Hyperventilation-induced bronchoconstriction (HIB) is a component of exercise-induced asthma (EIA) believed to result from the penetration of unconditioned air into the lung periphery. We used a canine model of EIA to examine the effect of hyperventilation on airway surface fluid (ASF) volume and osmolality, and to determine if the observed kinetics support the hypothesis that hyperventilation-induced changes in ASF osmolality initiate bronchoconstriction. Exposure of sublobar airways to dry air at baseline insufflation resulted in stable measurements of ASF volume, ASF osmolality, and periphe

PURPOSE: Allergic and asthma-related respiratory symptoms frequently co-occur in childhood and may reflect shared inflammatory mechanisms. However, population-based data on their overlap in Central and Eastern Europe remain limited.
METHODS: We conducted a cross-sectional analysis of 1293 children aged 5-9 years from the Polish arm of the Global Asthma Network survey. Respiratory symptoms included wheeze, exercise-induced wheeze, and cough unrelated to infection. Allergic symptoms included nasal symptoms, itchy/watery eyes, and rash. Symptom-domain presence was defined as ≥1 symptom, and mult

Bronchial asthma is a disease characterized by the condition of airway hyper-responsiveness, which serves to produce narrowing of the airway secondary to airway inflammation and/or various spasm-inducing stimulus. Nonspecific bronchoprovocation testing is an important method implemented for the purpose of diagnosing asthma; this test measures the actual degree of airway hyper-responsiveness and utilizes direct and indirect bronchoprovocation testing. Direct bronchoprovocation testing using methacholine or histamine may have superior sensitivity as these substances directly stimulate the airway

BACKGROUND: For the early identification of persistent asthma symptoms among young children with recurrent wheezing, it would be helpful to identify all available studies that have identified at least one factor for predicting the persistence of early wheezing. The objective of the present study was to perform a systematic review of all studies that have identified factors that predict the persistence of symptoms among young patients with recurring wheezing.
METHODS: A systematic review of relevant studies was conducted through searching in MEDLINE, EMBASE, CINHAL, and SCOPUS databases up to

Asthma occurs across the life course. Its optimal treatment includes the use of personalized management plans that recognize the importance of co-morbidities including so-called "dysfunctional breathing." Such symptoms can arise as a result of induced laryngeal obstruction (ILO) or alterations in the mechanics of normal breathing called breathing pattern disorders. Whilst these two types of breathing abnormalities might be related, studies tend to focus on only one of them and do not consider their relationship. Evidence for these problems amongst childhood asthmatics is largely anecdotal. The

BACKGROUND: Pediatric inducible laryngeal obstruction (ILO) is difficult to diagnose and treat. Patients often undergo multiple specialist referrals, and long-term outcomes are not well reported.
OBJECTIVES: To investigate the patterns of presentation, workup, and management of children who were diagnosed with ILO at the Stollery Children's Hospital.
METHODS: Retrospective review with a prospective cohort of pediatric patients diagnosed with ILO from 2015 to 2023. We collected the demographic data, diagnostic tests, specialist referrals, time to diagnosis, symptom burden, associated comorbid

Eighteen children aged 6 to 11 years old who fulfilled criteria for the diagnosis of neonatal meconium aspiration syndrome were investigated to find out the prevalence of previous and current respiratory symptoms, and abnormal pulmonary function tests, chest radiographs, and ventilation scans. Eleven of the 18 had no respiratory symptoms and eight of these had normal pulmonary function on testing. Two had mild limitation of expiratory airflow that did not respond to treatment with bronchodilators, and one had exercise induced bronchospasm that responded to treatment with bronchodilators. Seven

BACKGROUND: The pathophysiology of exercise-induced asthma is not well understood. Hypertonicity of the airway lining fluid resulting from loss of water due to hyperventilation is considered to play a role, but the precise mechanism by which hypertonicity can induce bronchoconstriction is unknown. Peptides of the endothelin (ET) family have potent smooth muscle contractile properties, and have been linked to airway narrowing in stable asthma. We postulated that ET release may contribute to the acute bronchoconstrictor response induced by a hypertonic stimulus.

The effects of maternal smoking during pregnancy and childhood environmental tobacco smoke (ETS) exposure on asthma and wheezing were investigated in 5,762 school-aged children residing in 12 Southern California communities. Responses to a self- administered questionnaire completed by parents of 4th, 7th, and 10th grade students were used to ascertain children with wheezing or physician-diagnosed asthma. Lifetime household exposures to tobacco smoke were assessed using responses about past and current smoking histories of household members and any history of maternal smoking during pregnancy.

