Pregnancy and puerperium are high susceptibility periods for the development of catastrophic antiphospholipid syndrome.

It is well known that antiphospholipid syndrome (APS) is associated with recurrent pregnancies losses, but is also associated with other obstetric features such as preeclampsia, uteroplacental insufficiency and preterm birth. Pregnancy is a hypercoagulable state than can be complicated by thrombosis, especially in those patients with an underlying thrombophilic disorder. Catastrophic APS is a rare form of presentation of the APS. Several trigger factors have been related with the catastrophic APS, including infections, anticoagulation withdrawal, surgery, neoplasms and lupus "flares". In aroun

Case studies in acute aortic dissection: strategies to avoid a catastrophic outcome.

Aortic dissection is a catastrophic illness that is a significant source of liability for hospitals if diagnosis and treatment are not done promptly. The diagnosis is often difficult to make because not all dissections have the typical presentation of sudden severe chest pain radiating to the back. Symptoms often include abdominal pain, flu-like complaints, vomiting and diarrhea, low back pain, stroke syndromes and syncope. Patients at risk include those with Marfan syndrome and other connective tissue diseases, familial aortic disease, age and hypertension. Aortic dissection is a different cl

Catastrophic presentation of mitochondrial disease due to a mutation in the tRNA(His) gene.

The authors describe a patient who presented with headache, seizures, and severe cerebral edema in whom they identified a novel mutation in the mitochondrial (mt-) tRNA(His) gene. This G12147A transition is heteroplasmic, predicted to disrupt a highly conserved base pair, and segregates with the cytochrome c oxidase deficiency in single muscle fibers.

[Clinical, neuro-radiological and prognostic aspects of post-encephalitic catastrophic epilepsies].

OBJECTIVE: To determine the prevalence of encephalitis and meningo encephalitis as the causative agents of catastrophic epilepsies (CE) and the incidence of post encephalitic CE, when catastrophic epilepsy is defined as often refractory to treatment and always associated with psychoneurological deterioration. PATIENTS AND METHODS: The prevalence of central nervous system (CNS) infections in determining West s syndrome (WS), Lennox Gastaut syndrome (LGS) and HHE syndrome (HHES) was detected in the large series published since 1980 in which the cause was stated. The incidence of CE in the cours

[Intraalveolar hemorrhage as a cause of acute respiratory failure in catastrophic antiphospholipid syndrome].

We report the case of a catastrophic antiphospholipid syndrome (CAPS), which developed without a known cause in a 38-year old man. Primary antiphospholipid syndrome was diagnosed one year earlier. Clinical manifestations of CAPS were dominated by diffuse intraalveolar haemorrhage leading to acute respiratory failure and renal failure. Syndrome of multiorgan failure developed despite the treatment with plasmapheresis, high doses of glycocorticosteroids, an intravenous gammaglobulins. The patient died 6 weeks after the admission.

Fatal vascular catastrophe in Ehlers-Danlos syndrome: a case report and review.

The Ehlers-Danlos syndrome (EDS) is a group of hereditary connective tissue disorders that are characterized by abnormalities of the skin, joints, and a diversity of other phenotypic manifestations. An awareness of the disease is vital for optimal outcome in this rare group of patients who may present with a variety of life-threatening illnesses. Ehlers-Danlos type IV has been associated with vascular catastrophes, perforated viscous, ruptured uterus, and pneumothorax (1-4). We present a case of aneurysmal formation and spontaneous rupture of the great vessels in a 15-year-old male with EDS ty

The repetitive vascular catastrophes of Behçet's disease: a case report with review of the literature.

Behçet's disease is a chronic, recurrent disease that most commonly presents as oral and genital ulcers and iritis. It is a multisystem disease with possible cardiovascular, pulmonary, neurologic, cutaneous and articular involvement. It is most often diagnosed in the 20 to 40 year age group, and vascular and neurologic involvement worsens the prognosis. Vascular manifestations include venous and arterial occlusions, arterial aneurysm and pseudoaneurysm formation. We present the case of a young man with Behçet's disease with multiple aneurysms, pseudoaneurysms, arterial occlusions and venous th

Management of catastrophic neonatal midgut volvulus with a silo and second-look laparotomy.

The case of a neonate with midgut volvulus and severe intestinal ischemia extending from the ligament of Treitz to the midtransverse colon is presented. Management consisted of abdominal silo application at the initial exploration to obviate the adverse physiological consequences of increased intraabdominal pressure from reperfusion edema of the intestine. The majority of the intestine was salvaged, and a short segment of ileum was resected. This method of treatment resulted in optimal ventilatory status, renal function, and cardiac performance. The advantages of temporary prosthetic wound cov

Catastrophic death losses in a dairy herd attributed to type D botulism.

Clostridium botulinum type D intoxication was diagnosed as the cause of death of 42 of 67 lactating cows in a southeast Texas dairy herd over an 11-day period. By necessity, the diagnosis was based on clinicopathologic findings, as the toxin could not, by standard laboratory tests, be demonstrated in affected cattle. The predominant clinical findings were hindlimb weakness/ataxia rapidly progressing to persistent recumbency. Affected cattle were alert until just before death, which occurred without notable agonal movements or respirations after 6 to 72 hours' recumbency. Abnormal laboratory fi

Catastrophic health expenditure associated to bellwether surgery care in Northcentral Ethiopia.

