction of this practice are related to the attitudes of doctors. Obstetricians usually are the doctors who attend these cases; an assessment of their attitudes regarding this situation was performed. Method Qualitative and quantitative. Self-administered anonymou

Rethinking Dementia Risk Prediction: A Critical Evaluation of a Multimodal Machine Learning Predictive Model.

A recent study by Ding et al. explores the integration of artificial intelligence (AI) in predicting dementia risk over a 10-year period using a multimodal approach. While revealing the potential of machine learning models in identifying high-risk individuals through neuropsychological testing, MRI imaging, and clinical risk factors, the imperative of dynamic frailty assessment emerges for accurate late-life dementia prediction. The commentary highlights challenges associated with AI models, including dimensionality and data standardization, emphasizing the critical need for a dynamic, compreh

Infected Anastomotic Pseudoaneurysm after Aortic Valve Replacement with Annular Enlargement.

Anastomotic pseudoaneurysm remains one of the main life-threatening complications of cardiac and thoracic aorta surgery. We report a rare case of infected pseudoaneurysm at the anastomotic line found during follow-up. Blood culture results suggested Enterococcus faecium infection. Transthoracic echocardiography and computed tomography scans revealed the presence of a pseudoaneurysm of the ascending aorta. The pseudoaneurysm was resected and the ascending aorta was reconstructed with an artificial vascular patch without complications. Reducing the anastomotic tension, with complete hemostasis a

Impact of Mechanical Circulatory Support on Posttransplant Outcomes.

Mechanical circulatory support (MCS) has markedly improved the likelihood of transplant among patients with advanced heart failure. Transplant survival following MCS is similar for supported and unsupported recipients. Transplant survival is only reduced following left ventricle assist device (LVAD) support complicated by infection, total artificial heart support, and extracorporeal life support. Despite allosensitization and a higher incidence of vasoplegia syndrome, posttransplant survival for durable LVADs is similar to patients with inotropes alone at the time of transplant. MCS as a bridg

Urinary incontinence: when to refer for procedural therapies.

Urinary incontinence is not life-threatening but rather is a social problem for older women. Therefore, 'patient selection' for a surgical intervention is up to the patient. The physician's role is to educate patients about available procedures, including potential complications and success rates. Invasive treatments for stress incontinence include retropubic or needle suspension, sling procedures, injections of periurethral bulking agents such as collagen, and implantation of an artificial sphincter. An electrical nerve stimulation device has recently been approved for urge incontinence. Many

Shared decision-making about prolonged invasive ventilation for adults with myotonic dystrophy type 1.

The option of tracheostomy and prolonged invasive mechanical ventilation (IMV) for chronic respiratory failure in myotonic dystrophy type 1 (DM1) poses challenges in decision-making. The Myotonic Dystrophy Foundation consensus recommendations on respiratory management do not directly address the implications of this intervention given the progressive physical and cognitive manifestations of DM1. Here, we consider IMV in DM1 using the ethical principles of beneficence, non-maleficence, autonomy and justice. We call for further research on post-tracheostomy morbidity, mortality, and quality of l
