Association of mitochondrial allele 4216C with increased risk for sepsis-related organ dysfunction and shock after burn injury.

Impaired mitochondrial activity has been linked to increased risk for clinical complications after injury. Furthermore, variant mitochondrial alleles have been identified and are thought to result in decreased mitochondrial activity. These include a nonsynonymous mitochondrial polymorphism (T4216C) in the nicotinamide adenine dinucleotide dehydrogenase 1 gene (ND1), encoding a key member of complex I within the electron transport chain, which is found almost exclusively among Caucasians. We hypothesized that burn patients carrying ND1 4216C are less able to generate the cellular energy necessa

Noninvasive Positive Pressure Ventilation in burns.

OBJECTIVE: Acute respiratory failure is a common complication of the severely burn-injured patient. Endotracheal intubation and mechanical ventilation is associated with a high rate of complications. Noninvasive Positive Pressure Ventilation (NIPPV) has been shown to be as effective as conventional ventilation in improving gas exchange and is associated with fewer complications with patients in acute hypercapnic and hypoxaemic respiratory failure. We report our experience with NIPPV in 30 burn patients. METHOD: The records of all burn patients from 1998 to 2000, where NIPPV was used as part o

Pulmonary embolism in burned children.

There are occasional reports in the literature concerning the incidence of pulmonary embolism in the postburn population, but reports of burned children are especially rare. The clinical diagnosis of pulmonary embolism is particularly difficult in these populations due to the postburn pulmonary complications of pneumonia, bronchopneumonia, respiratory distress syndrome, and changes incurred through inhalation injury. A retrospective review of all patient deaths occurring at this institution during the past 22 years was performed in order to document the incidence of pulmonary embolism in burne

Endoscopic treatment of glottic stenosis secondary to caustic injury with silastic keel placement: a case report.

The use of an endoscopic approach for the division of glottic webs or stenosis has been reported in the literature and has been mainly confined to the anterior commisure. We report a rare case of caustic injury to the upper aerodigestive tract that resulted in extensive web formation along the membranous vocal cord which was successfully treated with endoscopic lysis of the adhesions and the use of a silastic sheet keel as a stent.

Relationship Between Patient Characteristics and Number of Procedures as well as Length of Stay for Patients Surviving Severe Burn Injuries: Analysis of the American Burn Association National Burn Repository.

This study establishes important, national benchmarks for burn centers to assess length of stay (LOS) and number of procedures across patient profiles. We examined the relationship between patient characteristics such as age and total body surface area (TBSA) burned and number of procedures and LOS in the United States, using the American Burn Association National Burn Repository (NBR) database version 8.0 (2002-2011). Among 21,175 surviving burn patients (TBSA > 10-60%), mean age was 33 years, and mean injury size was 19.9% TBSA. Outcomes included the number of debridement, excision, autograf

[Acute death from scalding: vital reactions (author's transl)].

After a boiler explosion on a ship 12 men with cutaneous scalding of 90-100% of body surface were found dead. 15 other fatalities occured after different intervals. This investigation deals with the histomorphologic changes in those 12 cases of instantaneous death. The most important findings consisted of: 1. Thermal injuries of the skin with multiple subcutaneous and corial bubbles, intravascular heat coagulation, and edema in the transitional zone to the undamaged submucosa. 2. Coagulation necrosis of the tracheal and bronchial wall extending to alveolar ducts in central parts of the lung. M

Development and validation of web-based, interpretable predictive models for sepsis and mortality in extensive burns.

BACKGROUND: Burn injuries are a common cause of trauma globally, with extensive burns (≥ 50% total body surface area burned) associated with high rates of sepsis and mortality. This study aims to identify risk factors associated with sepsis and mortality in extensively burned patients and to develop accurate, interpretable predictive models via machine learning algorithms. METHODS: A retrospective cohort study was conducted utilizing data from two Burn Critical Care Units in Eastern China from 2012-2023. A total of 237 patients with extensive burns were included. We applied ten machine learni

Update on pediatric advanced life support guidelines.