BACKGROUND: Bellwether procedures-caesarean section, laparotomy, and open fracture management- are core indicators for monitoring safe, equitable, and affordable surgical care in a country. Although effective surgical care is essential for achieving universal health coverage and the Sustainable Development Goals, spending on surgical care leading catastrophic health expenditure among surgical patients, particularly low-income and middle-income countries (LMICs). This study assesses catastrophic health expenditure (CHE) associated with bellwether surgery to quantify financial risk and inform fu

Catastrophizing versus positive reappraisal: The cognitive depletion effects of workplace incivility.

As a low-intensity stressor with ambiguous intent, workplace incivility has sparked debate regarding its real impact. A key unresolved question is how daily incivility elicits specific cognitive processes that lead to sustained exhaustion over time. This study examines cognitive depletion-a critical state linked to impaired job performance-by investigating its relationship with workplace incivility and the mediating roles of catastrophizing and positive reappraisal across different time frames. Using a hybrid methodological approach that combines a five-day daily diary study with a three-wave

Age, Financial Hardship, and Catastrophic Health Expenditure After Emergency Surgery.

INTRODUCTION: Financial hardship (FH), defined as the adverse effects of medical costs on patients, is common among trauma and emergency surgery populations. Although government-sponsored insurance programs aim to financially protect older adults, this population remains at risk due to lower incomes and higher health care utilization. We evaluated the association between age and FH among emergency surgery patients. METHODS: We analyzed adults admitted through the emergency department for surgery using the Medical Expenditure Panel Survey (2017-2022). Age was the primary exposure, with older a

Phloem Catastrophe: Identifying Plant Tipping Points Through Bifurcation Analysis.

This chapter presents a mathematical framework to identify conditions leading to phloem failure using bifurcation analysis and catastrophe theory. The approach assumes that at equilibrium, sucrose production through leaf photosynthesis is balanced by sucrose transport within the phloem. Leaf photosynthesis is modeled according to stomatal optimality theories, which propose that stomatal opening adjusts to maximize carbon uptake while being constrained by resource availability (usually water per unit leaf area in the rooting zone). Sucrose transport in the phloem vasculature is described by the

Gluten Catastrophe: A Fatal Case of Refractory Celiac Disease Type 2.

Celiac disease (CeD) is an immune-mediated enteropathy triggered by gluten, while refractory CeD (RCD) represents a severe form with persistent villous atrophy despite adherence to a gluten-free diet. RCD type 2 (RCD2) is characterized by aberrant clonal intraepithelial lymphocytes and carries a poor prognosis. We report a fatal case of RCD2 in a 55-year-old man who initially presented with abdominal pain, weight loss, and chronic diarrhea. Despite negative celiac serologies, duodenal biopsies demonstrated villous atrophy, and human leukocyte antigen (HLA)-DQ2 positivity supported CeD. He deve

Psychological Burden of Rheumatoid Arthritis in Bangladesh: The Influence of Pain Catastrophizing and Depression.

Depression is a common issue among individuals with RA and is often linked to increased pain levels, greater disability and poorer overall health outcomes. Pain catastrophizing, which is frequently observed in people experiencing chronic pain, tends to worsen both physical discomfort and emotional distress related to rheumatic conditions. Owing to its significant impact, it has become a primary focus in recent treatment approaches. This cross-sectional study was conducted at the Rheumatology Department of Bangladesh Medical University in Dhaka. Adults aged 18 years or older who met the 2010 AC

Distal femur replacement after catastrophic failure of a total knee arthroplasty.

Catastrophic implant failure is a rare complication after total knee arthroplasty (TKA) that can lead to metallosis and pseudotumour formation requiring complex revision surgery. This case report describes a man in his 60s who presented with progressive knee pain, a squeaking implant and ligamentous instability 20 years post-primary TKA. Imaging demonstrated polyethylene failure, tibial baseplate breakage, extensive osteolysis and metallosis. The patient underwent a revision procedure with a distal femur replacement (DFR). Postoperatively he had no complications, re-admissions or revision surg

Coil Catastrophe Averted: A Technical Note on Retrieval From the Basilar Artery.

Early treatment of vertebral artery dissecting aneurysms (VADA) is associated with improved outcomes. Endovascular treatment options utilising both deconstructive and reconstructive techniques have been used. Coil migration may occur during parent vessel occlusion as a rare complication that can lead to further thromboembolic events. Various strategies for coil retrieval have been documented; however, management currently relies on the discretion of the proceduralist, as there is no established consensus on a standardised salvage technique. This technical note describes a coil embolisation par

Catastrophic Outcome of Pregnancy Complicated With Tricuspid Valve Nonbacterial Thrombotic Endocarditis.

Nonbacterial thrombotic endocarditis (NBTE) is a rare condition characterized by sterile fibrin-platelet vegetations on cardiac valves, typically associated with hypercoagulable states. Isolated tricuspid valve (TV) involvement during pregnancy is exceedingly rare, with no previously reported cases of fatal maternal and fetal outcome in the published literature. We report a 31-year-old woman (gravida 3) with a complex medical history including human immunodeficiency virus infection, chronic kidney disease, recurrent venous thromboembolism, and thrombocytopenia, who presented at 9 weeks of gest

Aortic perforation from central venous catheterization: a near-miss catastrophe.

Central venous catheterization for hemodialysis access carries a risk of inadvertent arterial cannulation, which can lead to catastrophic complications, including aortic arch perforation. We report the case of a 61-year-old man with stage 5 chronic kidney disease who sustained an aortic perforation during an attempted right internal jugular vein cannulation at an outside institution. Computed tomography angiography revealed the catheter traversing the right common carotid artery and brachiocephalic trunk, with an extraluminal tip located in the aortic arch, accompanied by hemopericardium and a
