Magnetic resonance imaging in neonates with total asphyxia.

Magnetic resonance (MR) findings in cases of total asphyxia, whose lesions are mainly in the brainstem and deep nuclei, have not been clarified. In this study, we investigated MR images in neonates with total asphyxia. MR images of six infants (three males and three females; gestational age, 35-39 weeks; birth weights, 1880-3572 g) with total asphyxia were examined. In all subjects, neonatal cortical MR lesions were limited to the hippocampus with highlighting on T1-weighted imaging (T1-WI). The neonatal MR lesions of the cerebral white matter were limited to the white matter between the insul

The role of oxygen in the delivery room.

As recently as the year 2000, 100% oxygen was recommended to begin resuscitation of depressed newborns in the delivery room. However, the most recent recommendations of the International Liaison Committee on Resuscitation counsel the prudent use of oxygen during resuscitation. In term and preterm infants, oxygen therapy should be guided by pulse oximetry that follows the interquartile range of preductal saturations of healthy term babies after vaginal birth at sea level. This article reviews the literature in this context, which supports the radical but judicious curtailment of the use of oxyg

Transfer of cortical motor representation after a perinatal cerebral insult.

In a 16-year-old boy with hemiplegia and severe, intractable epilepsy after a neonatal cerebral ischemic insult, cortical motor control was only equivocally assessed by functional magnetic resonance imaging. Therefore, high-precision navigated transcranial magnetic stimulation was performed, which demonstrated that cortical control of muscles on the paretic side was selectively affected. Leg muscle control was located in the contralateral hemisphere, as expected in healthy individuals, whereas forearm muscles were controlled from both hemispheres, and hand muscles were controlled only from the

Decreasing diagnoses of birth asphyxia in Canada: fact or artifact.

OBJECTIVE: We assessed temporal trends in birth asphyxia in Canada, to determine whether changes were real or secondary to changes in coding. METHODS: We used data from the Canadian Institute for Health Information Discharge Abstract Database to study the national incidence of birth asphyxia, by using International Classification of Diseases codes. We also studied birth asphyxia by using data from the Nova Scotia Atlee Perinatal Database. In the Nova Scotia Atlee Perinatal Database, we defined a case of birth asphyxia as a live birth with an Apgar score at 5 minutes of < or =3, depression at

[Value of maximum length sequences brainstem auditory evoked potential in neonatal asphyxia].

OBJECTIVE: To study the value of a new technique, maximum length sequences brainstem auditory evoked potential (MLS BAEP), in the assessment of the severity of brain damage following asphyxia in term neonates. METHODS: One hundred and three neonates with perinatal asphyxia and 26 normal term neonates were eligible for the study. Conventional and MLS BAEP examinations were performed within three days after birth. Of the 103 neonates with asphyxia, 17 did not suffer from HIE, 37 had mild HIE, 31 had moderate HIE, and 18 had severe HIE. The latencies and amplitudes of waves I, III and V, and the

[Which is better to resuscitate asphyxiated newborn infants: room air or pure oxygen?].

OBJECTIVE: To find out optional resuscitation gas for asphyxiated newborn infants based on a systemic review of the published studies that compared the effects of resuscitation of asphyxiated newborn infants with room air or pure oxygen. METHODS: Inclusion criteria were randomized or pseudo-randomized studies of asphyxiated newborn infants resuscitated with room air or pure oxygen. The studies published between Jan. 1966 and June. 2005 were searched in PubMed/EMBASE/the Cochrane library databases. All identified studies that fulfilled our inclusion criteria were included. We did a systemic re

Midazolam and amplitude-integrated EEG in asphyxiated full-term neonates.

AIM: In the present, prospective study, the relation between the levels of midazolam, its two active metabolites--1-hydroxy-midazolam (OH-midazolam) and 1-hydroxy-midazolam-glucuronide (glu-midazolam)--and the aEEG were examined. PATIENTS AND METHODS: Fifteen full-term neonates with seizures due to hypoxic-ischaemic encephalopathy admitted to our NICU were included. Midazolam (loading dose 0.05 mg/kg in 10 min, maintenance dose 0.15 mg/kg/h) was used as an add-on anti-convulsant after phenobarbital and lidocaine because of continuing seizures. Amplitude-integrated EEG background pattern was s

Room air resuscitation-two decades of neonatal research.

