The electrophysiologic effects of the new class-I antiarrhythmic drug pentisomide were investigated after intravenous (5 mg/kg) application in nine patients with atrioventricular nodal reentrant tachycardia and six patients with atrioventricular tachycardia. Pentisomide did not change sinus cycle length, effective refractory period of the right ventricle (ERP-RV), the AV-node (ERP-AVN) and QT interval. Intranodal (AH-interval) and infranodal conduction time (HV-interval), effective refractory period of the right atrium (ERP-A), QRS duration, antegrade effective refractory period of the accesso

The results of surgical repair of ostium primum atrioventricular septal defect show continued improvement. This improvement reflects the adv … surgical approach has been to place the interatrial baffle in such a way as to avoid the displaced atrioventricular node and thereby leave the coronary sinus in the left atrium. Our approach to repair of the so-called cleft in the left atrioventricular valve (in reality the space between the ventricular components of the bridging leaflets), is based on the unequivocal …

BACKGROUND: Arrhythmias after palliation of congenital malformations with functional monoventricle by different Fontan modifications are very common. Sinus node dysfunction and junctional rhythm are known to have a high prevalence and a detrimental impact on the optimal functioning of Fontan circulations. Maintaining sinus node function has high prognostic significance, and some cases have even been described where atrial pacing with restoring of atrioventricular synchrony was able to reverse protein-losing enteropathy with overt failure of the Fontan.
CASE SUMMARY: A 12-year-old boy with a c

The location and size of the coronary sinus in hearts with atrioventricular septal defect were investigated in relation to the known disposition of the atrioventricular conduction axis. We examined the morphology in 40 hearts and supplemented this series with two other hearts that had been serially sectioned previously. The coronary sinus received drainage from a persistent left superior caval vein in 5 hearts. Six cases of 40 had malalignment of the septal structures relative to the crux of the heart. In these, the conduction axis was anticipated to course in the position where the inlet vent

A case of digoxin overdose was described. Despite vigorous gastric lavage, the serum digoxin which was 0.8 ng/ml on admission rose to 7 ng/ml four hours later. The main side-effects were symptoms of nausea and anorexia and conduction defects including complete atrioventricular dissociation requiring pacemaker insertion. Opportunity was taken to study the site of heart block with His bundle electrography during the conduction disturbances. This showed that the site of block was situated above the His-Purkinje system in the atrioventricular (A-V) node.

Atrioventricular conduction defects after complete correction of tetralogy of Fallot are well known and their prevalence is decreasing with improved operative techniques. On the other hand, sinus node dysfunction has not been described after this type of surgery. The authors report the case of a 24 year old patient operated at the age of 2 for a favourable form of tetralogy of Fallot. In the long-term, this patient had a minimal anatomical sequel (mild pulmonary insufficiency) and first degree AVB with complete right bundle branch block with sinus bradycardia on the surface ECG. Between 1964 and 1971, 49 patients were discharged from hospital following atrial baffle repair for transposition of the great arteries. Electrocardiograms of 47 of these patients were available for review seven months to eight and a half years (average 31 months) after operation. Of the 46 patients in sinus rhythm prior to operation, 40 (87%) were in sinus rhythm with normal atrioventricular conduction when last seen. Four patients (8.7%) had benign supraventricular dysrhythmias: three were junctional, one was atrial. None was noted to experience rapid dysrhythmias. Two patients showed eviden

BACKGROUND: Nodoventricular accessory pathways (NV-APs) are rare embryonic remnants connecting the atrioventricular node to the ventricle. We describe their electrophysiological characteristics and ablation outcomes across a multicenter Asian cohort, highlighting a developmental link to Ebstein anomaly.
CASE SERIES: Five patients presented with supraventricular tachycardia due to NV-APs (4 concealed, 1 manifest). Programmed stimulation confirmed ventricular participation and excluded the atrium from the circuit. Two patients had Ebstein anomaly, clinically extending histological data regardin…

