Giant right atrial aneurysm presenting as right heart failure.

Idiopathic aneurysmal dilatations of the right atrium are rare anomalies. We report one such case of a young man presenting with fatigue, abdominal distension, pedal oedema, unremarkable cardiac examination except for raised jugular venous pressure, an electrocardiogram showing normal sinus rhythm with right bundle-branch block, and an radiograph of the chest showing cardiomegaly. The echocardiographic examination revealed a giant right atrium with low pressure tricuspid regurgitation. The computed tomography confirmed the findings of two-dimensional echocardiography. He was put on medical tre…

Unusual initial presentation of primary systemic (AL) amyloidosis with severe cardiomyopathy and fatal outcome.

Primary (AL) amyloidosis is the most common form of systemic amyloidosis seen in current clinical practice. The symptoms of the disease are usually vague, special features are seen in fewer than one fifth of patients, and the combination of organs and systems involved provides a clue for the diagnosis. We describe a patient in whom asymptomatic hepatomegaly, cardiomegaly, hyperlipidaemia and elevated serum alkaline phosphatase level were found during routine examination; the final diagnosis was primary systemic AL amyloidosis with severe cardiomyopathy, resulting in a fatal outcome within eigh…

Inferior sinus venosus defect associated with incomplete cor triatriatum dexter and patent foramen ovale.

Sinus venosus atrial septal defect (SVD) is a rare cardiac abnormality in adults. Particularly, the inferior type is difficult to depict by transthoracic echocardiography because of its infero-posterior location to the fossa ovalis. We describe the case of a 33-year-old woman whose chest X-ray taken during bronchopneumonia revealed a cardiomegaly. Further echocardiographic investigations showed an underlying inferior SVD, an incomplete cor triatriatum dexter and a large patent foramen ovale (PFO). The diagnosis was confirmed by cardiac magnetic resonance imaging (CMR) and during surgical repai…

Ortner's syndrome in association with mitral valve prolapse.

The case of an 83-year-old woman with a history of hypertension, valvular heart disease, atrial fibrillation, and cardiomegaly is presented. The patient also had progressive hoarseness of her voice and intermittent dysphagia. Ear, nose, and throat examination revealed left vocal cord paralysis. Echocardiography revealed severely dilated left (LA) and right atria (RA), moderate mitral regurgitation, severe tricuspid regurgitation, and prolapse of both these valves. A review of literature of Ortner's or cardiovocal syndrome is presented. Ortner's syndrome due to mitral valve prolapse has not bee…

Extrauterine pelvic arteriovenous malformation mimicking the clinical presentation of structural heart disease.

A case is reported of an extrauterine pelvic arteriovenous malformation involving branches of the internal iliac arteries. Cardiomegaly and a rough cardiac murmur were the clinical presentations mimicking a structural heart disease. A continuous bruit could only be detected by the diaphragm of the stethoscope applied firmly to the left lower abdomen. Multiple blood samplings from inferior vena cava, and iliac and femoral veins for determination of oxygen saturation may be necessary for suspected cases. However, selective arteriography remains the best method for diagnosing the presence, extent…

Bacterial pericarditis in infancy and childhood.

The authors report on a cooperative study of 43 cases of bacterial pericarditis observed in children. This disorder was suspected in patients with septicemia who developed symptoms and signs of pericarditis (precordial pain, muffled heart sounds, pericardial friction rub, cardiomegaly). Early diagnosis of this condition is now facilitated by echocardiography. A combination of medical and surgical treatments (appropriate antibiotic therapy after culture and sensitivity tests and early pericardial drainage) led to complete recovery in almost all of the cases (42 of 43). After long-term follow-up…

[Functional pulmonary atresia: a case report].

A male neonate, two days of age, was admitted with cyanosis and tachypnea. Chest roentgenograms showed massive cardiomegaly and decreased pulmonary vasculatures. Echocardiographic and the right ventriculographic findings were interpreted as showing primary pulmonary atresia with secondary tricuspid incompetence. At a Brock operation, however, there was no resistance in passing the instrument through the pulmonary valve. The baby died twelve hours after surgery. Postmortem examination showed that the tricuspid regurgitation was a cause of the functional pulmonary atresia (FPA). It is important …

The gastric bubble: roentgen observations.

The appearance of the stomach bubble provides many clues to underlying thoracoabdominal disorders. Illustrated in this article are (1) the major complications of large hiatal hernias: intrathoracic gastric volvulus and ulceration; (2) diaphragmatic abnormalities including inversion of the hemidiaphragm and intrathoracic displacement of abdominal contents because of diaphragmatic laceration or congenital herniation; (3) extrinsic displacement of the stomach bubble by splenomegaly (the occurrence of which in association with radiographic abnormalities in the chest--cardiomegaly, interstitial lun…

[Right atrial fibroma simulating cyanotic cardiac vitium in a newborn (author's transl)].

