BMP10-mediated ALK1 signaling is continuously required for vascular development and maintenance.

Hereditary hemorrhagic telangiectasia (HHT) is an autosomal-dominant vascular disorder characterized by development of high-flow arteriovenous malformations (AVMs) that can lead to stroke or high-output heart failure. HHT2 is caused by heterozygous mutations in ACVRL1, which encodes an endothelial cell bone morphogenetic protein (BMP) receptor, ALK1. BMP9 and BMP10 are established ALK1 ligands. However, the unique and overlapping roles of these ligands remain poorly understood. To define the physiologically relevant ALK1 ligand(s) required for vascular development and maintenance, we generated

Dietary Branched-Chain Amino Acids Modify Postinfarct Cardiac Remodeling and Function in the Murine Heart.

BACKGROUND: Branched-chain amino acids (BCAAs), which are derived from the diet, are markedly elevated in cardiac tissue following myocardial infarction (MI). Nevertheless, it remains unclear whether dietary BCAA levels influence post-MI remodeling. METHODS: To investigate the impact of dietary BCAAs on cardiac remodeling and function after MI, we fed mice a low or a high BCAA diet for 2 weeks before MI and for 4 weeks after MI. Cardiac structural and functional changes were evaluated by echocardiography, gravimetry, and histopathological analyses. Immunoblotting was used to evaluate the effe

Neonatal Impedance Cardiography in Asphyxiated Piglets-A Feasibility Study.

OBJECTIVES: Impedance cardiography (ICG) is a non-invasive method for continuous cardiac output measurement and has the potential to improve monitoring and treatment of sick neonates. PhysioFlow® is a signal-morphology ICG-system showing promising results in adults with low and high cardiac output, but no data from neonates or neonatal models exist. The aim of this study was to investigate PhysioFlow® feasibility in asphyxiated newborn piglets. METHODS: Fifteen piglets, under continuous arterial heart rate (HR) and blood pressure (BP) monitoring, were asphyxiated until asystole. Cardiopulmona

Kratom-Associated Acute Right Ventricular Dysfunction: Diagnostic and Management Insights.

BACKGROUND: Kratom (Mitragyna speciosa) is a psychoactive herbal supplement with stimulant and opioid-like properties, with emerging reports linking it to cardiovascular toxicity. However, its association with right ventricular (RV) function remains unclear. CASE SUMMARY: We report the case of a 52-year-old man who presented in shock with hypoxia and multiorgan dysfunction. Echocardiography revealed severe RV dilation with McConnell's sign, whereas chest computed tomography angiography ruled out pulmonary embolism. Right heart catheterization confirmed elevated right-sided pressures and high

Ventilatory control in humans: constraints and limitations.

Below the lactate threshold ((thetaL)), ventilation (V(E))responds in close proportion to CO(2) output to regulate arterial partial pressure of CO(2) (PaCO2). While ventilatory control models have traditionally included proportional feedback (central and carotid chemosensory) and feedforward (central and peripheral neurogenic) elements, the mechanisms involved remain unclear. Regardless, putative control schemes have to accommodate the close dynamic 'coupling' between and V(E) and V(CO2). Above (thetaL), PaCO2 is driven down to constrain the fall of arterial pH by a compensatory hyperventilati

Emergency transcatheter aortic valve replacement in patients with cardiogenic shock due to acutely decompensated aortic stenosis.

AIMS: To assess outcomes of TAVR as a rescue therapy in patients with cardiogenic shock due to acutely decompensated aortic stenosis. METHODS AND RESULTS: Of 771 high-risk patients who underwent TAVR, 27 (3.5%; 78±9 years; 12 men) were treated emergently due to acutely decompensated aortic stenosis with cardiogenic shock. SAPIEN and CoreValve prostheses were implanted in 11 and 16 patients, respectively: the transfemoral access route was used in 25. Three patients died within 72 hours of successful valve deployment, and a further six died within a month, giving a 30-day mortality of 33.3%, wh

Type-D personality and cortisol in survivors of acute coronary syndrome.

OBJECTIVE: To test the hypothesis that Type-D personality is associated with elevated cortisol levels in patients 4 months after an acute coronary syndrome (ACS). METHODS: Salivary cortisol profiles were measured at home in 70 coronary heart disease patients (Mean age = 60.90 years, SD = 10.7, 17% female) 4 months after hospitalization for ACS. Eight saliva samples were taken over the course of 1 day. RESULTS: Thirty eight percent of the ACS patients were defined as Type-D. Cortisol profiles showed a typical diurnal pattern, with low levels in the evening, high levels early in the day. Type-

Renal hemodynamic consequences of angiotensin-converting enzyme inhibition in congestive heart failure.

Angiotensin-converting enzyme (ACE) inhibitors are of benefit in the management of heart failure. In some studies in patients with heart failure, a decline in renal function occurred more frequently in patients treated with enalapril maleate, a longer-acting agent, than in those treated with captopril, a shorter-acting drug. Patients experiencing a decline in renal function had a number of predisposing hormonal and hemodynamic factors. In one report, these factors included an initial fall in blood pressure that was sustained, lower cardiac output, and a relatively high fixed dose of enalapril

Prophylactic Ligature of AV Fistula Prevents High Output Heart Failure after Kidney Transplantation.

