Concurrent detection of cholecystocolic fistula and hepatic abscess by hepatobiliary scintigraphy.

We report the case of a 58-year-old male with cholecystocolic fistula and hepatic abscess that were not diagnosed by routine radiologic examinations, but concurrently detected by hepatobiliary scintigraphy with 99mTc-PMT, and confirmed at operation. Hepatobiliary scintigraphy is noninvasive and useful in the detection of a biliary enteric fistula and lesions about related organs. Concurrent detection of cholecystocolic fistula and hepatic abscess on hepatobiliary series is of great clinical value in planning surgical intervention.

Computed tomography demonstration of cholecystogastric fistula.

Cholecystogastric fistula is a rare complication of chronic cholecystitis or long-standing cholelithiasis. It results from the gradual erosion of the approximated, chronically inflamed wall of the gall bladder and stomach with fistulous tract formation. The present case describes the direct visualization of a cholecystogastric fistula by computed tomography in a patient without prior biliary system complaints.

Arteriocholedochal fistula: an unusual cause of hemobilia.

We report an unusual cause of hemobilia in a patient with a transhepatic biliary catheter. Hemobilia was due to an extrahepatic fistula between the gastroduodenal artery and the common bile duct and was responsible for significant blood loss. The fistula was successfully treated with transarterial embolization that resulted in no further episodes of hemobilia during the following 12 months.

[Biliary endoprosthesis causing ileal perforation--a case report].

We experienced ileal perforation caused by dislocated biliary endoprosthesis in 59 years old female patient. The endoprosthesis was implanted due to biliary fistula after laparoscopic cholecystectomy 2 years before the perforation. It seems that endoprosthesis dislocation and the perforation were the result of too long stay of endoprosthesis. After the surgical management and the removal of the prosthesis patient was cured. Although ileal perforation caused by dislocated biliary endoprosthesis is rare, clinicians should be aware of the possibility of its occurrence.

Successful treatment of isolated draining intrahepatic biliary ducts in a pediatric liver transplant recipient using alcohol sclerotherapy.

Alcohol sclerotherapy is useful for the treatment of biliary cutaneous fistulas in patients who have undergone hepatic resection. The same principle can be applied for transplant recipients with isolated draining biliary ducts. Percutaneous therapy of isolated draining biliary ducts with absolute alcohol (ie, 100% ethanol) was successfully performed in a 7-year-old patient who had undergone orthotopic liver transplantation. The increasing use of segmental living and split cadaveric liver grafts, with the subsequent increase risk in biliary complications, necessitates more efficient therapy for

Cholecystocolonic fistula.

The cholecystocolonic fistula is an atypical variant of biliary disease. When presenting with symptomatic disease, surgical treatment with cholecystectomy, fistula takedown and possible colonic resection are indicated, however the role of surgery in asymptomatic patients, especially those deemed higher risk is less clear. Herein we present a case of an incidentially discovered asymptomatic cholecystocolonic fistula in a higher risk surgical patient managed nonoperatively. The presentation and treatment options for this disease are discussed in relation to their application to this patient.

Spontaneous resolution of a gallstone ileus.

Gallstone ileus is an uncommon complication of cholelithiasis, usually associated with an internal biliary fistula. Management of gallstone ileus is surgical with enterolithotomy the procedure of choice, followed by fistula closure either as a one or two stage procedure. In this case a 66 year old female presented with colicky abdominal pain, computed tomography (CT) clearly showing a gallstone ileus and cholecystoduodenal fistula. Despite this the patient refused surgery and went on to have spontaneous resolution of the obstruction and passage of gallstones.

[Choledochoduodenal fistula: an unusual complication of duodenal ulcer].

We report a case of choledochoduodenal fistula in a patient with a duodenal ulcer and poor compliance to treatment. The fistula tract was demonstrated on a plain abdominal X-ray (presence of air in the biliary tract), and was confirmed by a fistulography from the site of the ulcer (opacification of the bile duct). A Finsterer type 2/3 gastrectomy was performed in this patient, leading to the treatment of the ulcer and disappearance of the fistula following a gastrojejunal shunt of the duodenum. Clinical outcome was excellent.

Posttraumatic bronchobiliary fistulae due to foreign body remnants after  a road traffic injury: a case report.

BACKGROUND: Bronchobiliary fistula is an extremely rare disease that involves abnormal communication between a hepatic segment and bronchial tree. It is mostly caused by untreated hydatid cyst, liver abscess, iatrogenic stenosis, and, rarely, trauma. CASE PRESENTATION: We experienced an extremely rare case of bronchobiliary fistula after motor vehicle accident. A 15-year-old Persian boy visited our clinic with chief complaints of persistent pleuritic chest pain, productive cough, weight loss, and fever for 2 months. Coronavirus disease 2019 reverse transcription polymerase chain reaction test…

Anesthesia management in neonatal congenital bronchobiliary fistula: case report and literature review.

BACKGROUND: There is very little published literature and none that discussed care in a neonate regarding anesthetic risk and management of neonate with congenital bronchobiliary fistula during thoracoscopy and thoracotomy. This article analyzes related risk factors and literature review from perioperative ventilation, circulation and other aspects of management. CASE PRESENTATION: A neonate diagnosed as congenital bronchobiliary fistula combined with severe chemical pneumonia, consolidation of the lungs, and infection was facing the risk of anaesthesia under thoracoscopy exploration surgery,…

[Internal biliodigestive fistulas].

Experience in surgical treatment of 53 patients with internal biliodigestive fistulas is analysed. The formation of the fistula in all cases … Hepatorenal insufficiency developed in 8, incompetence of the choledochus sutures in 3, an external biliary fistula in 2, hepatic abscess in 1, pancreatitis in 2, and suppuration of the postoperative wound in 3 patients. Among the 53 patients treated by operation, 49 (92.5%) recovered and 4 (7.5%) died from various postoperative complications.

Endobronchial Watanabe Spigot in the treatment of bronchobiliary fistula.

Bronchobiliary fistula (BBF) is a rare condition in which an abnormal communication exists between the bile ducts and the bronchial tree. Malignancy is the most common etiology of BBF, although many others are possible. A 74-year-old male patient with an inoperable Klatskin tumor presented with a complaint of yellow-green sputum and cough; the patient underwent fiber-optic bronchoscopy based on a preliminary diagnosis of bronchobiliary fistula. Using fiber-optic rigid bronchoscopy, the laterobasal segment of the lower right lung lobe was occluded using three pieces of 5-mm Endobronchial Watana…

[An unusual cause of respiratory failure. Bronchobiliary fistula].

The authors describe the treatment of a patient with an inborn bronchobiliary fistula. The ex post finding that the fistula had two insertions in the bronchus called in the final stage for a combined thoraco-abdominal approach with resection of the right lower pulmonary lobe with ligature of the fistula beneath the diaphragm. Although inborn bronchobiliary fistulae are very rare, they must be considered in the differential diagnosis of causes of chronic respiratory distress syndrome in children. Early diagnosis and adequate surgical treatment make it possible to prevent serious chronic cardior…
