The value of including optokinetic nystagmus testing in electronystagmography.

Seventy-nine out of 172 patients with abnormal horizontal or vertical optokinetic nystagmus had a specific condition or disease that was diagnosed following a complete ENG. Ocular vertigo, MS, motion sickness, brain stem lesions, cerebellar tumors, cerebellar atrophy, parietal lobe tumors, and Harada syndrome were the diagnoses made. Optokinetic asymmetry--horizontal, vertical, or a combination of these--was the most consistent finding in these conditions (Table). In six patients with cerebellar tumor, abnormal findings on OPK testing suggested the need for further evaluation when physical and

Parkinsonism as a late presentation of lymphomatosis cerebri following high-dose chemotherapy with autologous stem cell transplantation for primary central nervous system lymphoma.

Primary central nervous system lymphoma is an aggressive form of non-Hodgkin lymphoma arising in the eyes, meninges, spinal cord, or brain. Treatment of primary CNS lymphoma with a combination of high-dose chemotherapy and autologous stem cell transplantation has been shown to have high rates of remission which is frequently sustained for multiple years. Recurrence of primary CNS lymphoma generally presents with one or multiple contrast enhancing lesions on MRI. In rare cases, lymphoma cells may proliferate diffusely within the brain parenchyma without mass formation, a pattern termed lymphoma

[Method of unidimensional echolocation of the cerebral aqueduct and upper portions of the 4th ventricle in healthy subjects and in pathology].

The authors suggest a method of unichamber echo location of the aqueduct and upper parts of the fourth ventricle by means of which it is possible to determine displacement of these parts of the c.s.f. system in a plane close to the frontal plane in development of a volumetric focus in the posterior cranial fossa. The value of the method in making the diagnosis of a surgical pathological condition in the posterior cranial fossa is almost the same as the value of unichamber echo diagnosis of supratentorial processes.

Brain tumor in newborn babies.

5 cases of brain tumor in newborn babies under 2 months are presented. 4 of them were supratentorial teratoma and originated from the midline, and 1 was a glioma at the cerebelloponitine angle. 2 cases died before surgery and 2 cases after surgery. In our 5th case a benign teratoma of 150 g was removed from the third ventricle. He was discharged and enjoyed rather good health for 2 years and 1 month before dying of recurrence of tumor. Internal hydrocephalus associated with the huge tumor was responsible for the sudden deterioration of general condition. Therefore, shunting procedures should b

Immunosuppressive Tumor Microenvironment and Therapeutic Landscape of Diffuse Intrinsic Pontine Glioma.

Diffuse intrinsic pontine glioma (DIPG) is a highly aggressive and fatal pediatric brainstem tumor, characterized by rapid progression and extremely poor prognosis. Its infiltrative growth within the pons, combined with the critical function and inaccessibility of the brainstem, makes surgical resection different and significantly limits therapeutic options. Moreover, the molecular and cellular heterogeneity of DIPG complicates therapeutic development, and despite decades of research, the current standard of care remains limited to palliative radiotherapy. In recent years, tumor immunology in

Pseudohallucination and Pilocytic Astrocytoma in the Pons.

BACKGROUND: The pons, a critical structure located in the brainstem, plays a multifaceted role in various neurological functions, and any disruption to its integrity can lead to profound and diverse clinical manifestations. Recent reports have highlighted the association between pontine lesions and conditions such as emotional incontinence, rage, auditory hallucinations, and visual disturbances. We refer to a novel case that causes false hallucinations. We discuss a patient with a tumor in the pons exhibiting a rare constellation of symptoms that included unstable auditory, visual, and percept

Brainstem glioblastoma in a neurofibromatosis type 1 patient: A rare but aggressive presentation.