BACKGROUND: Arteriovenous (AV) fistulas are considered the gold standard for ensuring safe and long-term vascular access in patients with haemodialysis-dependent end-stage renal disease. However, previous studies demonstrated that high-flow AV fistulas might add additional cardiovascular burden in the post-transplant setting, leading to frequent fistula closure in this population. Currently, there is no consensus regarding management of high-flow fistulas in post-transplant patients with stable kidney function. The present randomized controlled trial examines the effect of prophylactic AV fist

Excessive hypercholesterolemia in pregnancy alters maternal cardiac remodeling during pregnancy and postpartum.

Excessive hypercholesterolemia (eHC) in pregnancy is associated with complications such as preeclampsia, yet its impact on maternal cardiac adaptation and long-term cardiovascular health remains unclear. We hypothesized that eHC impairs maternal cardiac structure and function during pregnancy and that these effects persist postpartum. Sprague Dawley rats were fed either a control diet (CTL) or high-cholesterol diet (eHC) from gestational day (GD) 6-20 (term = 22 days). Echocardiography was performed before pregnancy, at GD20, and 3 mo postpartum. Plasma and left ventricular tissue were collect

Oxygenated right ventricular assist device as part of veno-venopulmonary extracorporeal membrane oxygenation to support the right ventricle and pulmonary vasculature.

COVID-19 infection can lead to severe acute respiratory distress syndrome (ARDS), right ventricular (RV) failure and pulmonary hypertension. Venovenous extracorporeal membrane oxygenation (V-V ECMO) has been used for patients with refractory hypoxemia. More recently dual-lumen right atrium to pulmonary artery oxygenated right ventricular assist devices (Oxy-RVAD) have been utilized in the severe medical refractory COVID ARDS setting. Historically, animal data has demonstrated that high continuous non-pulsatile RVAD flows, leading to unregulated and unprotected circulation through the pulmonary

High-output heart failure in a newborn.

High-output cardiac failure is rare in newborns. Emergent diagnosis and management of this pathology is crucial. We report the case of a child, currently 12-months old; obstetric background is non-contributory. Clinic observation on D1 was normal except for the presence of a systolic cardiac murmur; cardiological evaluation revealed mild ventricular dysfunction of the right ventricle. On the third day of life, she developed cardiac failure with gallop rhythm, hepatomegaly and a murmur in the anterior fontanel; an echocardiogram confirmed clinic aggravation with biventricular dysfunction and ri

Baroreflex Activation Therapy and Subcutaneous ICDs: A Manageable Electrical Mismatch.

BACKGROUND: Baroreflex activation therapy (BAT) improves symptoms and quality of life in patients with heart failure with reduced ejection fraction, while subcutaneous implantable cardioverter-defibrillators (S-ICDs) protect against sudden cardiac death without intravascular hardware. Unipolar BAT stimulation poses a theoretical risk of interference with S-ICD sensing. CASE SUMMARY: A 52-year-old man with heart failure with reduced ejection fraction, end-stage renal disease on dialysis, and an S-ICD underwent BAT implantation. During routine hemodialysis, he experienced ventricular fibrillati

Enabling forward uncertainty quantification and sensitivity analysis in cardiac electrophysiology by reduced order modeling and machine learning.

We present a new, computationally efficient framework to perform forward uncertainty quantification (UQ) in cardiac electrophysiology. We consider the monodomain model to describe the electrical activity in the cardiac tissue, coupled with the Aliev-Panfilov model to characterize the ionic activity through the cell membrane. We address a complete forward UQ pipeline, including both: (i) a variance-based global sensitivity analysis for the selection of the most relevant input parameters, and (ii) a way to perform uncertainty propagation to investigate the impact of intra-subject variability on

Myocardial dysfunction in meningococcal septic shock.

PURPOSE OF REVIEW: The underlying pathophysiology of sepsis has long been disputed. Systemic vasodilatation is important in the development of shock and, in septic critically ill adults who have been volume resuscitated, the systemic pressure is often low and the cardiac output high. In septic children however, and especially in those with meningococcal septic shock, poor cardiac output as a consequence of depressed myocardial function seems to be important, often being the cause of death in these patients. There is much evidence for disturbance of myocardial performance, yet despite the liter

[The cardiac hazard of tocolysis and antagonizing possibilities. II. Communication: protection of the myocardium by means of substitution of magnesium].

In a comparative study the value of the substitution of magnesium for the evaluation of cardiac side-effects of tocolysis has been assessed with regard to haemodynamic as well as to metabolic and morphologic alterations. Haemodynamic studies at the thoracotomized dog using the technique described in the first publication showed significantly less severe haemodynamic alterations in a group with normal magnesium blood levels than in the second group with reduced magnesium blood levels. Significant differences could be found at systolic blood pressure, cardiac output, velocity and acceleration of

[Giant omphalocele correction in a patient with pentalogy of Cantrell].

Pentalogy of Cantrell is a rare congenital malformation characterised by a large defect in the lower region of the sternum, diaphragm, and lower abdominal region. It is also characterised by the migration of organs, and its prognosis depends on the presence of cardiac malformations. We present the case of an 18 year-old male subjected to a scheduled giant omphalocele correction. Invasive monitoring, including cardiac output, was used to avoid episodes of instability due to the increase in abdominal pressure and the increase in venous return (as had happened on two previous operations). The pro
