Hammertoe correction with wire fixation of varying length.

BACKGROUND: Proximal interphalangeal (PIP) joint fusion with Kirschner (K) -wire fixation remains a popular strategy for hammertoe correction. This study was performed to evaluate the effect of length of wire fixation on clinical outcomes. METHODS: A retrospective review of all hammertoe reconstructions by a single surgeon was performed. Wire length was chosen at the surgeon's discretion. Outcomes were assessed with metatarsophalangeal (MTP) congruency, pin complications, and PIP union. RESULTS: 157 toes underwent reconstruction. Seventy had wires that spanned the MTP and 87 that did not. Wi

Osteotomy for clinodactyly.

Clinodactyly, finger deviation in the coronal plane, affects the little finger most commonly. Patients typically tolerate clinodactyly without functional limitation, but with increasing deformity, they may have difficulties with specific tasks such as playing musical instruments. In those who do not tolerate the deformity, surgical options include physiolysis for young patients and osteotomy for older patients. An opening wedge osteotomy increases finger length while correcting the coronal deformity. Surgical results of the opening wedge osteotomy are satisfying and complications are rare, wit

A technique for using strain gauges to evaluate airbag interaction with cadaveric upper extremities.

Radius and ulna fractures from airbag deployment onto the forearm have been reported in the literature. Based on laboratory experiments with eight cadaveric upper extremities, this paper presents a method for using strain gages to evaluate upper extremity loading during airbag deployment. The technique provides strain rates, bending moments, and time of fracture for the radius and ulna. Planar rosettes (350 omega, 5% strain) were selected as the best choice given the application to bone with a rosette being placed mid-shaft on both the anterior and posterior surfaces of the radius and ulna. Nonunion of the scaphoid in children treated by Herbert screw fixation and bone grafting. A report of five cases.

In children, nonunion of fractures of the waist of the scaphoid is uncommon. Treatment has included bone grafting without internal fixation, bone grafting with Kirschner-wire fixation, and screw fixation without bone grafting. We report the successful use of Herbert screw fixation and iliac-crest bone grafting in five cases, all of which healed without further surgery. At an average follow-up of 3.3 years, the range of motion, grip strength, carpal stability, and radiological appearances were all excellent.

Marked differences in local bone remodelling in response to different marrow stimulation techniques in a large animal.

Marrow stimulation, including subchondral drilling and microfracture, is the most commonly performed cartilage repair strategy, whereby the subchondral bone plate is perforated to release marrow-derived cells into a cartilage defect to initiate repair. Novel scaffolds and therapeutics are being designed to enhance and extend the positive short-term outcomes of this marrow stimulation. However, the translation of these newer treatments is hindered by bony abnormalities, including bone resorption, intralesional osteophytes, and bone cysts, that can arise after marrow stimulation. In this study,

The use of CO(2) laser in revision stapes surgery: experimental studies on heat transmission to the vestibule.

The aim of the study was to assess the effect of CO(2) laser on stapes prostheses and measure the heat transmission to the vestibule in experiment model. CO(2) laser was applied on two types of prostheses with power settings (2 and 6W; 0.05 s). Transmission of heat to the 'vestibule' was measured using type K thermocouple and DC-80 data logger during application of the laser on prostheses using a training model of temporal bone. Application of the laser on stainless steel prosthesis did not have any effect on the structure of the prosthesis. The use of the laser on the fluoroplastic-wire pisto

Biodegradable and Biocompatible Adhesives for the Effective Stabilisation, Repair and Regeneration of Bone.

Bone defects and complex fractures present significant challenges for orthopaedic surgeons. Current surgical procedures involve the reconstruction and mechanical stabilisation of complex fractures using metal hardware (i.e., wires, plates and screws). However, these procedures often result in poor healing. An injectable, biocompatible, biodegradable bone adhesive that could glue bone fragments back together would present a highly attractive solution. A bone adhesive that meets the many clinical requirements for such an application has yet to be developed. While synthetic and biological polymer

Management of acute paediatric fractures treated surgically in the UK: a cross-sectional study.

INTRODUCTION: The epidemiology of acute paediatric orthopaedic trauma managed surgically across the NHS is poorly described. Compliance against national standards for the management of supracondylar humeral fractures is also unknown at a national level. METHODS: Collaborators in 129 NHS hospitals prospectively collected data on surgically managed acute paediatric orthopaedic trauma cases. Data were collected over a seven-day period and included demographics, injury characteristics, operative details and timing of surgery. A national audit was also undertaken to evaluate compliance with the Br

Risk factors for fracture redisplacement after reduction and cast immobilization of displaced distal radius fractures in children: a meta-analysis.

PURPOSE: Displaced distal radius fractures in children are common and often reduced if necessary and immobilized in cast. Still, fracture redisplacement frequently occurs. This can be prevented by fixation of fracture fragments with K-wires, but until now, there are no clear guidelines for treatment with primary K-wire fixation. This meta-analysis aimed to identify risk factors for redisplacement after reduction and cast immobilization of displaced distal radius fractures in children, and thereby determine which children will benefit most of primary additional K-wire fixation. METHODS: Eight

Adhesive-enhanced sternal closure to improve postoperative functional recovery: a pilot, randomized controlled trial.

BACKGROUND: We previously established a proof-of-concept in a human cadaveric model where conventional wire cerclage was augmented with a novel biocompatible bone adhesive that increased mechanical strength and early bone stability. We report the results of a single-center, pilot, randomized clinical trial of the effects of adhesive-enhanced closure of the sternum on functional postoperative recovery. METHODS: In 55 patients undergoing primary sternotomy, 26 patients underwent conventional wire closure and were compared with 29 patients who underwent adhesive-enhanced closure, which consisted

Atlanto-axial stabilization with posterior transarticular screw fixation: technical description and report of 22 cases.

Magerl's technique of combining C1-C2 posterior screw fixation with a supplemental bone-wire fusion has been advocated for the management of atlanto-axial instability. Between October 1990 and August 1992, a modification of this technique was used in the treatment of 22 patients with this disorder. In the absence of spinal deformity or neoplastic disease, screw fixation and bony fusion were used alone without associated wiring, thus avoiding the risk of neural injury resulting from the sublaminar passage of wire and the retrodisplacement of ventral structures. Patient ages ranged from 30 month