Glioblastoma multiforme is a highly aggressive central nervous system tumor, rarely seen in the infratentorial region, particularly in patients with Neurofibromatosis Type 1 (NF1). While NF1 is more commonly associated with low-grade gliomas, the development of high-grade gliomas such as glioblastoma remains uncommon and poses diagnostic and therapeutic challenges. We report a rare case of brainstem glioblastoma IDH-wild type in an adolescent with NF1, initially presenting with nonspecific neurological symptoms. Imaging revealed a lesion in the cerebello-medullary region, and histopathology co

[Current Status and Future Perspectives of Clinical Trials for Diffuse Intrinsic Pontine Gliomas(DIPG)].

Diffuse intrinsic pontine glioma (DIPG) remains one of the most devastating pediatric central nervous system tumors, with a median survival of approximately 11 months despite decades of research. The identification of histone H3 K27M mutations marked a pivotal moment, leading to major advancements in understanding the molecular and epigenetic characteristics of these tumors. This discovery enabled molecular classification and provided a basis for the development of novel therapeutic strategies. In recent years, clinical trials have investigated molecular targeted agents and epigenetic modulato

Impact of Nanotechnology on Glioblastoma Studies: From Brain Stem Cells to Neoplastic Development and Cancer Gene Therapy.

The brain malignant tumor Glioblastoma multiforme (GBM) has a median survival of 14-16 months using current treatments; thus, understanding the pathology of GBM is crucial for proposing new therapies and increasing overall survival outcomes. Therefore, this study aimed to analyze different elements, particularly growth factors and the related signal transduction pathways, which play a role in brain neoplastic development, from stem cells to established solid brain tumors, and the application of current immunology techniques, molecular biology, and nanotechnology. Targeting growth factors, espe

Radiotherapy-Induced Stevens-Johnson Syndrome/ Toxic Epidermal Necrolysis in an Adolescent Girl With Diffuse Glioma of the Pons.

Radiotherapy is a rare cause of Stevens-Johnson syndrome (SJS)/ toxic epidermal necrolysis (TEN), especially in the pediatric age group. Most of the reported cases were concomitantly started on anti-epileptic drugs. Herein, we present a case of radiotherapy induced SJS/TEN in an adolescent girl in the absence of anti-epileptic drug use. Radiotherapy should be included as a potential cause for SJS/TEN, recognizing the important impact on management of the underlying malignancy.

Spontaneous early-onset neurodegeneration in the brainstem and spinal cord of NSG, NOG, and NXG mice.

The spectrum of background, incidental, and experimentally induced lesions affecting NSG and NOG mice has been the subject of intense investigation. However, comprehensive studies focusing on the spontaneous neuropathological changes of these immunocompromised strains are lacking. This work describes the development of spontaneous early-onset neurodegeneration affecting both juvenile and adult NSG, NOG, and NXG mice. The study cohort consisted of 367 NSG mice of both sexes (including 33 NSG-SGM3), 61 NOG females (including 31 NOG-EXL), and 4 NXG females. These animals were primarily used for p

Diencephalic syndrome-like presentation of brainstem tumor: a series case-based review.

PURPOSE: Russel described a rare clinical entity known as diencephalic syndrome (DS) in 1951, which was traditionally caused by a neoplasm in the hypothalamic-optic chiasmatic region. DS is characterized by severe emaciation despite adequate or slightly reduced caloric intake, locomotor hyperactivity, euphoria and other minor features. Current evidence suggests that a rare population of children with a similar phenotype may have their tumor located in the posterior fossa instead, defining the DS-like presentation, a rare entity with few cases reported in the literature. METHODS: A thorough se

No wrong decisions in an all-wrong situation. A qualitative study on the lived experiences of families of children with diffuse intrinsic pontine glioma.

BACKGROUND: Diffuse intrinsic pontine glioma (DIPG) is a rare, but lethal pediatric brain tumor with a median survival of less than 1 year. Existing treatment may prolong life and control symptoms, but may cause toxicity and side effects. In order to improve child- and family-centered care, we aimed to better understand the treatment decision-making experiences of parents, as studies on this topic are currently lacking. PROCEDURE: The data for this study came from 24 semistructured interviews with parents whose children were diagnosed with DIPG in two children's hospitals in Switzerland and d

Proton radiotherapy in the treatment of IDH-mutant diffuse gliomas: an early experience from shanghai proton and heavy ion center.

