The neonatal morbidity and mortality of low birth weight neonates.

During the period between January 1, 1987 and March 31, 1991, a total of 454 low birth weight neonates, classified if below 2,500 grams, admitted to this hospital, 450 of them could be evaluated retrospectively. Among the 450 cases, 41 (9.1%) were transferred from outside hospitals within 24 hours of birth; and 409 were original hospital inpatients. The sex distribution was quite even (M/F = 227/223), but more than half of them were small-for-date (56.4%). Multiple pregnancy accounted for 22.9%, and toxemic mother, for 11.6%. Neonatal mortality rates were 23.9% for those with birth weight belo…

Psychosocial stressors and low birth weight: development of a questionnaire.

Low birth weight is a major public health problem because it is a major contributor to infant mortality as well as to various types of morbidity among young children. Of particular concern is that black women have an increased risk of low birth weight babies compared with white women. Many etiologic factors for low birth weight have been identified, but even within homogeneous strata of risk, black women have a greater risk of low birth weight babies than do white women. The reasons for this excess risk are not well understood. Available evidence suggests that exposure to psychosocial stressor…

Birth weight in twins: an analysis of discordancy and growth retardation.

Sonographic estimation of fetal weight offers the potential of antenatal diagnosis of discordancy and growth retardation in twins. Although standards for intertwin birth weight percent differences in infants over 2500 g are available, similar norms are not available at lower weights. Intertwin birth weight percent differences in live-born twins above 500 g in 500-g increments were analyzed. Intertwin birth weight differences, expressed as a percentage of the weight of the larger twin, were relatively uniform across a wide range of birth weights. Differences above 15% were increasingly more lik…

[Which light for phototherapy of the low birth weight infant?].

We have compared three different fluorescent light sources (true light, blue light, true light + blue light) for phototherapy in 155 low birth weight newborn babies affected with non haemolytic hyperbilirubinaemia. Our data suggest that blue light is significantly more effective than true light. Surprisingly the association of true light with blue light is significantly more effective than both true light and blue light separately. The greater effectiveness associated with the better tolerance of true light + blue light respect to blue light only, makes it preferable than phototherapy regimen,…

Nutritional supplementation and the outcome of pregnancy. I. Birth weight.

Colombian women at risk of malnutrition were enrolled in a health care program and randomly assigned to supplementation and control groups at the beginning of the third trimester of pregnancy. The net dietary intake increments resulting from supplementation amounted to 155 cal and 20 g of protein per day. Supplementation had a significant effect on the mean birth weight of male infants, but not that of female infants; the mechanisms responsible for the sex differences remain to be elucidated. The randomized trial design of the experiment and the documented similarity between the experimental a…

Effect of folic acid supplement on birth weights of infants.

Earlier observations from this Institute have indicated that the infants born to mothers receiving folic acid supplements in addition to iron during pregnancy were heavier than those born to mothers receiving iron alone. Since birth weights are influenced by maternal factors like height and parity, a trial was carried on in matched controls with iron and iron and folic acid during the last 12 to 16 weeks of pregnancy. Simultaneously, placental function was also assessed. The effect of the folic acid supplements was more marked on first born children. Incidence of small-for-dates births was hal…

Effects of Liver Enzyme Levels During Pregnancy on Birth Weight.

OBJECTIVES: To investigate the association between maternal liver enzyme concentrations during pregnancy and the risk of abnormal birth weight. METHODS: This is a prospective birth cohort study querying the pregnant women from Fujian Maternal and Child Health Hospital, affiliated with Fujian Medical University, China. Liver enzyme levels, including gamma-glutamyl transferase (GGT), alanine aminotransferase (ALT), and aspartate aminotransferase (AST), were measured in the first and third trimesters, and changes in liver enzyme levels were calculated based on these measurements. The outcomes we…

Carbohydrate-Mediated Pregnancy Gut Microbiota and Neonatal Low Birth Weight.

The effects of gut microbiota on the association between carbohydrate intake during pregnancy and neonatal low birth weight (LBW) were investigated. A prospective cohort study was conducted with 257 singleton-born mother-child pairs in Taiwan, and maternal dietary intake was estimated using a questionnaire, with each macronutrient being classified as low, medium, or high. Maternal fecal samples were collected between 24 and 28 weeks of gestation, and gut microbiota composition and diversity were profiled using 16S rRNA amplicon gene sequencing. Carbohydrates were the major source of total ener…

Impact of low birth weight on staged single-ventricle palliation.

BACKGROUND: To assess the impact of low birth weight on early and late outcomes after staged palliation for single ventricle. METHODS: Patients after stage 1 palliation for single ventricle in our institution were retrospectively included and divided into two weight groups: 2.5 kg or less (low birth weight) and more than 2.5 kg. The impact of low birth weight on mortality and on the progression to further palliation stages (bidirectional Glenn, stage 2, and total cavopulmonary connection, stage 3) was assessed. RESULTS: A total of 452 patients were included. Patients with low birth weight (n…

Anesthetic management of extremely low-birth-weight neonates for laparotomy.

Anesthetic management of extremely low-birth-weight (ELBW) neonates is always accompanied by many dilemmas and challenges. Here, we report a case in which 512 g of ELBW newborns underwent exploratory laparotomy for perforation. Anesthesia management of such ELBW infants has not been reported in the literature.

Late Onset Hearing Loss in Very Low Birth Weight Infants.

OBJECTIVE: To determine the incidence of late onset hearing loss and associated risk factors in very low birth weight (VLBW) infants. STUDY DESIGN: Retrospective study (2003-2015) of post-discharge hearing outcomes and risk factors in the VLBW infant population, before and after the institution of a standardized follow-up program. RESULTS: Late onset hearing loss increased from 2.9 per 100 VLBW infants to 7.8 per 100 after instituting a monitoring protocol. The follow-up compliance rate nearly doubled. Both infants with late-onset sensorineural hearing loss and those with a conductive compon…

Transcutaneous CO2 Monitoring in Extremely Low Birth Weight Premature Infants.

Extremely low birth weight (ELBW) premature infants are particularly susceptible to hypocarbia and hypercarbia, which are associated with brain and lung morbidities. Transcutaneous CO2 (TcCO2) monitoring allows for continuous non-invasive CO2 monitoring during invasive and non-invasive ventilation and is becoming more popular in the NICU. We aimed to evaluate the correlation and agreement between CO2 levels measured by a TcCO2 monitor and blood gas CO2 (bgCO2) among ELBW infants. This was a prospective observational multicenter study. All infants < 1000 g admitted to the participating NICUs du…

Association of ambient air pollution exposure with low birth weight.

Increasing evidence has shown that exposure to air pollution is linked to adverse birth outcomes, but the results are not consistent. This study was performed on a subset of participants from the UK Biobank between 2006 and 2010. The land use regression (LUR) model was constructed to calculate the concentrations of particulate matter (PM2.5, PM2.5-10 and PM10), nitrogen oxides (NOx), and nitrogen dioxide (NO2). Binary logistic/multivariate linear regression models were applied to explore the potential linear relationships between air pollution exposure and newborn low birth weight (LBW) or BW.…
