Topical Zinc Oxide Nanoparticle Formulations for Acne Vulgaris: A Systematic Review of Pre-Clinical and Early-Phase Clinical Evidence.

Background and objectives: Antibiotic resistance in Cutibacterium acnes is undermining topical macrolides and clindamycin, prompting renewed … on to rigorously designed, adequately powered randomised trials that will benchmark ZnO-NPs against benzoyl peroxide and retinoids, optimise dosing for efficacy versus phototoxicity, and establish long-term dermatological safety.

Efficacy and Tolerability of a Novel Cosmetic and Over-the-Counter Facial Acne Regimen Versus a Prescription Treatment.

BACKGROUND: The SkinMedica Acne Treatment Platform (SM Regimen) was formulated to treat acne without overdrying the skin. We evaluated efficacy and tolerability of the SM Regimen (including a novel 1% salicylic acid Acne Clarifying Cleanser and 2% salicylic acid Acne Treatment Lotion) versus a prescription formulation (Rx Regimen; including adapalene 0.1%/benzoyl peroxide 2.5%) in a diverse population of adults with mild to moderate facial acne. METHODS: This single-center, double-blind, randomized study enrolled adults (18-45 years) with Fitzpatrick skin types (FST) I-VI. SM Regimen or Rx Re…

Abnormal cathepsin B activity in Batten disease.

Fibroblasts cultured from patients with various forms of neuronal ceroid-lipofuscinosis (NCL; Batten disease) showed variably decreasing cathepsin B activity with increasing passage number and months in culture in the presence of fetal calf serum. Cathepsin H activity and that of a wide range of lysosomal hydrolases was unaffected by these conditions. Cathepsin B activity was assayed either colorimetrically (N alpha-benzoyl-DL-Arg-beta-naphthylamide; BANA), fluorimetrically (Z-Arg-Arg-methylcoumarin), or autoradiographically, following NaDodSO4-12.5% polyacrylamide gel electrophoresis ([125]Ty

Acne vulgaris: a clinical update.

Acne vulgaris is a chronic inflammatory condition with a substantial psychosocial burden. Early effective therapy reduces the risk of permanent scarring and mental health impacts. Combination topical therapy is recommended as initial and maintenance treatment for most patients (e.g. benzoyl peroxide and a topical retinoid). Topical antibiotics should not be used long-term, or as monotherapy, because of the risk of antibiotic resistance. Management has shifted away from prolonged oral antibiotic courses. If oral antibiotics are used, they should be time-limited (generally no more than 3 months)…

Management of Acne in Pregnancy.

Acne is one of the most common dermatological conditions to affect women of childbearing age, so it is important to consider the safety of long-term acne treatments on women who could become pregnant. In this review article, we clarify what management options are available to treat acne during pregnancy. Topical treatments, typically first-line for acne, such as azelaic acid, clindamycin, erythromycin, metronidazole, benzoyl peroxide, salicylic acid, dapsone, and retinoids, were reviewed. Systemic treatments, such as zinc supplements, cephalexin, cefadroxil, amoxicillin, azithromycin, erythrom…

Emerging lasers and light-based therapies in the management of acne: a review.

Acne vulgaris, commonly known as acne, is the most prevalent skin disorder affecting mainly adolescents and young adults, though it can affe … ecting their self-image and psychosocial well-being. Standard treatments include topical retinoids, benzoyl peroxide, topical antibiotics, and, for more severe cases, systemic antibiotics or isotretinoin, which require prolonged periods of compliance. All these pharmacologic treatments have a risk of side effects ranging from mild ones, like skin irritation and dr …

The effect of vehicle formulation on acne medication tolerability.

Combination acne medications provide enhanced treatment opportunities. A commonly used acne therapy may combine a topical antibiotic with benzoyl peroxide (BPO) to prevent antibiotic resistance while optimizing control of microcomedone formation with a retinoid. Unfortunately, this combination of highly efficacious medications may cause irritation because of the inherent skin irritancy of BPO and retinoids. The present study was undertaken to determine if vehicle optimization of a clindamycin-BPO formulation could increase the tolerability of an added retinoid. Forty-six women with mild to mod…

[Acne comedonica following radiation therapy].

HISTORY AND CLINICAL FINDINGS: A 50-year-old female patient underwent extirpation of a moderately differentiated, partially tubular ductal carcinoma of the left mamma and dissection of the left axilla followed by postoperative radiation therapy (total dose 58/48 gy). 2 weeks after the radiation therapy was stopped, treatment with goserelin subcutaneously was started. 6 weeks later numerous open and closed comedons appeared which were sharply confined to the radiation areas of left mamma and sternum. DIAGNOSIS, TREATMENT AND COURSE: Because of the clinical diagnosis of acne comedonica followin

Itching in acne--an unusual complication of therapy.

Pruritus at the site of active acne has not been described before as a complication of acne therapy. We report 8 cases seen over a 3-year period at our department with localized itching mainly at the active acne sites, within 2-6 weeks of starting effective acne therapy. Seven had oral tetracyclines (oxytetracycline, Vibramycin or Minocin) with or without topical antibiotics or benzoyl peroxide. Only 1 patient received oral isotretinoin. The severity of itch was mild to moderate in most cases with the severest reaction in the isotretinoin-treated patient. All cases resolved within 4 weeks, 7 w…

Adjuvant alternative treatment with chemical peeling and subsequent iontophoresis for postinflammatory hyperpigmentation, erosion with inflamed red papules and non-inflamed atrophic scars in acne vulgaris.

The standard management of acne vulgaris in Japan includes a combination of topical treatment with benzoyl peroxide (BPO) and BPO/clindamycin (CLDM), topical adapalene and systemic antimicrobials. However, the treatment of therapy-resistant complications such as postinflammatory hyperpigmentation (PIH), erosions with inflamed red papules and atrophic scars has not been established. We performed chemical peeling with glycolic acid and iontophoresis with ascorbyl 2-phosphate 6-palmitate and DL-α-tocopherol phosphate for the treatment of PIH, erosions with inflamed red papules and non-inflamed at…

Topical antimicrobial treatment of acne vulgaris: an evidence-based review.

Topical antimicrobial treatment is indicated for mild to moderate acne vulgaris. Our literature review includes searches of Ovid, MEDLINE, EMBASE, and the databases of the Cochrane Library. A detailed search strategy is included. All searches were limited to controlled trials and systematic reviews. No year limits were applied to the searches, but we focused on trials, guidelines, and reviews published since 2004, the year that the last review of topical antimicrobials was published in this journal. Several controlled trials demonstrate that benzoyl peroxide, topical antibiotics, and topical r…

Treating acne with antibiotic-resistant bacterial colonization.

INTRODUCTION: Acne is a chronic skin disorder of the pilosebaceous unit; it has a multifactorial pathogenesis. Propionibacterium acnes within the follicle is considered to be a triggering factor of inflammation in acne. Antibiotics have been the primary treatment against P. acnes for more than 40 years. However, a gradual increase in the prevalence of antibiotic-resistant strains of P. acnes has been observed. AREAS COVERED: This review discusses the pathophysiology of antibiotic-resistant acne development. It focuses on strategies to minimize the development of resistance and, most important
