An approach to the management of radial longitudinal deficiency.

This paper describes an approach to the management of radial longitudinal deficiency. Early treatment includes manipulation and splintage pr … llows optimal wrist motion. Late treatment of radial longitudinal deficiency includes techniques of bone lengthening and correction of radial deviation deformity. Three cases are presented in which the Ilizarov technique was used to lengthen the forearm and to correct deformity. The complications of forearm lengthening are high but these techniques do allow signif …

Tibial lengthening with ankle arthrodesis in poliomyelitic patients with unilateral dysfunction of both knee extension and ankle dorsiflexion.

OBJECTIVE: The aim of this study was to assess the results of tibial lengthening with ankle arthrodesis in poliomyelitic patients with unilateral dysfunction of both knee extension and ankle dorsiflexion. METHODS: Twenty-five consecutive adult patients with poliomyelitic sequelae of unilateral dysfunction of both knee extension and ankle dorsiflexion were treated. Patients had an average shorthening of 5.4 cm (range: 4.5-6.5 cm) on the affected side. The tibia was osteotomized and lengthened with external fixators, which were then replaced with plates supplemented with bone grafting. Ankle ar

What risk factors predict usage of gastrocsoleus recession during tibial lengthening?

BACKGROUND: Tibial lengthening is frequently associated with gastrocsoleus contracture and some patients are treated surgically. However, the risk factors associated with gastrocsoleus contracture severe enough to warrant surgery during tibial lengthening and the consistency with which gastrocsoleus recession (GSR) results in a plantigrade foot in this setting have not been well defined. QUESTIONS/PURPOSES: We compared patients treated with or without GSR during tibial lengthening with respect to (1) clinical risk factors triggering GSR use, (2) ROM gains and patient-reported outcomes, and (3

Efficacy of PRECICE Nail in Treatment of Adult Patients With Posttraumatic Femoral Leg Length Discrepancy.

Posttraumatic, limb length discrepancy in adults is a challenge to treat, and multiple treatment protocols over the years have shown varying levels of success and complications. Before the introduction of the PRECICE nail in 2011, our preferred method of limb lengthening used an Ilizarov or Taylor Spatial frame. To assess the PRECICE nail, we evaluated the accuracy and complications during treatment in a series of skeletally mature patients with posttraumatic femoral limb length discrepancy. The surgical technique along with a case series of 8 patients are described in detail. On average, the

[Disruption of anterointernal weight bearing: sequelae of surgical treatment of hallux valgus].

Sequelae of forefoot surgery range between 30 and 40%; the most frequent ones are transfer metatarsalgia and deformity relapse. Forefoot surgery complications disrupt biomechanics. Anterointernal weight bearing alteration is most frequent, due to involvement of the metatarsal, the phalanx or both. Metatarsophalangeal arthrodesis normalizes the length of the first ray and pressure transmission. The use of this technique has made it possible to confirm pain relief, as well as improved function and a better cosmetic result, which allows the patient to wear ormal footwear.

Combined monolateral external fixation and intramedullary nailing for oligotrophic femoral nonunion with limb length discrepancy: a case series of technical strategy and surgical outcomes.

Bone lengthening using a monolateral external fixator is an effective technique for treating femoral nonunion associated with limb length discrepancy (LLD). This method involves gradual femoral lengthening to correct shortening and rotational deformities, while promoting bone healing through compression at the nonunion site. We present a case series of three patients demonstrating the successful integration of monolateral external fixation with intramedullary nailing for oligotrophic nonunions (4-8 cm LLD). Preoperative planning includes assessment of limb length discrepancy, evaluation the ca…

The management of spastic deformities of the foot and ankle.

The most common ankle and foot deformities in cerebral palsy are equinus, equinovalgus, equinovarus, calcaneus and hallux valgus. It makes little difference how the length of the triceps surae is re-established and how its stretch reflex is weakened as long as the patient is carefully chosen, the procedure done well and the postoperative regimen prolonged and detailed. The most common cause of failure is inadequate use of night support during growth to prevent recurrence. Significant equinovalgus has been successfully managed by heel cord lengthening and subtalar arthrodesis. The arthrodesis s

Evaluating YouTube as a source of information on limb lengthening surgery: A quality and reliability analysis.

OBJECTIVES: This study aims to evaluate the content quality and reliability of YouTube videos on limb lengthening surgery. MATERIALS AND METHODS: On July 5th, 2025, a YouTube search was performed using the keywords "limb lengthening surgery" and "leg lengthening surgery." The first 100 videos were reviewed; duplicates and those without English audio were excluded, resulting in 53 videos for analysis. Basic characteristics including views, upload time, duration, comments were recorded. Videos were categorized by source as "physician," "speaker," or "patient," and by theme as "general informati

New Technologies in Pediatric Deformity Correction.

The ability to correct limb deformities is one of the core elements of pediatric orthopedics. The term, orthopedics, is derived from the Greek language and means straightening (ortho) children (paidos). New advances in the evaluation and management of children with limb alignment or limb length issues are constantly appearing. This review highlights some of the recent technologies that have been developed to improve the care of these children.

Pre-operative distraction lengthening for radial longitudinal deficiency.

Patients treated for total radial aplasia have been reviewed. It was possible to re-align the carpus by radialization in five out of six limbs treated with pre-operative distraction, but in only one out of six treated without distraction. The average improvement in radial angulation in the non-distraction group was 19 degrees and in the distraction group 38 degrees. The average improvement in radial translation was 16 mm and 17 mm in the two groups respectively. Pre-operative distraction with the Kessler device permits re-alignment of the hand without skeletal resection or excessive tension on

Experimental physeal distraction in immature sheep.

An experimental study on physeal distraction was carried out in the distal femur of 45 two-month-old lambs for the purpose of identifying the basic mechanism of lengthening as well as assessing growth cartilage viability after lengthening. The animals were divided into three groups, each with three subgroups, according to the distraction rate employed (2 mm/day, 1 mm/day, 0.5 mm/day) and the time at which the animals were killed (end of lengthening, 1.5 months postlengthening, and at six months of age). Another group of three animals whose femurs were lengthened at a rate of 0.5 mm/day was kil

Humeral lengthening for septic neonatal growth arrest.

A 9-year-old girl was treated by humeral lengthening for a 8.5 cm deficit caused by a septic arthritis 2 days after birth. A Wagner distraction apparatus was used after an osteotomy was done distal to the deltoid insertion. Six centimeters of length were gained. A second lengthening of about 7.5 cm is planned at the end of the patient's growth. No complications were encountered; by dividing the lengthening into two procedures, none are anticipated. At 1-year follow-up, there was a full range of shoulder, elbow, and hand motion; the humeri were virtually the same length.
