Morbidity of the conchal cartilage donor site is evaluated in this article. Forty-two patients (from 1984-1994) with 44 donor sites were reviewed retrospectively. In 27 cases an anterior approach was used and in 17 cases a posterior approach was used. Four complications were observed: two cases of delayed healing in the post auricular approach, one case of flattening of the ear after an anterior approach, and one case with an anterior hypertrophic scar was seen. Only one case needed further surgical intervention. The anterior approach is technically easy, safe, and quick to perform but should …

Cartilage matrix protein (CMP) is a non-collagenous component of cartilage with a yet unknown function. In this study we used in situ hybridization to investigate the temporal and spatial distribution of CMP transcripts during human embryonic and early fetal development, and compared it to the pattern of expression observed for collagen types I, II, X, and decorin. The distribution of CMP and collagen type II transcripts followed a similar pattern in the embryonic bone anlage, the fetal growth plate, and the developing vertebral column. Expression was highest in the upper hypertrophic and lowe…

Although the ideal reconstructive material for augmentation rhinoplasty continues to challenge plastic surgeons, there exists no report in the literature that confines the use of irradiated homologous costal cartilage, first reported by Dingman and Grabb in 1961, to dorsal nasal augmentation. The purpose of this paper is to present a retrospective analysis of the author's experience using irradiated homologous costal cartilage in augmentation rhinoplasty. Twenty-seven dorsal nasal augmentations were performed in 24 patients between 16 and 49 years of age with a follow-up ranging from 1 to 27 m…

Articular cartilage contains at least 2 proteoglycans (PGs) which aggregate with hyaluronate: one larger, richer in chondroitin sulphate (CSRPG); the other smaller, relatively richer in keratan sulphate (KSRPG). With maturation the ratio of CSRPG/KSRPG decreases. In order to test the hypothesis that osteoarthritic cartilage contains an increased amount of proteoglycans characteristic of immature cartilage, experimental osteoarthritis (OA) was induced in 11 dogs by transection of the anterior cruciate ligament. Proteoglycan populations were assessed by composite agarose polyacrylamide gel elect…

Perfusion studies in swine with early lesions of osteochondrosis demonstrated that lamellar areas of chondrocyte necrosis within reserve zones of growth areas occurred only in regions of nonperfused articular cartilage. Articular cartilage with a similar anatomical location was perfused in some animals. Occasionally, nonperfused articular cartilage showed vascular alterations within cartilage canals without evidence of significant perivascular or lamellar necrosis. By light microscopy, some vessels within or adjacent to nonperfused articular cartilage had normal morphology; however, ultrastruc…

Osteoarthritis or 'Joint Failure' is a multi-factorial disease with a final common pathway of cartilage degeneration and bone eburnation. The association of arthritic disease and joint degeneration with the deposition of sodium urate crystals in gout and calcium pyrophosphate crystals in pseudogout (chondrocalcinosis) is clinically well established. Electron microscopy coupled with electron probe analysis has revealed the presence of various other calcium phosphate crystals in joint tissues and fluids. We have found three new morphological types of apatite crystals in human articular cartilage…

There is a high incidence of articular cartilage lesions together with other injuries of the knee joint. Single lesions are far more seldom. Difficulties in diagnosing cartilage lesions and the slow tendency toward recovery cause severe problems in treating this type of knee disorder. Elimination of enzymatic and mechanical interference and restoration of normal metabolic processes should be the therapeutic result. The plan of treatment depends on the pattern of the lesion. Conservative procedures or surgical intervention are possible methods of treatment. Both forms of therapy should avoid an…

Survival characteristics of forty-three specimens of living human bone and articular cartilage from the knees of eight renal-transplant donors were studied, using a drop-tower device. Autoradiography and light and scanning electron microscopy revealed no evidence of chondrocyte death or structural damage until stress levels of twenty-five newtons per square millimeter were reached, corresponding to strains on the order of 20 to 30 per cent and involving energy absorption of one millijoule per cubic millimeter. The data for strain rates of 500 and 1000 s-1 suggest that impact loads sufficient t…

