Are Major Dementias Triggered by Poor Blood Flow to the Brain? Theoretical Considerations.

There is growing evidence that chronic brain hypoperfusion plays a central role in the development of Alzheimer's disease (AD) long before dyscognitive symptoms or amyloid-β accumulation in the brain appear. This commentary proposes that dementia with Lewy bodies (DLB), frontotemporal dementia (FTD), and Creutzfeldt-Jakob disease (CJD) may also develop from chronic brain hypoperfusion following a similar but not identical neurometabolic breakdown as AD. The argument to support this conclusion is that chronic brain hypoperfusion, which is found at the early stages of the three dementias reviewe

Corticospinal Fibers With Different Origins Impact Motor Outcome and Brain After Subcortical Stroke.

BACKGROUND AND PURPOSE: Motor deficit is the most common disability after stroke, and early prediction of motor outcome is critical for early interventions. Here, we constructed a fine map of the corticospinal tract (CST) for early prediction of motor outcome and for understanding the secondary brain changes after subcortical stroke. METHODS: Diffusion spectrum imaging data from 50 healthy adults were used to reconstruct fine maps of CST with different origins, including primary motor area (M1), primary sensory area (S1), premotor cortex, and supplementary motor area (SMA). Their diffusion pr

Inflammatory projections after focal brain injury trigger neuronal network disruption: An 18F-DPA714 PET study in mice.

Due to the heterogeneous pathology of traumatic brain injury (TBI), the exact mechanism of how initial brain damage leads to chronic inflammation and its effects on the whole brain remain unclear. Here, we report on long-term neuroinflammation, remote from the initial injury site, even after subsiding of the original inflammatory response, in a focal TBI mouse model. The use of translocator protein-positron emission tomography in conjunction with specialised magnetic resonance imaging modalities enabled us to visualize "previously undetected areas" of spreading inflammation after focal cortica

Global brain inflammation in stroke.

Stroke, including acute ischaemic stroke and intracerebral haemorrhage, results in neuronal cell death and the release of factors such as damage-associated molecular patterns (DAMPs) that elicit localised inflammation in the injured brain region. Such focal brain inflammation aggravates secondary brain injury by exacerbating blood-brain barrier damage, microvascular failure, brain oedema, oxidative stress, and by directly inducing neuronal cell death. In addition to inflammation localised to the injured brain region, a growing body of evidence suggests that inflammatory responses after a strok

Music therapy for acquired brain injury.

BACKGROUND: Acquired brain injury (ABI) can result in impairments in motor function, language, cognition, sensory processing and emotional disturbances. This may severely reduce a survivor's quality of life. Music therapy has been used in rehabilitation to stimulate brain functions involved in movement, cognition, speech, emotions and sensory perceptions. A systematic review is needed to gauge the efficacy of music therapy as a rehabilitation intervention for people with ABI. OBJECTIVES: To examine the effects of music therapy with standard care versus standard care alone or standard care com

[Subclinical attention changes in transient ischemic attacks in the vertebrobasilar region].

INTRODUCTION: A significant number of patients who have had cerebrovascular illness apparently recover their former abilities completely but return to normal life with subtle cognitive deficits which may affect their daily lives. Such is the situation of patients with transitory ischemic accidents who present with sustained, undiagnosed attention deficits. OBJECTIVES: To identify subclinical alterations due to attention deficits in patients with transitory ischemic accidents, and to contribute to the study of the physiopathological mechanisms involved in the integration of this function. Prosopagnosia and alexia without object agnosia.

Following a trauma causing bilateral posterior brain damage, a patient complained of dyslexia and prosopagnosia, but not object agnosia. On testing she showed intact recognition of object drawings, even when it was assessed with perceptually demanding tasks such as Ghent's overlapping figures and Street completion test. This pattern of deficit is inconsistent with Farah's (1990) prediction that the simultaneous occurrence of alexia and prosopagnosia is invariably associated with object agnosia. The patient's reading performance had the features typically found in letter-by-letter readers. On f

[Diagnostic and therapeutic measures in craniocerebral trauma in the intensive care unit].

Diagnostic and therapeutic measures after severe head-injury in the intensive care unit are discussed. The main goal of all efforts consists in minimizing secondary brain damage. Adequate shock therapy in the initial phase proves crucial for the later outcome. Methods and indications of intracranial pressure monitoring are described. Electrophysiological tests, transcranial doppler sonography, EEG and serial CCT controls provide valuable additional information. Frequent clinical assessments however remain the golden standard despite all technical advances. Surgical and conservative modalities

Focal brain damage enhances experimental allergic encephalomyelitis in brain and spinal cord.

The immunological basis of multiple sclerosis (MS) is well recognized but the factors inducing MS lesions are unclear. In this study, we test the hypothesis that focal brain injury, inflicted during the pre-clinical stages of experimental allergic encephalomyelitis (EAE), will enhance the severity of immunological damage in the cerebral hemispheres and spinal cord. Acute EAE was induced in 30 Lewis rats by the injection of guinea pig spinal cord homogenate in complete Freund's adjuvant. A cryolesion to the surface of the left cerebral hemisphere was induced at 3 days (n = 6) or 8 days (n = 10)

Validity and reliability of the revised Neuropsychological Impairment Scale (NIS).

