This paper deals with the synthesis of information from different studies when there is lack of independence in some of the contrasts to be combined. This problem can arise in several different situations in both case-control studies and clinical trials. For efficient estimation we appeal to the method of generalized least squares to estimate the summary effect and its standard error. This method requires estimates of the covariances between those contrasts that are not independent. Although it is not possible to estimate the covariance between effects that have been adjusted for confounding f

The effective reproduction number of an infection, denoted Re, may be used to monitor the impact of a vaccination programme. If Re is maintained below 1, then sustained endemic transmission of the infection cannot occur. In this paper we discuss methods for estimating Re from serological survey data, allowing for age and individual heterogeneity. We describe semi-parametric and parametric models, and obtain an upper bound on Re when vaccine coverage and efficacy are not known. The methods are illustrated using data on mumps and rubella in England and Wales.

The analysis of family-study data sometimes focuses on whether a dichotomous trait tends to cluster in families. For traits with variable age-at-onset, it may be of interest to investigate whether age-at-onset itself also exhibits familial clustering. A complication in such investigations is that censoring by age-at-ascertainment can induce artifactual familial correlation in the age-at-onset of affected members. A further complication can be that sample inclusion criteria involve the affection status of family members. The purpose here is to present an approach to testing for correlation that

BACKGROUND: This paper aims to determine the family history distribution of a particular disease as a result of its specific genetic aetiology, defined by the mode of inheritance (dominant or recessive), allele frequency, and increased risk in carriers of the genetic risk (genetic relative hazard). Here, 'family history' is the number of affected first-degree relatives of a defined index person (termed the proband), who may be either affected or unaffected with the disease in question.
METHODS: We consider model parameters relevant for female breast cancer in Australia. Breast cancer affects

BACKGROUND: It has been suggested that prolonged exposure to sunlight may induce systemic or local immune alterations, which may facilitate the development of skin cancer and, perhaps, non-Hodgkin's lymphona. The effects of prolonged sunlight exposure on peripheral blood cells were studied.
METHODS: Leukocytes and lymphocyte subpopulations of 12 volunteers aged 10-45 were investigated before and after a 3-week summer holiday in seaside resorts in Greece. Lymphocyte phenotypes were estimated using monoclonal antibodies and flow cytometry.
RESULTS: There were no significant differences with re

BACKGROUND: Hepatitis C is transmitted by transfusion of unscreened blood, through injecting drugs, from mother-to-child and, on occasion, sexually. Transmission generally requires that the infector is hepatitis C virus (HCV) RNA positive, a 'carrier'. About three-quarters of injectors who are hepatitis C antibody positive are HCV-RNA positive and so infectious to others. Incubation periods from HCV infection to cirrhosis and hepatocellular carcinoma are even longer than from HIV infection to AIDS, being counted in decades; they depend on age, gender, alcohol consumption and co-infection with

BACKGROUND: European guidelines for breast-cancer screening recommend an integrated approach of mammography screening with subsequent assessment and biopsy, if required, in one screening unit under permanent quality control, for which target values are released. Although the calculation of the respective rates (e.g. for participation, assessment, biopsy, or cancer detection) appears trivial, the statistical assessment of their compatibility with the target values is less obvious. This is especially true if subjects with a positive diagnostic result leave the screening-assessment chain prematur

An outbreak of anthrax occurred in the city of Sverdlovsk in Russia in the spring of 1979. The outbreak was due to the inhalation of spores that were accidentally released from a military microbiology facility. In response to the outbreak a public health intervention was mounted that included distribution of antibiotics and vaccine. The objective of this paper is to develop and apply statistical methodology to analyse the Sverdlovsk outbreak, and in particular to estimate the incubation period of inhalational anthrax and the number of deaths that may have been prevented by the public health in

We present a spatial model for the mean and correlation of highly dispersed count data, and apply it to individual-level counts of the nematode Wuchereria bancrofti, a parasite of humans which causes the disease lymphatic filariasis. Our model uses the negative binomial distribution, whose shape parameter is a convenient index of over-dispersion. Spatial association is quantified in terms of a characteristic length, which has an intuitive interpretation as the distance over which correlation decreases by half. Demographic surveillance and mapping enable us to include individual-level covariate

BACKGROUND: Policies and measures for the prevention of human immunodeficiency virus Type 1 (HIV-1) transmission require adequate information about the risk profile of AIDS which is time-, place- and population-dependent. We have studied the risk factors for AIDS among men in Greece, a country with relatively low incidence of AIDS.
METHODS: A case-control study of all male patients with incident disease, who have been diagnosed in the major university-affiliated, AIDS Unit from February 1995 through August 1997 was conducted in Athens, Greece, a country with relatively low incidence of AIDS.