The functioning of the heart rhythm can exhibit a wide variety of dynamic behaviours under certain conditions. In the case of rhythm disorders or cardiac arrhythmias, the natural rhythm of the heart is usually involved in the sinoatrial node, the atrioventricular node, the atria of the carotid sinus, etc. The study of heart related disorders requires an important analysis of its rhythm because the regularity of cardiac activity is conditioned by a large number of factors. The cardiac system is made up of a combination of nodes ranging from the sinus node, the atrioventricular node to its Purki…

The electrophysiologic properties of bepridil, a calcium channel blocker with additional effects on fast response tissues, were investigated in 10 patients with atrioventricular accessory pathways. Seven patients had Wolff-Parkinson-White syndrome, and three had concealed atrioventricular pre-excitation. A dose of 4 mg/kg was administered intravenously over five minutes. Bepridil increased the AH interval and the functional refractory period of the atrioventricular node. The effective refractory periods of the right atrium and right ventricle were also increased. Bepridil prolonged refractorin…

Verapamil is supposed to suppress the initiation of circus movement supraventricular tachycardia by affecting the atrioventricular node. In electrophysiological tests, programmed stimulation is usually performed by using the same location for pacing and premature stimulus. Spontaneous ectopic activity starts from a different location than the sinus node and can therefore find altered re-entry conditions. In this study a 3D computer model based on Huygen's principle is used for simulation of the spread of excitation in the human heart in combination with a posterobasal, right or left lateral ac…

BACKGROUND: Sinus node dysfunction is the most common indication for antibradycardiac permanent pacing after heart transplantation. Lack of atrioventricular synchrony during pacing can result in symptoms ranging from mild chest discomfort to severe manifestations such as dyspnea, hypotension, and cardiovascular collapse, all of which are ascribed to pacemaker syndrome. In infants pacemaker syndrome is often only recognized in the face of marked hemodynamic compromise.
RESULTS: This report details an unusual account of pacemaker syndrome in an infant after orthotopic heart transplantation.

Cardiac arrhythmias, notably Wolff-Parkinson-White syndrome, are known to represent a major issue in patients with Ebstein's malformation of the tricuspid valve. Abnormal conducting circuits, however, can also be produced by pathways extending either from the atrioventricular node or the ventricular components of the atrioventricular conduction axis, direct to the crest of the muscular ventricular septum. We hoped to provide further information on the potential presence of such pathways by investigations of six autopsied examples of Ebstein's malformation. All were studied by histological sect…

BACKGROUND: Mathematical modelling has allowed calculation of the length of the slow and fast pathways in typical atrioventricular nodal reentrant tachycardia (AVNRT). The length of the slow pathway has been correlated with the measured length of the right inferior extension in human histologic specimens, but no histology data exist about the fast pathway.
METHODS: In preparations of cadaveric human hearts, the AV node was identified, and the site of the fast pathway was projected according to both existing evidence and results of our electroanatomic mapping. This permitted measurement of the

BACKGROUND: Specific pacing methods to unmask the existence of the dual atrioventricular (AV) nodal pathway in patients with dual AV nodal non-reentrant tachycardia remain to be established.
OBJECTIVE: This study aimed to determine the electrophysiological characteristics of dual AV nodal non-reentrant tachycardia by its responses to specific pacing methods.
METHODS: Five patients diagnosed as having dual AV nodal non-reentrant tachycardia were retrospectively investigated.
RESULTS: Atrial pacing could not induce the clinical tachycardia as continuous double firing in any of the 5 patients,

PURPOSE: To evaluate nature of AV nodal activation in patients with AVNRT using high density electro-anatomic mapping (HD-EAM).
METHODS: HD-EAM was created in 30 patients with AVNRT from the triangle of Koch (ToK) in sinus rhythm (SR). Isochronal late activation maps (ILAM) were created. EAMs were analyzed for slow pathway (SPW) and fast pathway (FPW) activation. A pivot point (PP) was defined where FPW and SPW collided and pivoted back to the AV node (AVN). Conduction was assessed with programmed extrastimulus (PES) in 9 patients until FPW refractory period (ERP). The change in PP distance f