The clinical picture in a cyanotic newborn suggests pulmonary atresia or tricuspid atresia. Angiocardiography shows a right atrial tumor, at necropsy a ping-pong-ball sized fibroma is found. Hypoxia and congestive heart failure occurred and the baby died on his eleventh day of life. Uncertain cardiac symptomatology should always raise the suspicion of a cardiac tumor. Cardiac tumors with predominant intramural growth can show pure cardiomegaly- and/or cardiac arrhythmias, congestive failure, while intracavitary growth may lead to various kinds of in- and outflow obstruction and/or embolisation…

[Enlarged heart caused by massive subepicardial lipomatosis with idiopathic dilatation of the right cavities].

The case is reported of a man of 48 with known longstanding gross cardiomegaly which was completely asymptomatic. Angiocardiography and coronary arteriography showed dilatation of the right side of the heart, and especially of the auricle. In addition, the ventricle was separated from the diphragm by a transparent non-fluid area. At operation, gross dilatation of the right atrium was confirmed, but no causative lesion could be found; there was also marked lipomatosis which involved particularly the area beneath the right ventricle. It is difficult to classify this most unusual case as either a…

The hypertensive diseases. Evidence that systemic hypertension is a greater risk factor to the development of other cardiovascular diseases than previously suspected.

This report summarizes the clinical frequencies of systemic hypertension and necropsy evidence of cardiomegaly in various cardiovascular conditions (Table I), termed "the hypertensive diseases" because of their frequent association with systemic hypertension. Although long recognized as a major risk factor, systemic hypertension appears to be an even greater risk factor to the development of various cardiovascular conditions than previously appreciated. Hypertension by itself appears to be the sole underlying factor in most cases of nontraumatic cerebral arterial or aortic (dissection = partia…

Evaluating adult cor triatriatum with total anomalous pulmonary venous connections by multidetector computed tomography angiography.

A 19-year-old female patient was admitted to our hospital with dyspnea, chest pain, and shortness of breath. A chest radiograph showed mild cardiomegaly. Echocardiography revealed an extra chamber in the heart. To evaluate this abnormality, ECG-gated 16-detector-row computed tomography angiography was performed. Multidetector computed tomography (MDCT), showing cor triatriatum with total anomalous pulmonary venous connections (TAPVC), clearly revealed cardiac and vascular anatomy. ECG-gated cardiac MDCT is a useful tool for detection and characterisation of cor triatriatum and related anomalie…

Clinical and immunohistochemical findings in a case of neonatal Marfan syndrome.

A newborn girl with extreme cardiomegaly discovered by fetal ultrasound after 34 gestational weeks is presented. The girl was delivered through a Caesarean section. After birth, multiple skeletal stigmata and generalized cardiac involvement with abnormal valves and dilated great arteries suggested Marfan syndrome. The girl died at the age of 10 h. The postmortem examinations showed cardiovascular lesions typical of Marfan syndrome. Immunofluorescence studies from cultured fibroblasts of the patient showed decreased amounts of immunostained fibrous material, supporting the clinical diagnosis of…

Prediction of radiographic abnormalities by the use of bag-of-features and convolutional neural networks.

This study evaluated the feasibility of bag-of-features (BOF) and convolutional neural networks (CNN) for computer-aided detection in distinguishing normal from abnormal radiographic findings. Computed thoracic radiographs of dogs were collected. For the purposes of this study, radiographic findings were used to distinguish between normal and abnormal in the following areas: (1) normal cardiac silhouette vs. cardiomegaly, (2) normal lung vs. abnormal lung patterns, (3) normal mediastinal position vs. mediastinal shift, (4) normal pleural space vs. pleural effusion, and (5) normal pleural space…

Diagnosis of absent ductus venosus in a population referred for fetal echocardiography: association with a persistent portosystemic shunt requiring postnatal device occlusion.

OBJECTIVE: The purpose of this series was to assess the incidence, anatomic variants, and implications of an absent ductus venosus (ADV) in patients referred for fetal echocardiography. METHODS: We searched our fetal cardiology database for diagnoses of ADV from May 2003 to December 2006. RESULTS: During the study period, we performed 1328 fetal echocardiographic examinations in 990 fetuses. We found 6 cases of ADV (6/1000). Indications for fetal echocardiography were cardiomegaly, dilated umbilical or systemic veins, and extracardiac abnormalities. We identified 5 anatomic variants of ADV. …
