Third and fourth branchial cleft cysts - A 25-year institutional experience of imaging detection and pathologic diagnosis.

BACKGROUND AND PURPOSE: Third and fourth branchial cleft cysts (BCCs) are rare and often mimic other lower-neck cystic lesions. Tract conspicuity is frequently limited on single-time-point imaging, particularly during inflammation. We report a 25-year institutional experience evaluating Proportion Undergoing Surgery and Pathology-Confirmed BCC Rate. MATERIALS AND METHODS: We retrospectively queried radiology reports (January 2000-December 2024) to identify suspected third and fourth BCCs. Reports and images were reviewed to classify lesions; pathology was cross-referenced for operated cases.

Branchiootic syndrome--a clinical case report and review of the literature.

Branchiootoic syndrome is part of the spectrum of brachiootorenal disorders. Brachiootorenal disorder is a rare autosomal dominant condition, characterized by malformations of the outer, middle, and inner ear, which are associated with branchial and renal anomalies. We describe a case of bilateral branchiootoic syndrome and discuss the anatomy of second branchial cleft fistulae and the surgical management of this uncommon condition. We report the case of a 6-year-old girl referred to our department with bilateral intermittently discharging neck swellings. Clinical examination revealed bilatera

Branchial cleft cyst at an unusual location: a rare case with a brief review.

A branchial cleft cyst (BCC) commonly presents as a solitary, painless mass in the neck of a child or young adult. They are most commonly located along the anterior border and the upper third of the sternocleidomastoid muscle in the anterior triangle of the neck. It is very rare for a BCC to manifest in other locations, especially in the posterior triangle of the neck. BCCs are believed to be derived from the branchial apparatus, mostly from the second branchial arch, although many theories have been proposed to explain the aetiology of BCCs. It is possible for BCCs to be easily misdiagnosed a

First branchial arch fistula: diagnostic dilemma and improvised surgical management.

First branchial cleft anomalies are uncommon, and only sporadic case reports are published in the literature. They account for 1% to 8% of all the branchial abnormalities. The often variable presentation and tract siting of first arch fistulae have led to misdiagnosis. The misdiagnosis results in inappropriate/ineffective treatment and recurrence of the sinus tract. We present a 19-year-old woman who presented to the ENT outpatient department with episodic discharge from a long-standing fistula anterior to the left sternomastoid muscle. This was associated with repeated episodes of ipsilateral

Management of congenital fourth branchial arch anomalies: a review and analysis of published cases.

BACKGROUND/PURPOSE: Congenital fourth branchial arch anomalies are uncommon entities, heretofore described only in case reports, affecting primarily children, and typically presenting as a cervical inflammatory process. The aim of the study was to collect appropriate data on the diagnosis, treatment, and outcome of this condition and to suggest guidelines for its management. METHODS: We conducted a structured review of the literature for cases explicitly identified as congenital fourth branchial arch anomalies or meeting anatomical criteria for this condition. We computed descriptive statisti

[Tumors of the submandibular triangle].

Analysed material consists of 180 patients with different kind of submandibular triangle tumors, excluded disturbances of the submandibular gland. All these patients were treated in ENT Department of County Hospital No 1 in Rzeszów in 1974-1993. Among 180 patients in 97 (54%) cases, the presence of inflammatory tumors were stated: non specific inflammation in 74 (76%) patients and specific inflammation in 23 (24%) patients. In 10 (6%) patients occurred branchial or dermoid cysts. Neoplastic tumors occurred in 73 (40%), malignant tumors in 64 (86%) patients and benign ones in 9 (12%) patients.

p16(INK4A) immunohistochemical staining may be helpful in distinguishing branchial cleft cysts from cystic squamous cell carcinomas originating in the oropharynx.

BACKGROUND: We investigated p16(INK4A) expression in branchial cleft cysts and its utility in distinguishing branchial cleft cysts from metastatic head and neck squamous cell carcinomas (SCCs) in fine-needle aspiration biopsies (FNABs). METHODS: A study set comprising 41 resections (15 SCC and 26 branchial cleft cysts) and a test set of 15 FNABs (11 SCC and 4 branchial cleft cysts) were analyzed with p16(INK4A) immunohistochemistry and human papillomavirus (HPV) polymerase chain reaction (PCR)/pyrosequencing. Cases with discrepant p16(INK4A) and PCR/pyrosequencing results were further evaluat

Management of second branchial cleft abnormalities in adults.