Accidents are a leading cause of death for children in several nations. Motor vehicle accidents are among the most common causes of cardiac arrest. Other causes include drowning, burns, gunshot wounds, poisoning, smoke inhalation, and airway obstruction caused by asphyxiation from foreign bodies. Approximately 50% to 65% of children who require cardiopulmonary resuscitation are younger than 1 year of age, with most being younger than 6 months of age. To prevent loss and improve quality of life, it is imperative to initiate measures to improve oxygenation and treat cardiac dysfunction. Guidelin

[Anesthesia and resuscitation in Arabo-Islamic medicine: analytic study through Ibn Sina].

Nervous system anatomy knowledge acquired by arabo-islamic physicians enabled them to know its physiology represented by its excitability and its conductibility, and to understand pain physiology. Ibn Sina in his book entitle "The Canon of Medicine" precise anesthetic drugs and their side effects. Among these anesthetic means there were opium and ice. Ibn Sina distinguished organic pain and psychogenic pain. He used sedative and antalgic and soporific drugs in treatment of some psychologic diseases as melancholia. Moslem physicians were the first to use cold water to treat superficial burns. A

A model for acute kidney injury in severe burn patients.

INTRODUCTION: In patients with severe burns, morbidity and mortality are high. One factor related to poor prognosis is acute kidney injury. According to the AKIN criteria, acute kidney injury has 3 stages based on urine output, serum creatinine level, and renal replacement therapy. In this study, we aimed to create a decision tree for estimating risk of acute kidney injury in patients with severe burn injuries. METHODS: We retrospectively evaluated 437 adult patients with ≥20% total burn surface area injury who were treated at the Baskent University Ankara and Konya Burn Centers from January

Dementia: A risk factor for burns in the elderly.

BACKGROUND: Older people are disproportionately at risk of burn and have a high risk of dementia; however the impact of dementia on risk of burn is unknown. METHOD: Linked hospitalisation and death records for individuals aged 65 years and older admitted to a NSW hospital for a burn over the ten year period 2003-2012 were analysed. Demographic and burn characteristics and health outcomes were compared for people with and without dementia. Incidence rates were calculated per 100,000 population and negative binomial regression was used to examine temporal trends. RESULTS: Of the 1535 older peo

A review of the utility of high-frequency oscillatory ventilation in burn and trauma ICU patients.

PURPOSE OF REVIEW: The purpose was to examine the utility of high-frequency oscillatory ventilation (HFOV) in trauma and burn ICU patients who require mechanical ventilation, and provide recommendations on its use. RECENT FINDINGS: HFOV may be beneficial in burn patients with smoke inhalation injury with or without acute lung injury/acute respiratory distress syndrome (ARDS), as it improves oxygenation and minimizes ventilator-induced lung injury. It also may have a role in improving oxygenation in trauma patients with blast lung injury, pulmonary contusions, pneumothorax with massive air lea

Transfusion Requirement in Burn Care Evaluation (TRIBE): A Multicenter Randomized Prospective Trial of Blood Transfusion in Major Burn Injury.

OBJECTIVE: Our objective was to compare outcomes of a restrictive to a liberal red cell transfusion strategy in 20% or more total body surface area (TBSA) burn patients. We hypothesized that the restrictive group would have less blood stream infection (BSI), organ dysfunction, and mortality. BACKGROUND: Patients with major burns have major (>1 blood volume) transfusion requirements. Studies suggest that a restrictive blood transfusion strategy is equivalent to a liberal strategy. However, major burn injury is precluded from these studies. The optimal transfusion strategy in major burn injury

Predictors of carbon monoxide and hydrogen cyanide exposure in smoke inhalation patients.

OBJECTIVE: A prospective study of civilian (nonfirefighter) smoke inhalation patients was carried out to test the hypotheses that: 1) absorption of carbon monoxide and hydrogen cyanide from smoke can be predicted by clinical examination and historical data; and, more specifically 2) a history of exposure to burning synthetic polymers is an important predictor of systemic cyanide levels. METHODS: The study was conducted over a three-year period at six urban hospitals. Patients with or without burns who were exposed to smoke within five hours of hospital arrival were sampled for carboxyhemoglob