Experimental as well as clinical studies have demonstrated that room air is as efficient as pure oxygen for newborn resuscitation. Recent data even indicate that outcome is improved if pure oxygen is avoided. Thus, in a meta-analysis, neonatal mortality was significantly lower in those newly born infants resuscitated with 21% than with 100% oxygen. Short-term recovery is also improved in the room air group since time to first breath is shorter, heart rate at 90 s and 5 min Apgar score are higher. Animal data indicate that injury in a number of organs, including the brain, is aggravated by givi

[Neuroimaging techniques for the diagnosis of brain damage in newborns].

Recently, magnetic resonance (MR) has become the technique of choice in evaluating neonatal central nervous system (CNS) diseases. It is the only imaging technique that can discriminate myelinated from neonatal unmyelinated white matter; it offers the highest sensitivity in detecting acute anoxic injury of neonatal brain, and, with proper coils and sequences, it can exquisitely depict neonatal brain anatomy and locate pathology. With new generation high-field MR imaging units it is now possible to obtain images of molecular diffusion. Preliminary reports suggest that diffusion imaging may be u

Can magnesium sulfate reduce the risk of cerebral injury after perinatal asphyxia?

Hypoxia-ischemia produces brain damage by processes that continue for many hours after reoxygenation/reperfusion. This provides a window of opportunity for therapy aimed at preventing further loss of brain cells. Sulfate magnesium can prevent posthypoxic brain injury by blocking glutamate receptors within the calcium (Ca++) ion channel. We used sulfate magnesium in nine newborn infant after perinatal hypoxia. We investigated the brain damage, by ultrasound examination, on third day, in first, second and third week, and third, sixth month of life. We have estimated the neurological development

[Antepartum brain injuries].

It is generally accepted that congenital brain lesions and especially cerebral paralysis are produced nearly always during the process of delivery. The origin can be hypoxia or obstetrical trauma. This has been true until the 70s, but from that period onwards, the incidence due to the delivery process has been reduced so dramatically, that its appearance can be considered as an accident, as in the umbilical cord prolapsus or during abruptio placentae. More than 80% of the cerebral paralysis occur before delivery due to a variety of causes that are detailed in this report and only the resting 2

Animal models for the study of perinatal hypoxic-ischemic encephalopathy: a critical analysis.

We critically evaluated various design features from 292 animal studies related to perinatal hypoxic-ischemic encephalopathy (HIE). Rodents were the most frequently used animals in HIE research (26%), followed by piglets (23%) and sheep (22%). Asphyxia with or without ischemia was the most predominant method of producing experimental brain damage, but there were significant variations in specific details, particularly regarding the method and duration of brain insult. In 71% (207/292) of studies the CNS outcomes were tested within 24 h of experimental insult and in 29% (85/292) they were teste

[Plasma cortisol levels in the newborn infants with hypoxic and traumatic injuries of the spinal cord].

Adrenal glucocorticoid function was studied in 56 newborns with natal injuries of the cervical section of the spine and the contribution of the hypoxic factor to such injuries defined. The babies were examined by the cliniconeurologic and electron-neuromyographic methods in order to specify the level of the injury. Blood serum and umbilical blood hydrocortisone levels were radioimmunoassayed immediately at birth and on days 5-7 of life. Spinal injury at the C1-C4 level was associated with low hydrocortisone levels, that may be regarded as an additional criterion for the differentiation of the

[Congenital right diaphragmatic eventration manifested with pleural effusion. Report of one case].

A 2,980-gram female infant was born to a 25-year-old mother at the gestational age of 34 weeks with the chief problems of asphyxia and respiratory distress. Prenatal sonogram at 34th week of gestation showed significant pleural effusion, mediastinal shift, polyhydramnios and large for date. Soon after birth, she was put on intubation and ventilator therapy. Physical examination revealed poor chest wall excursion. Breathing sound was markedly decreased over the right lung field. Abdomen was soft and slightly distended with the liver palpable 0.5 cm below the right costal margin and 2 cm below t

Myoglobinuric renal failure in a newborn after traumatic delivery.

Fetal and neonatal asphyxia is the main cause of transient or acute renal failure (ARF) in neonates. Rhabdomyolysis and subsequent myoglobinuria have been rarely reported in neonates. We describe a case of ARF in a newborn infant in whom asphyxia, birth trauma and hypovolemic shock precipitated rhabdomyolysis which contributed to ARF.