Wheezing is a commonly encountered complaint by patients seen in sports medicine practice. Wheezes are a continuous musical sound heard best on expiration and can originate from one or more of several defined anatomical locations in the human airway. While common causes of wheezing include exercise-induced bronchoconstriction, postnasal drip, and asthma, wheezing also follows specific respiratory infections and can persist for months after the onset of symptoms. Abnormal lung physiology following pneumonia can persist for decades. These postinfectious pulmonary changes affect the ability of at

Protection of the health of the athlete is required for high level sporting performance. Acute respiratory illness is the leading cause of illness and can compromise training and competition in athletes. To date the focus on respiratory health in athletes has largely been on acute upper respiratory infections and asthma/exercise induced bronchoconstriction (EIB), while nasal conditions have received less attention. The nose has several important physiological functions for the athlete. Nasal conditions causing obstruction to airflow can compromise respiratory health in the athlete, negatively

BACKGROUND: Having asthma has in previous reports been related to a lower physical activity level. At the same time the prevalence of asthma among elite athletes is high. The aim of this study was to investigate the association between physical activity level and asthma.
METHODS: A postal questionnaire was completed by 25,610 individuals in Sweden. Current asthma was defined as having had an asthma attack during the last 12 months or current use of asthma medication. The participants were asked how often and for how many hours a week they were physically active.
RESULTS: In the population 18

Although several tests are available to assess the presence and severity of bronchial hyperresponsiveness (BHR), there is no agreement on the most appropriate stimulus. The most commonly used stimuli are methacholine, histamine, and exercise. Daily peak expiratory flow (PEF) variation has been reported to correlate with the severity of BHR, and in recent years this has been widely used because of its noninvasiveness and ease of performance. This study was carried out to determine the relationship among these four commonly used measures of bronchial responsiveness in asthmatic children. For thi

Laryngomalacia is a well-recognized cause of airway obstruction and inspiratory stridor in infants. As children grow and become more active, laryngomalacia may manifest in different, unexpected ways. Otherwise healthy athletes may generate enough inspiratory force to draw the aryepiglottic folds into the endolarynx, causing a subtotal glottic obstruction. This problem may be overlooked or attributed to asthma, lack of fitness, or functional abnormalities. The purpose of this report is to review the prevalence, diagnosis, and treatment of exercise-induced laryngomalacia (EIL) in children and yo

Chronic obstructive pulmonary disease (COPD), asthma, and heart failure (HF) frequently coexist, forming a high-risk clinical triad characterized by overlapping symptoms and pathophysiological mechanisms. This COPD-asthma-HF overlap complicates diagnosis and treatment due to shared clinical features such as dyspnea, wheezing, and exercise intolerance. Epidemiologically, up to 30% of COPD patients have concomitant HF, while 15-20% of obstructive airway disease patients exhibit asthma-COPD overlap, many of whom also develop HF due to chronic hypoxia, systemic inflammation, or shared risk factors

Leukotrienes (LT), both the cysteinyl LTs, LTC(4), LTD(4) and LTE(4), as well as LTB(4) have been implicated in the clinical course, physiologic changes, and pathogenesis of asthma. The cysteinyl LTs are potent bronchoconstrictors, which have additional effects on blood vessels, mucociliary clearance and eosinophilic inflammation. In addition, the cysteinyl LTs are formed from cells commonly associated with asthma, including eosinophils and mast cells. LTB(4), whose role is less well defined in asthma, is a potent chemoattractant (and cell activator) for both neutrophils and eosinophils. In th

Three atmospheric pollutants are discussed: Sulfur dioxide (SO2) acts as irritant gas on upper airways, trachea and large bronchi. Bronchoconstriction by SO2 is enhanced during work. Dose-response correlation may be observed with SO2 concentrations and bronchial hyperreactivity. Deaths and morbidity rates of patients with COPD parallel peaks of SO2 concentration such as occurred in the 1956 London smog. The mechanisms involved seem to be the same in cross sectional as in long term SO2 effects on human airways. Ozone (O3) is a major irritant pollutant. O3 penetrates deeply into the small airway

Dexamethasone-isonicotinate aerosol (DIA) was administered to twenty-nine steroid-dependent children with chronic perennial asthma, in an open trial during one year. An attempt was made to withdraw systemic corticosteroid therapy. As judged by clinical results, inhaled dexamethasone-isonicotinate controlled the asthma quite as well as did previous therapy. Oral corticosteroids were withdrawn in 27 patients, and the dosage considerably reduced in another two. Cushingoid features subsided. There was a statistically significant improvement in most pulmonary function parameters (airway obstruction

BACKGROUND: Exercise-induced bronchoconstriction (EIB) is common among adolescents and athletes. While typically assessed with spirometry, oscillometry may offer complementary insights. This cross-sectional study examined how fractional exhaled nitric oxide (FeNO) and symptoms relate to postexercise airway responses assessed via spirometry and oscillometry, and whether these associations differ between nonathletes and athletes.
METHODS: Subsamples from two adolescent cohorts (N = 241; 143 nonathletes, 98 athletes) completed questionnaires, FeNO measurements, and an EIB test using spirometry,
