OBJECTIVES: To investigate whether there is an increased risk of cancer associated with lithium treatment in patients with bipolar disorder compared to the general population.
METHODS: A nationwide Swedish register study of incidence rate ratios (IRRs) of total cancer and site-specific cancer in the 50-84-year age range was carried out in patients with bipolar disorder (n = 5,442) with and without lithium treatment from July 2005 to December 2009 compared to the general population using linked information from The Swedish Cancer Register, The National Patient Register, and The Drug Prescripti…

Although we have gained enormous insights into neurobiological and psychological underpinnings of bipolar disorder (BD) symptoms, our knowledge concerning pathogenic mechanisms initiating recurrent affective episodes is still fragmentary. Previous research has highlighted the role of significant life events and social rhythm in recurrent episodes of mania and depression. However, most studies share the drawback of retrospective self-report data, which are prone to recall biases and limited introspective abilities. Therefore, more objective data, such as neuropsychological and neurobiological m…

BACKGROUND: Emerging research suggests that low self-compassion may be linked to psychopathology and in particular depressive symptoms. To further elucidate this topic, the present study investigated self-compassion in patients with Bipolar Disorder (BD).
METHOD: Thirty remitted BD patients were compared to thirty age- and sex matched controls on the Self-Compassion Scale (SCS). The BD patients also completed the Altman Self-Rating Mania Scale (ASRM), the Major Depression Inventory (MDI), the Work and Social Adjustment Scale (WSAS), the Satisfaction With Life Scale (SWLS) and the Internalized…

The role of stimulants for treating severe depression remains controversial, especially when it comes to bipolar depression. Potential benefits have to be weighed against risks, including addictive potential and treatment-emergent mania. But not all stimulants are the same. Modafinil and its R-enantiomer armodafinil seem to have positive augmentation effects when coupled with standard treatment of bipolar depression, while also having a relative low risk of addiction and manic switches. A recent hypothesis derived from the observation of hypovigilance in manic patients postulates that modafini

BACKGROUND: Though poorly defined, hypersomnia is associated with negative health outcomes and new-onset and recurrence of psychiatric illness. Lack of definition impedes generalizability across studies. The present research clarifies hypersomnia diagnoses in bipolar disorder by exploring possible subgroups and their relationship to prospective sleep data and relapse into mood episodes.
METHOD: A community sample of 159 adults (aged 18-70 years) with bipolar spectrum diagnoses, euthymic at study entry, was included. Self-report inventories and clinician-administered interviews determined feat…

Pediatric bipolar disorder, once thought rare, has gone through stages of conceptualization. DSM criteria were reinterpreted such that children and adolescents, particularly those with ADHD, were commonly diagnosed with bipolar disorder and thought to be atypical by adult standards. Research criteria separated pediatric bipolar patients into 3 phenotypes, including a research diagnosis of "severe mood dysregulation." DSM-5 largely maintained previous criteria for bipolar disorder at all ages and created a new diagnosis called "disruptive mood dysregulation disorder," categorized as a depressiv…

BACKGROUND: There is evidence that patients with bipolar disorder (BD) score higher on affective temperament ratings compared to healthy controls (HCs). Moreover, unaffected relatives demonstrate similar patterns as BD patients suggesting that such temperaments are related to the genetic risk for BD and may serve as endophenotypes for the disorder. It is unknown whether affective temperaments are associated with other core features of BD, such as impairments in neurocognition. This study examined the relationship between affective temperaments and neurocognition in patients with BD and in HCs.…

BACKGROUND: Evidence supports the role for mitochondrial impairment in the pathophysiology of bipolar disorder (BD). BD has been associated with decreased mitochondrial electron transport chain activity and increased oxidative stress. Also, mitochondrial DNA (mtDNA) encodes mitochondrial electron transport chain proteins and has been associated with altered oxidative stress. Preclinical studies showed that lithium treatment increased mtDNA content, but no study has directly assessed mtDNA content in subjects with BD in vivo. Also, the effects of lithium treatment on mtDNA content have never be…

Many studies have reported neurological and neuropsychiatric symptoms, including a significantly increased risk of depression and suicide, from exposure to low to moderate levels of organic solvents. However, few studies have reported a link between organic solvent exposure and bipolar disorder. The authors present a case of a 43-y-old man who developed new-onset bipolar disorder following exposure to organic solvents in a textile printing plant. The man had elevated blood levels of several common solvents. His symptoms abated following avoidance of solvent exposures at his workplace and after…