OBJECTIVE: Branchial cleft anomalies are developmental disorders of the neck. Our aim is to report the diagnostic and treatment procedure followed in three cases of second branchial cleft abnormalities in adults. METHODOLOGY: A patient aged over 40 underwent surgical excision of a well-encapsulated cystic neck structure and two further patients of a branchial sinus and branchial fistula respectively. Prior to surgery the patients were assessed by means of imaging techniques, FNA cytology and cytometric DNA analysis. RESULTS: Neutrophils, debris, mature squamous epithelial cells including deg

Parapharyngeal space tumours.

Parapharyngeal space tumours are rare, forming 0.5% of all head and neck tumours. They are of interest because of the varied histological types, diagnostic puzzle and challenge at surgical excision. Thirteen patients, seen over an eight year period (from 1981 to 1988) in the Department of Otolaryngology, Singapore General Hospital, are presented. Clinically most patients presented with a painless upper neck lump and on examination, a lateral pharyngeal wall bulge was noted. The main investigation after ruling out a nasopharyngeal primary tumour is a high resolution Computer Tomographic (CT) sc

Familial aggregation of neuroectodermal and gastrointestinal tumors.

A family with multiple tumors is presented. Four cases were childhood neoplasms derived from the neuroectoderm, as was a malignant melanoma in a young female and a branchiogenic cyst in her brother. He also had an early onset of an adenocarcinoma of the transverse colon. Four further cases of gastrointestinal tumors and one case of acute myelogenous leukemia had occurred in the family. One of the childhood tumors was a bilateral retinoblastoma. It is discussed as to whether the tumors in the studied family were due to a genetic factor resulting in a neuroectodermal embryopathy, or were caused

Image cytometry DNA-analysis of fine needle aspiration cytology to aid cytomorphology in the distinction of branchial cleft cyst from cystic metastasis of squamous cell carcinoma: a prospective study.

OBJECTIVE: Frequently, the distinction between branchial cleft cyst and cystic metastases from squamous cell carcinoma is difficult by cytomorphology. In a prospective study, we investigated the need for, and the value of, image cytometry DNA-analysis as a complement to cytologic evaluation of cystic lesions in the neck. STUDY DESIGN: Image cytometry DNA-analysis was performed on the fine needle aspiration cytology smears from 50 patients, referred to our department, with a solitary cystic lesion in the lateral region of the neck. METHODS: Smears from aspirates were Giemsa stained and cytolo

Congenital fistula of the fourth branchial pouch.

Between 1986 and 1995, 128 patients were treated for various head and neck congenital malformations at Saint-Luc University Hospital, Louvain. We report three cases of fourth branchial pouch cysts requiring surgical removal. One of these cases presented with a third branchial pouch remnant on the same side and subsequently a fourth branchial pouch sinus. To our knowledge, this is the first case published in the literature. A fourth branchial pouch sinus tract can become manifest clinically by recurrent episodes of neck abscess or acute suppurative thyroiditis (especially in infants). The tract

Cervical node metastases presenting with features of branchial cysts.

For many years it has been recognized that seemingly benign neck cysts may contain carcinoma. The true incidence is unknown. This paper investigated nine out of 270 patients presenting with a neck mass--which proved to contain a squamous carcinoma. Records (from a 30-year period) of over 3400 patients with squamous carcinoma of the head and neck, were examined. The histology slides were reviewed, the number of cystic lesions was noted and also the clinical outcome. Out of the 270 patients nine presented with a cystic lesion and these were studied. Six cystic lesions were originally diagnosed a

Posterior Auricular and External Auditory Canal Involvement from an Infected Preauricular Fistula Mimicking a First Branchial Cleft Cyst.

Preauricular fistula is a common congenital anomaly that typically presents with localized infection near the auricular pit. We report a case of an infected preauricular fistula in a 9-year-old girl presenting with atypical inflammation involving the posterior auricular sulcus and the external auditory canal, initially mimicking a first-branchial cleft anomaly. Despite prolonged antibiotic therapy, the infection persisted. Contrast-enhanced computed tomography suggested a branchial cleft lesion. However, intraoperative findings demonstrated that the fistulous tract terminated within the auricu

A cross-sectional evaluation of outcomes of pediatric branchial cleft cyst excision.

OBJECTIVE: To examine complications following pediatric branchial cleft cyst excision by surgical specialty, demographics, and comorbid conditions. METHODS: A retrospective review of the National Surgical Quality Improvement Program database was performed. Pediatric cases from January 1, 2015 through May 1, 2017 with a current procedural terminology code of 42810 (excision branchial cleft cyst or vestige, confined to skin and subcutaneous tissues) or 42815 (excision branchial cleft cyst, vestige, or fistula, extending beneath subcutaneous tissues and/or into the pharynx) were included.
