Long-haul COVID: healthcare utilization and medical expenditures 6 months post-diagnosis.

BACKGROUND: Despite extensive evidence that COVID-19 symptoms may persist for up to a year, their long-term implications for healthcare utilization and costs 6 months post-diagnosis remain relatively unexplored. We examine patient-level association of COVID-19 diagnosis association of COVID-19 diagnosis with average monthly healthcare utilization and medical expenditures for up to 6 months, explore heterogeneity across age groups and determine for how many months post-diagnosis healthcare utilization and costs of COVID-19 patients persist above pre-diagnosis levels. METHODS: This population-b

Coronary Artery Calcification on Prior Non-Gated Chest CT in Patients With Myocardial Infarction: Reporting Rates and Implications for Cardiovascular Risk Stratification.

BACKGROUND: Coronary artery calcification (CAC) is a strong independent predictor of cardiovascular events and can be reliably identified on non-gated thoracic CT (NGTCT). International consensus statements recommend routine reporting of incidental CAC with visual grading and management prompts. However, real-world adherence to these recommendations remains variable, and no data currently exist for New Zealand. METHODS: We conducted a retrospective observational study of patients presenting with acute myocardial infarction (MI) to Tauranga and Whakatāne Hospitals between January 2021 and Dece

18-Month-Old with Lethargy and Accelerated Idioventricular Rhythm in Prehospital Setting: A Case Report.

INTRODUCTION: We report a case of accelerated idioventricular rhythm (AIVR) identified by Emergency Medical Services (EMS) monitoring of an infant presenting with lethargy and respiratory distress. Accelerated idioventricular rhythms are rare ventricular rhythms originating from the His-Purkinje system or ventricular myocytes, consisting of >3 monomorphic beats with gradual onset and termination.1 An AIVR is usually well-tolerated and does not require treatment, though sustained arrythmia may induce syncope, and the rhythm has been seen in newborn infants with congenital heart diseases.1 Monit

Use of eptifibatide as a bridge antiplatelet agent for intrathecal drug delivery system placement.

Use of antiplatelet agents is becoming increasingly common, and their management may require new strategies if neuroaxial techniques are to be employed in patients who will not tolerate discontinuation of antiplatelet therapy. The patient was a 46-year-old man with a past medical history significant for coronary artery disease and who had undergone 14 stents. He developed stent thrombosis (ST) while on clopidogrel. Following the ST, he was subsequently placed on prasugrel. While on prasugrel, the patient presented for an intrathecal drug delivery system (IDDS) trial and placement due to severe

Safety and Efficacy of MA01-100: A Prospective Multicenter Trial of China's First Domestic Implantable Cardiac Monitor.

BACKGROUND: Unexplained syncope and palpitations are common chief complaints in the outpatient department of cardiology. Their sporadic and unpredictable poses significant diagnostic challenges. Implantable cardiac monitors (ICMs) can monitor arrhythmia events efficiently, and can overcome the shortcomings of traditional electrocardiogram (ECG) detection tools such as Holter. However, the high cost of imported ICM devices limits accessibility for patients in China. The MA01-100 (Singular Medical, Suzhou, China), the first domestically developed ICM, may offer a cost-effective alternative. This

Incremental diagnostic and prognostic value of contemporary stress echocardiography in a chest pain unit: mortality and morbidity outcomes from a real-world setting.

BACKGROUND: Clinical assessment often cannot reliably or rapidly risk stratify patients hospitalized with suspected acute coronary syndrome. The real-world clinical value of stress echocardiography (SE) in these patients is unknown. Thus, we undertook this study to assess the feasibility, safety, ability for early triaging, and prediction of hard events of SE incorporated into a chest pain unit for patients admitted with acute chest pain, nondiagnostic ECG, and negative 12-hour troponin. METHODS AND RESULTS: Accordingly, 839 consecutive patients who underwent clinical, ECG, and SE assessments

Frequency and Echocardiographic Correlates of Right Ventricular Dysfunction in Patients With Old Myocardial Infarction and Reduced Ejection Fraction.

BACKGROUND: Old myocardial infarction (MI) with reduced left ventricular ejection fraction (LVEF) is a major cause of cardiovascular morbidity and mortality, particularly in low- and middle-income countries. While left ventricular dysfunction has been extensively studied, right ventricular (RV) involvement remains underappreciated despite evidence of its prognostic relevance in other cohorts. This study aimed to determine the frequency of RV dysfunction and its echocardiographic correlates in patients with old MI and reduced LVEF. METHODS: This was a single-centre, hospital-based, observation