Bipolar affective disorder (BD) is a long-term mental disorder presenting remittent depressive, hypomanic, manic or mixed episodes of illness and low symptomatic or asymptomatic intermediate periods. It is related to a serious risk of self-destructive behaviour. Patients are typically symptomatic approximately half of the time. Treatment focuses on preventing the recurrence of episodes. The care is based on mood stabiliser and second generation antipsychotic medication in accordance with the patient's current symptoms. When added to pharmacotherapy, psychosocial interventions can improve the o

Although depressive symptoms have been consistently associated with lower quality of life and increased disability in bipolar disorder, their relation to manic symptoms is not as clear cut. A great overlap between these outcomes complicates the understanding of how they cluster together. We used exploratory factor analysis to clarify the relation between these constructs. We consecutively recruited 320 patients with bipolar disorder, and interviewers rated the Hamilton Depression and Anxiety Rating Scales, the Young Mania Rating Scale (YMRS), Clinical Global Impression (CGI), and the Global As…

OBJECTIVES: Health-related quality of life (HRQOL) refers to an individual's overall sense of wellbeing, and subjective physical, psychological, and social functioning. HRQOL is significantly affected in patients with bipolar disorder (BD), a chronic affective disorder characterized by mood fluctuations, sleep and cognitive impairment, and impact on inter-personal relationships, all of which contribute to HRQOL impairment. This article reviews the relevant literature on the potential causes and magnitude of HRQOL impairment in BD and role of treatment interventions in restoring HRQOL in this p…

Antiphospholipidsyndrome (APS) is an autoimmune disorder which causes a hyper-coagulable state characterized by recurrent thrombosis. It has a diverse range of central nervous system manifestations. We describe a case of a 61 year old man with bipolar disorder and APS, and we compare this to a previously reported case. Additionally, we reviewed literature regarding APS-related markers and the relationship of APS to other psychiatric and neurologic illnesses. We discuss possible mechanisms for an association between APS and bipolar disorder. We encourage clinicians to be aware of this possible …

BACKGROUND: Bipolar disorder is a common recurrent illness with high levels of chronicity. Treatment resistance persists despite the use of established medications, such as lithium and valproate. New medications are required for the treatment of refractory cases. Some open-label reports have suggested that the anticonvulsant tiagabine may be efficacious in bipolar disorder. There is a need to clarify the evidence available, in the form of randomised controlled trials, for its use in the treatment of acute affective episodes in bipolar disorder.
OBJECTIVES: To review the evidence for the effic…

The subset of patients with late-life onset of bipolar disorder is widely recognized to represent an etiologically diverse group with high rates of neurologic comorbidities., Efforts to characterize the unique clinical and neurobiological features of this heterogeneous population have yielded inconsistent findings., Although there is growing evidence that geriatric-onset bipolar disorder is often attributable to a variety of secondary organic etiologies, we would like to present a case of bipolar disorder emerging in late life where no clear organic cause was identified. This case highlights t…

OBJECTIVE: The best known neurobehavioral effects of testosterone are on sexual function and aggression. However, testosterone and other androgens may be involved in the pathophysiology of mood disorders and suicidal behavior. This is the first study to examine whether there is a relation between testosterone levels and clinical parameters in bipolar suicide attempters.
METHODS: Patients with a DSM-IV diagnosis of a bipolar disorder (16 males and 51 females), in a depressive or mixed episode with at least one past suicide attempt were enrolled. Demographic and clinical parameters, including l…

BACKGROUND: Circadian rhythm instability and abnormalities of melatonin secretion are considered as trait markers of bipolar disorder. Melatonin is secreted by the pineal gland. We investigated pineal volume in patients with bipolar disorder, and expected to observe smaller than normal pineal glands in cases of bipolar disorder.
METHODS: The primary outcome was the total pineal volume measured for each pineal gland with T1 MRI sequence. Twenty patients with bipolar I and II disorder and twenty controls were recruited. Pineal glands with large cysts (type 3) were excluded.
RESULTS: After excl…

Suicide rates of bipolar patients are among the highest of any psychiatric disorder, and improved identification of risk factors for attempted and completed suicide translates into improved clinical outcome. Factors that may be predictive of suicidality in an exclusively bipolar population are examined. White race, family suicide history, and history of cocaine abuse were predictive of suicidal histories. Gender, nicotine use, medical comorbidity, and history of alcohol and other drug abuse were not, although a trend was noted for a history of benzodiazepine abuse. Attempts, although less comm