BACKGROUND: Family history of stroke has been considered as a risk factor for stroke, but this relationship is controversial. Some, but not all, studies have found an independent association between family history of stroke and stroke. This study examines the relationship between family history of stroke and all stroke types combined, cerebral infarction and intraparenchymatous haemorrhage.
METHODS: This is an incidence type, population-based case-control study. Cases were defined following WHO criteria and controls were randomly selected from the study-base population. Family history of stro

BACKGROUND: Our objective was to develop and test a methodology for inferring the percentage of prisoners currently using opiates from the percentage of prisoners testing positive for opiates in random mandatory drugs testing (rMDT).
METHODS: The study used results from Willing Anonymous Salivary HIV (WASH) studies (1994-6) in six adult Scottish prisons, and surveys (1994-5 and 1997) in 14 prisons in England and Wales. For Scottish prisons, the percentage of prisoners currently using opiates was determined by assuming, with varying empirical support, that: current users of opiates in prison w

At least 20 million women in developed countries are estimated to be currently using hormone replacement therapy (HRT). Almost 100 epidemiological studies have reported on the relationship between the use of HRT and the risk of cancer of female reproductive organs, namely the breast, uterus or ovary. Cancer at these sites is common and there are a priori reasons why the use of hormonal therapy to 'replace' the endogenous production of ovarian hormones after the menopause might increase the risk of these cancers. The available evidence indicates that the risk of breast cancer or endometrial can

BACKGROUND: Attention throughout Europe continues to focus on bovine spongiform encephalopathy (BSE), with increasing evidence linking it to the new variant of Creutzfeldt-Jakob disease in humans. In particular, recent attention has been directed at Portugal, where the incidence of confirmed BSE cases continues to rise.
METHODS: We modelled the age-specific incidence of BSE in Portuguese-born cattle by birth cohort as a function of: the survival distribution; the cohort-specific incidence of BSE infection; the age-specific probability, conditional on survival, that an infected animal will exp

Alzheimer's Disease (AD) research increasingly recognizes the importance of characterizing the joint and relative dynamics of biomarker trajectories during the preclinical phase. Jack's model stands as the most well-known theoretical framework of these processes. While it offers conceptual guidance on modeling, it is not a directly implementable model for observed data. Key challenges include: (i) biomarker measurements are often noisy, depend on individual characteristics, and often exhibit complex temporal and cross-sectional correlations; (ii) data are sampled sparsely and may cover only a

Social determinants of health (SDOH) are key drivers of cardiovascular disease (CVD), but their relationships differ substantially across regions, necessitating the design of locally tailored intervention strategies. In this work, we propose a novel clustering approach to partition US counties into different clusters based on similarities in their SDOH-CVD associations, providing a data-driven regional partition to inform region-specific interventions. To enhance the interpretability of the clustering result, we adopt the SDOH framework from the Agency for Healthcare Research and Quality to gr

Acute diarrheal disease is a leading cause of death in children under age 5, disproportionately impacting children in low-resource settings. Although many cases could respond to antibiotic treatment, the benefits of widely prescribing antibiotics must be weighed against the risks of antimicrobial resistance. These challenges motivate development of individualized treatment guidelines for diarrheal disease. In this study, we utilize a framework for creation and evaluation of individualized treatment rules that leverage diagnostic and clinical information to make treatment recommendations. In co

Estimating optimal dynamic treatment regimes (DTRs) from observational data is often complicated by nonignorable missing covariates arising from informative monitoring in clinical practice. A recent weighted Q-learning approach addresses such nonignorable missingness but relies on parametric Q-function models. In multistage settings, misspecification of Q-function models can propagate backwards through pseudo-outcomes and accumulate bias, both directly through Q-function estimation and indirectly through biased estimation of missingness propensities, raising robustness concerns. Motivated by t