PURPOSE: According to the presence or absence of isocitrate dehydrogenase (IDH) mutation, the 2021 WHO classification system bisected diffuse gliomas into IDH-mutant tumors and IDH-wildtype tumors. This study was aimed to evaluate the outcomes of proton radiotherapy treating IDH-mutant diffuse gliomas. PATIENTS AND METHODS: Between May 2015 and May 2022, a total of 52 consecutive patients with IDH-mutant diffuse gliomas were treated at Shanghai Proton and Heavy Ion Center. Tumor histologies were 33 cases of astrocytoma and 19 cases of oligodendroglioma. Tumor classified by WHO grade 2, 3 and

Diffuse Intrinsic Pontine Glioma (DIPG): Breakthrough and Clinical Perspective.

Diffuse intrinsic pontine glioma (DIPG) mainly affects children with a median age of 6-7 years old. It accounts for 10% of all pediatric tumors. Unfortunately, DIPG has a poor prognosis, and the median survival is generally less than 16-24 months independently from the treatment received. Up to now, children with DIPG are treated with focal radiotherapy alone or in combination with antitumor agents. In the last decade, ONC201 known as dopamine receptor antagonist was uncovered, by a high throughput screening of public libraries of compounds, to be endowed with cytotoxic activity against severa

Current knowledge on the immune microenvironment and emerging immunotherapies in diffuse midline glioma.

Diffuse midline glioma (DMG) is an incurable malignancy with the highest mortality rate among pediatric brain tumors. While radiotherapy and chemotherapy are the most common treatments, these modalities have limited promise. Due to their diffuse nature in critical areas of the brain, the prognosis of DMG remains dismal. DMGs are characterized by unique phenotypic heterogeneity and histological features. Mutations of H3K27M, TP53, and ACVR1 drive DMG tumorigenesis. Histological artifacts include pseudopalisading necrosis and vascular endothelial proliferation. Mouse models that recapitulate hum

Early life-threating enlargement of a vestibular schwannoma after gamma knife radiosurgery.

Stereotactic gamma knife radiosurgery (GKS) may induce a transient enlargement of vestibular schwannomas (VS). This phenomenon, known as pseudoprogression or swelling, starts at about 3 months following GKS, peaks at about 6 months, and typically subsides thereafter, usually without significant neurological deterioration. We describe a 34-year-old female who developed an aggressive enlargement of a VS 1 month after GKS. The patient was treated with an immediate external ventricular drainage and surgical resection via retrosigmoid approach for an acute neurological deterioration due to hydrocep

Diffuse intrinsic pontine glioma: molecular landscape and emerging therapeutic targets.

PURPOSE OF REVIEW: Diffuse intrinsic pontine glioma (DIPG) is a fatal childhood brainstem malignancy. Despite advances in understanding of the molecular underpinnings of the tumor in the past decade, the dismal prognosis of DIPG has thus far remained unchanged. This review seeks to highlight promising therapeutic targets within three arenas: DIPG cell-intrinsic vulnerabilities, immunotherapeutic approaches to tumor clearance, and microenvironmental dependencies that promote tumor growth. RECENT FINDINGS: Promising therapeutic strategies from recent studies include epigenetic modifying agents

Clinical trials for diffuse intrinsic pontine glioma: the current state of affairs.

PURPOSE: Diffuse intrinsic pontine glioma (DIPG) is a lethal high-grade pediatric brainstem tumor without a cure. Despite numerous clinical trials over the last decades, the prognosis has remained poor. The aim of this update was to report on the status and outcomes of all clinical trials for DIPG performed to better understand the landscape of research efforts for this diagnosis to date. METHODS: The ClinicalTrials.gov database was reviewed in May 2019 for all possible interventional clinical trials that included DIPG as a diagnosis of primary investigation. These were then screened against
