Smart eyeglasses with an integrated electrooculogram (EOG) device (JINS MEME ES_R®, JINS Inc.) were evaluated as a quantitative diagnostic tool for blepharospasm. Blepharospasm in connection with chronic subdural haematoma is an uncommon pathology and usually subsides after treatment of the haematoma. Different mechanical and vascular factors may play a role by affecting domapinergic transmission in basal ganglia. A case of a 72-year-old patient is presented, who developed transient blepharospasm as a result of a chronic subdural haematoma and whose symptoms regressed after surgical management.

We present a patient with a facial movement disorder that has characteristics of both blepharospasm and bilateral asynchronous hemifacial spasm. Because of the increased incidence of blepharospasm in patients with hemifacial spasm, our patient's clinical presentation is probably not a chance occurrence, but rather a manifestation of some predisposition for these two movement disorders. Botulinum A toxin is recognized and approved for symptomatic treatment of hemifacial spasm and blepharospasm. The state of trials is good, although double-blind placebo-controlled studies have been carried out involving only a small number of cases. Open case control studies have been done with large patient collectives. In both indications, treatment with botulinum toxin reaches the highest EBM degree in a critical evaluation.

A family study in 29 patients with idiopathic adult-onset blepharospasm (n = 16) and cranial-cervical dystonia (n = 13) was undertaken by examining 189 first-degree relatives. Idiopathic blepharospasm is reexamined and the differential diagnosis discussed on the basis of the signs and symptoms of 27 unpublished cases. As in tardive dyskinesia, middle-aged and elderly women are mostly affected. Contrary to general opinion, spontaneous improvement occurred in about one-fifth of the patients. Blepharospasm is a focal dystonia characterized by involuntary, repetitive eye closure. Orofacial and oromandibular dystonia describe involuntary dystonic movements of orofacial and oromandibular musculature. Hemifacial spasm is characterized by repetitive synchronous contraction of facial nerve innervated muscles on one side of the face. In this article, the clinical presentation, epidemiology, and approaches to treatment are reviewed. Technical aspects of using botulinum toxin for treatment and reported outcomes are discussed.