The revised Neuropsychological Impairment Scale (NIS) is a 95-item, self-reporting screening measure of neuropsychological symptoms. The present report examines the concurrent validity and test-retest stability of the NIS with a neurologically stable sample. The validation study found higher than chance correlations between the NIS and the validity battery and higher correlations with tests that are more sensitive to cognitive impairment. Results also suggest that patients may have more accurate awareness of certain areas of cognitive functioning (academic skills, speech/language functioning)

The subarachnoid spaces in craniosynostosis.

A review of 85 patients with various forms of craniosynostosis showed predictable patterns of dilatation of the subarachnoid spaces in regions of compensatory skull growth. The characteristic pattern in sagittal synostosis (43 patients) included dilatation of the frontal and occipital subarachnoid spaces associated with the elongation of the anteroposterior dimension of the skull and widening of the interhemispheric fissure. In 11 patients with unilateral coronal synostosis, dilatations of the subarachnoid spaces over the contralateral frontal lobe, the sylvian regions, and the ipsilateral tem

Musical hallucinations: a statistical analysis of 46 cases.

Musical hallucinations remain rare, are more common in women, and their onset is often related to ear pathology, particularly deafness. When brain disease is involved, pathology of the right or non-dominant hemisphere seems overrepresented. Psychosis and personality traits play a minimal role in their development. A discriminant function was constructed that correctly predicted the presence of brain disease (89%) and included the following clinical variables: being male, having acute onset musical hallucinations, and absence of deafness, psychiatric disease, and other forms of hallucination.

Dyslexia subtypes: genetics, behavior, and brain imaging.

This study was designed to identify inherited subtypes of specific dyslexia and to characterize these types by a variety of studies. A previous linkage study in large three-generation families resulted in a LOD score of 3.24 at a 13% recombination frequency between dyslexia and normal variations for the short arm of chromosome 15. The odds for linkage with chromosome 15 markers are better than 1,000 to 1. We estimate that 30% of an extended series of families show linkage to chromosome 15 polymorphisms. Other linkages remain to be identified. PET scanning is being used to examine measures of r

Computerized tomographic scan assessment of alcoholic brain damage and its potential reversibility.

The assessment of alcoholic brain damage by computerized tomographic (CT) scanning is reviewed and discussed. Alcoholics showed greater cerebral atrophy than aged-matched neurological controls. Supratentorial atrophy measurements correlated significantly with some neurobehavioral assessment measures. The cerebral atrophy reversed in some subjects with maintained abstinence. Computerized assessment of cerebral spinal fluid volume (cerebral atrophy) and mean cerebral density showed decreased cerebral spinal fluid volume and increased cerebral density with maintained abstinence over 4 weeks in a

Brain-orientated intensive care treatment in severe neonatal asphyxia. Effects of phenobarbitone protection.

The effect of applying brain-orientated neonatal intensive care for term infants with severe neonatal asphyxia was studied. Such treatment included protective phenobarbitone administration together with assisted ventilation and other measures to counteract postasphyxial cerebral oedema and any abrupt changes in blood pressure and oxygenation. The mortality rate and incidence of long-term sequelae were reduced appreciably, resulting in a 0-1 year mortality rate of 14% (previously 50%) and an incidence of neurodevelopmental handicap at 18 months of 17% (previously 50%). It is important in the ma

Subdural effusion protects the aging brain from harmful ventriculomegaly.

The human brain loses its volume and its function during aging. The solid part of the brain within the intracranial space, the brain parenchyma, decreases in volume with age; while the cerebrospinal fluid (CSF) volume increases. With progressive loss of brain parenchymal volume (BPV), CSF may shift from cerebral ventricles to the subdural space, forming subdural effusion (SDE), whose role in the brain aging process remains unclear. We hypothesize that damages associated with ventriculomegaly can be lessened after formation of SDE. As the BPV decreases, the enlarged ventricular surface area cau

Caregiver depression following traumatic brain injury (TBI): a consequence of adverse effects on family members?

Many studies have demonstrated that the behaviour of individuals with traumatic brain injury (TBI) predicts the emotional adjustment of their caregivers. The primary objective of the present study was to obtain an understanding of potential moderating and mediating variables between carer depression and analogous stressors. Seven sets of predictor variables (demographic variables, concurrent stressful life events, behavioural problems, social role problems, extent of adverse effects on family members, appraisal, and support) and the criterion variable of depression in caregivers were examined.

White matter alterations in crossing fibres following traumatic brain injury.

Following traumatic brain injury, the ability of conventional diffusion-weighted MRI analysis techniques to resolve tract-specific white matter damage, particularly in crossing fibre regions, is limited. Using fixel-based analysis, this study aimed to identify white matter abnormalities in chronic traumatic brain injury patients and to resolve the effects of traumatic brain injury on distinct white matter tracts, especially in crossing fibre regions. In this cross-sectional study, diffusion-weighted MRI were acquired from adults with chronic moderate-to-severe traumatic brain injury (N = 29; m

EPS Mid-Career Award 2006. Understanding anterograde amnesia: disconnections and hidden lesions.

Three emerging strands of evidence are helping to resolve the causes of the anterograde amnesia associated with damage to the diencephalon. First, new anatomical studies have refined our understanding of the links between diencephalic and temporal brain regions associated with amnesia. These studies direct attention to the limited numbers of routes linking the two regions. Second, neuropsychological studies of patients with colloid cysts confirm the importance of at least one of these routes, the fornix, for episodic memory. By combining these anatomical and neuropsychological data strong evid