BACKGROUND: The adoption of homologous costal cartilage (HCC) in rhinoplasty is hindered by conflicting reports on resorption and warping. Current literature often conflates fresh-frozen and irradiated grafts, obscuring distinct biological profiles. The objective of this systematic review and meta-analysis was to evaluate the clinical and biomechanical outcomes of different preparations of cadaveric costal cartilage compared to autologous rib cartilage in rhinoplasty.
METHODS: A systematic review was conducted per PRISMA 2020 guidelines (PROSPERO ID: CRD42025603758). Studies comparing homolog…

BACKGROUND: The clinical benefits of combining cartilage procedures with high tibial osteotomy (HTO) remain undetermined. This study aimed to evaluate the additional clinical benefits by comparing a combined procedure group with an isolated HTO group, with stratification based on the success of cartilage regeneration.
METHODS: Patients who underwent medial open-wedge HTO from 2010 to 2022 with a minimum 2-year follow-up were retrospectively reviewed. Patients were divided into 2 groups: HTO + combined cartilage procedures (C group) and isolated HTO (I group). C group was further divided into …

Cartilage in the head of sturgeon or salmon has been gaining attention as a rich source of functional chondroitin sulfate (CS) or proteoglycans. Although the cartilage was found in the heads of other bony fishes, the structure of CS and its core protein, especially aggrecan, was not fully investigated. In this study, comprehensive analysis of CS and aggrecan in the head cartilage of 10 bony fishes including sturgeon and salmon was performed. The 4-O-sulfation to 6-O-sulfation ratio (4S/6S ratio; S: sulfate residue) of CS in Perciformes was ≧1.0, while the 4S/6S ratios of CS from sturgeons and …

Congenital anomalies of the nose are rare but may be associated with syndromes affecting craniofacial structures. Herein, we report a case of a congenital absence of lower lateral cartilage seen during an open rhinoplasty in a 23-yr-old lady with no underlying health conditions. Medical and surgical history were unremarkable and there were no evidences conducted of any previous traumatic facial injuries. During physical examination, a significant nostril asymmetry was noted to be present. In addition, cotton test showed no evidence of obstruction. The absence of lower lateral cartilage on the …

The prevalence of diseases that affect the articular cartilage is increasing due to population ageing, but the current treatments are only palliative. One innovative approach to repair cartilage defects is tissue engineering and the use of mesenchymal stem/stromal cells (MSCs). Although the combination of MSCs with biocompatible scaffolds has been extensively investigated, no product is commercially available yet. This could be explained by the lack of mechanical stimulation during in vitro culture and the absence of proper and stable cartilage matrix formation, leading to poor integration aft…

Thrombospondins (TSPs) are widely known as a family of five calcium-binding matricellular proteins. While these proteins belong to the same family, they are encoded by different genes, regulate different cellular functions and are localized to specific regions of the body. TSP-5 or Cartilage Oligomeric Matrix Protein (COMP) is the only TSP that has been associated with skeletal disorders in humans, including pseudoachondroplasia (PSACH) and multiple epiphyseal dysplasia (MED). The pentameric structure of COMP, the evidence that it interacts with multiple cellular proteins, and the recent repor…

OBJECTIVE: In the treatment of unilateral paralytic dysphonia, traditional arytenoid adduction is designed to place suture through the muscular process of the arytenoid attaching anteriorly to the thyroid ala. In contrast with the suture direction of this technique, a new paramedian approach to arytenoid adduction anchors anteroinferiorly to the cricoid cartilage, mimicking the force action of the lateral cricoarytenoid muscle (the major adductor of the larynx). This study investigated the influence of these changes in suture direction on the vocal fold level as well as the vocal outcomes in t…

Cartilage microfracturation is a surgical technique specifically designed to address chondral defects, which are injuries to the cartilage that covers the ends of bones in joints. These defects can result from traumatic injuries, degenerative conditions such as osteoarthritis, or congenital abnormalities. The primary objective of microfracture surgery is to promote the regeneration of functional cartilage tissue, thereby restoring joint function, alleviating pain, and enhancing mobility. The procedure involves creating small, controlled perforations, or microfractures, in the subchondral bone …
