We examine pre-COVID declines in transit ridership, using Southern California as a case study. We first illustrate Southern California's unique position in the transit landscape: it is a large transit market that demographically resembles a small one. We then draw on administrative data, travel diaries, rider surveys, accessibility indices, and Census microdata for Southern California, and demonstrate a strong association between rising private vehicle access, particularly among the populations most likely to ride transit, and falling transit use. Because we cannot control quantitatively for t…

BACKGROUND AND OBJECTIVES: The homeless population in the US is aging. Cognitive impairment is prevalent in this population, yet little is known about the neurologic etiologies of such impairment. Addressing this gap in knowledge is important because homeless older adults with cognitive impairment due to neurodegenerative disease may need lifelong tailored support to obtain and maintain housing. In this study, we characterized the neurocognitive health of a sample of adults who experienced homelessness for the first time after age 50 using gold standard behavioral neurology examination practic

BACKGROUND: Hospital-based care accounts for one third of US health spending or over $1 trillion annually, yet a detailed all-payer assessment of what services contribute to this spending is not available.
STUDY DESIGN: Cross-sectional and longitudinal evaluation of hospital financial statements from acute-care general hospitals in California between fiscal years 2007 and 2016. The amounts spent on 41 different revenue centers were included. The primary outcome was state-level and hospital-level spending for each revenue center including decomposing growth trends into changes in volume and pr…

The California Arbovirus Surveillance Program was initiated over 50 years ago to track endemic encephalitides and was enhanced in 2000 to include West Nile virus (WNV) infections in humans, mosquitoes, sentinel chickens, dead birds and horses. This comprehensive statewide program is a function of strong partnerships among the California Department of Public Health (CDPH), the University of California, and local vector control and public health agencies. This manuscript summarizes WNV surveillance data in California since WNV was first detected in 2003 in southern California. From 2003 through …

The western blacklegged tick, Ixodes pacificus, an important vector in the western United States of two zoonotic spirochetes: Borrelia burgdorferi (also called Borreliella burgdorferi), causing Lyme disease, and Borrelia miyamotoi, causing a relapsing fever-type illness. Human cases of Lyme disease are well-documented in California, with increased risk in the north coastal areas and western slopes of the Sierra Nevada range. Despite the established presence of B. miyamotoi in the human-biting I. pacificus tick in California, clinical cases with this spirochete have not been well studied. To as…

Although West Nile virus (WNV) infects many avian species, the prevalence and impact of WNV in hummingbirds (Family Trochilidae) have not been described. We focused on hummingbird mortality and WNV infection using data from the state of California's Dead Bird Surveillance Program for the years 2005-17. The overall prevalence of WNV in tested carcasses was 10.1% (49/486), with annual prevalence ranging from 0-28%. Prevalence by county ranged from 0-40% while prevalence by species ranged from 0-12%. We demonstrated that hummingbirds were naturally infected with WNV and displayed temporal and spa…

Sporadic cases of cryptococcosis have been reported in marine mammals, typically due to Cryptococcus neoformans and, more recently, to Cryptococcus gattii in cetaceans. Cryptococcus albidus, a ubiquitous fungal species not typically considered to be pathogenic, was recovered from a juvenile California sea lion (Zalophus californianus) rescued near San Francisco Bay, California. Yeast morphologically consistent with a Cryptococcus sp. was identified histologically in a lymph node and C. albidus was identified by an rDNA sequence from the lung. Infection with C. albidus was thought to have contr…

Bartonella spp. were detected in rats (Rattus norvegicus) trapped in downtown Los Angeles, California, USA. Of 200 rats tested, putative human pathogens, B. rochalimae and B. tribocorum were found in 37 (18.5%) and 115 (57.5%) rats, respectively. These bacteria among rodents in a densely populated urban area are a public health concern.

Breeder squab candidates between the ages of 6 and 16 wk were submitted to the California Animal Health and Food Safety Laboratory, Turlock branch, as a result of respiratory distress and increased mortality. These cases were submitted from one Northern California commercial squab operation on three separate occasions occurring between December 2007 and March 2008. Severe trichomoniasis was identified, primarily in the tracheal epithelium and lung of squabs, with few or no lesions in the oral cavity, crop, esophagus, and livers, where the organism commonly infiltrates. Infiltration of the trac…

California shares a 150-mile international border with Mexico. Traditionally, this border has seen non-stop illegal migration. In the 1990s, the Border Patrol began a concerted effort to establish and maintain control of the border, beginning in urban San Diego. This heightened law enforcement presence, known as Operation Gatekeeper, changed the westernmost segment of the border from the most permeable to the least permeable. This enforcement pushed migrants into more dangerous crossing areas in eastern San Diego and Imperial Counties, making their trip longer and more physically challenging a…

BACKGROUND: There is no consensus whether firefighters are at increased cancer risk for particular cancers. Previous studies have been small, mostly investigated cancer mortality, and suggested increased risks for brain, bladder, testicular, prostate, thyroid and colo-rectal cancers, leukemia, and melanoma.
METHODS: Records of all male cancers registered in California during 1988-2003 were obtained. Firefighters were identified from occupation and industry text fields. Logistic regression analysis used other cancers as controls.
RESULTS: Of the 804,000 eligible records, 3,659 had firefightin…

CONTEXT: Between 1994 and 2003, tuberculosis (TB) cases in California declined 33% (4834 to 3224). However, in 2003 California reported the largest number of cases in the nation, and over the past decade the proportion of cases with multidrug-resistant tuberculosis (MDR-TB) has not decreased.
OBJECTIVE: To describe the magnitude, trends, geographic distribution, clinical characteristics, risk factors, and outcomes of MDR-TB cases reported to the California registry of Reports of Verified Cases of TB. DESIGN, SETTING, AND CASES: Analysis of 38,291 TB cases reported from all 61 local health jur…

OBJECTIVE: Craniosynostosis, a malformation caused by premature closure of 1 or more cranial sutures, is a rare birth defect usually of unknown cause; however, it is often associated with advanced maternal age. Because fertility treatments are also associated with increased maternal age, this study investigated the possible association between fertility treatments and craniosynostosis.
METHODS: Data from the Birth Defect Risk Factor Surveillance study were used, which included infants who were delivered from 1993 through 1997 in California, Georgia, and Iowa. Cases were defined as infants who…

BACKGROUND: Work-related asthma is a leading cause of occupational respiratory illness.
METHODS: Work-related asthma was studied in California over a 36-month period, from March 1, 1993 to February 29, 1996. The surveillance system identified cases from Doctor's First Reports (DFRs), a mandated physician reporting system. Structured follow-up telephone interviews of DFR asthma cases were conducted to collect work history, exposure, and medical information. Statewide employment data was used to calculate disease rates among industry groups.
RESULTS: Based on 945 cases of work-related asthma, …
